Abstract: ABSTRACT:Objective To investigate the high-risk human papilloma virus (HPV)test in atypical squamous cell of undetermined significance (ASCUS).Methods A total of 870 patients with ASCUS tested by cytology were performed with HR-HPV test,colposcopy and cervical biopsies.Results In 870 ASCUS patients,there were 602 cases with chronic cervical inflammation,124 cases with cervical intraepithelial neoplasia (CIN)Ⅰ,70 cases with CIN Ⅱ,59 cases with CIN Ⅲ and 15 cases with cervical cancer.In HPV positive group with 350 cases,there were 128 cases with high-grade lesion or worse condition (36.57%).In HPV negative group with 520 cases,there were 16 cases with high-grade lesion (3.08%) and none with cancer.The frequency of high-grade lesion in HPV positive group and negative group was 36.57% and 3.08%, the difference was significant between the two groups (P < 0.05 ).Based on the pathological diagnosis,the sensitivity of HPV in detecting high-grade lesion or worse condition was 88.89%, the specificity 69.42%,the positive predictive value 36.57%,and the negative predictive value 96.92%.All patients were divided by age,there were significant difference in HPV infection rates among groups(P <0.05).The incidences of high-grade lesion in different groups were similar.In the group of women over 60 years old,the incidence of high-grade lesion between HPV positive group and negative group showed no statistic difference (P > 0.05 ).In the other groups,the difference was significant.For women over 60 years old,the sensitivity and specificity of HPV test were lower.In the other group,the differences of sensitivities were not significant and the specificity raised with age.Conclusion Among patients of ASCUS,the incidence of cervical precancerous lesion are getting younger,it is necessary to make further examination or follow-up for those less than 30 years old,HPV test is useful and accurate to distribute patients of ASCUS less than 60 years old.
