A comparative study of applying TSPOT.TB and TST in the diagnosis of LTBI in TB professionals

Abstract ] Objective ToinvestigatethevalueofT-SPOT.TBinthediagnosisoflatent
tuberculousinfection ( LTBI ) inTBprofessionals.Methods Eight-fivecasesofTBmedicalstaff
asTBprofessionalswereselected.Eight-fivecasesofhealthypeoplewereselectedforcontrol
group , allwithoutpreviousTBhistoryorsymptomsand withnormalchestfilm.Apply T-
SPOT.TBwasdetectedbythespecificinterferon γ ( IFN-γ ) leveloftheperipheralblood.
Tuberculinskintest ( TST ) wasusedalso.TBbacilluswhilecarryingoutparallelTSTtests.
Results T-SPOT.TBresultsshowthatthedifferencebetweenthestudygroupandthecontrol
groupwasstatisticallysignificant.TSTindicatesthatthereisnostatisticdifferencebetweenthe2
groups.Thepositiveratecomparisonofthetwomethodsshowsthatthereisstatisticdifference
betweenthe2groups ( P <0.05 ) .Consistencycheckindicatesthatthereispoorconsistency
betweenthetwotest methods ( Kappa=0.123 ) .Conclusion ①Closelycontacting with TB
infectedpatientsisariskfactorwhichleadstoMTBinfection.Peoplewithclosecontacthavea
higherrateofbecomingLTBIpatientsthanthosewithoutclosecontact.②T-SPOT.TBpositive
resultsarepositivelycorrelatedwiththosewhocloselycontactwithTBinfectedpatients , while TSTpositiveresultsarenotcorrelated , probablyduetoaveryhighBCGvaccinationratein
China.③T-SPOT.TBhasahigherspecificitythanTSTinLTBIdiagnosis.④T-SPOT.TBhasa
lowcorrelationwithTSTandservesasamoreaccuratemethodofLTBIdiagnosis.
