Effects of Helium-Oxygen Mechanical Ventilationin in patients with acute exacerbation of chronic obstructive pulmonary disease

Abstract ] Objective Toinvestigatetheeffectofhelium-oxygenmechanicalventilationon
peakairwaypressure ( Ppeak ), partialpressureofcarbondioxide ( PaCO 2 ), partialpressureof
oxygen ( PaO 2 ), Interleukin8 ( IL-8 ) andtumornecrosisfactorα ( TNF-α ) inpatientswithacute
exacerbationofchronicobstructivepulmonarydisease ( AECOPD ) .Methods Sixtycasesof
mechanicalventilationwereselectedandrandomlydividedintotheexperimentalgroupandthe
controlgroup , with30casesineachgroup.Allpatients weretreated with modifiedclosed
ventilator.Experimentalgroup wastreated with helium-oxygen mechanicalventilation ( 60%
helium vs 40%oxygen ) .Controlgroupwastreatedwithnitrogen-oxygenmechanicalventilation
(
60% nitrogen vs 40% oxygen ), ThechangesofPpeak , PaO 2 , PaCO 2 wereobservedbefore treatment , 30min , 90min , 120minaftertreatment.Enzymelinkedimmunosorbentassaywere
usedtodetectserumconcentrationsofIL-8andTNF-αatfourtimepointsin2groupsofpatients.
Results ThePpeakandPaCO 2 inexperimentalgroupweredecreasedsignificantly30minafter
treatmentandthenremainedstability.Butthecontrolgroupdidnotimprovedsignificantly.The
PaO 2 inexperimentalgroupwasincreasedsignificantly30minaftertreatmentandthenremained
stability.Butthe controlgroup did notimproved significantly.TheIL-8 and TNF-αin
experimentalgroupdecreasedsignificantly120minaftertreatment , whilethecontrolgroupdid
notimprovedsignificantly.Conclusion Helium-oxygen mechanicalventilationcansignificantly
reduceAECOPD patientsairway pressure , improveoxygenationandreducecarbon dioxide
retention.Whenthetreatmentreachesacertaintime , theheliumoxygenmechanicalventilation
canreduceconcentrationsofIL-8andTNF-α , whichhasacertainregulatingeffectonchronic
airwayinflammation.Atthesametime , Theimprovedventilatorcanbeachievedusingthe
heliumcycle , whichgreatlysavesthecostoftreatment.
