Comparative study of using different levels of positive endexpiratory pressure with dexmedetomidinebased sedation regimen after offpump coronary artery bypass grafting

Abstract: ] Objective Toevaluatetheuseofdexmedetomidine-basedsedationregimenon
differentlevelsofpositiveend-expiratorypressure ( PEEP ) afteroff-pumpcoronaryarterybypass
grafting ( OPCABG ) .Methods Thisisanopenprospectivestudy with60consecutiveadult
patientsafterOPCABGontheuseofdexmedetomidine-basedsedationregimenundervolume-
controlledmechanicalventilation ( VCV ) withdifferentPEEP , dividedintothreegroups ( Group
A : PEEP= 0cmH 2 O , GroupB : PEEP= 5cmH 2 O , GroupC : PEEP= 10cmH 2 O ) .Results
Fromthepointofweaningfrom mechanicalventilationuntil12hoursafterOPCABG , compared
withGroupAandB , theapplieddoseofdexmedetomidinewasincreased ( P <0.05 ), average
mechanicalventilationwasshorten ( P <0.05 ), durationinICUandhospitalwasshorten ( P <
0.05 ), andPaO 2 atpostoperative12hand24hwasincreased ( P <0.05 ) inGroupC.Conclusion
PEEPat10cmH 2 Ocansignificantlyimproveprognosis.HigherlevelofPEEPrequired more
dexmedetomidine , but the patients were associated with shorter duration of mechanical
ventilationinpostoperativeOPCABGpatients.
