摘要: [摘要] 目的 探讨胎儿脐动脉与大脑中动脉阻力指标在胎儿缺氧预测中的应用价值。方法 选取妊娠孕妇
120 例,其中正常妊娠者 62 例为对照组,胎儿缺氧妊娠者 58 例为观察组,应用彩色多普勒超声检测仪检测胎儿脐
动脉、大脑中动脉的搏动指数(
pulsatilityindex , PI )、阻力指数( resistanceindex , RI )以及收缩期峰值和舒张末期流
速之比(
peaksystolicvelocity
/ enddiastolicvelocity ,
S / D ),计算比较脐动脉、大脑中动脉的各项阻力指标比值。结
果 观察组胎儿脐动脉阻力指标均明显高于对照组( P <0.01 ),大脑中动脉阻力指标均明显低于对照组(
P <
0.01 );胎儿脐动脉、大脑中动脉各项阻力指标比值 PI 脐动脉/大脑中动脉 、 RI 脐动脉/大脑中动脉 及 S / D 脐动脉/大脑中动脉 均高于对照组,
差异有统计学意义( P <0.01 ),并以 PI 脐动脉/大脑中动脉 >1.
00 、 RI 脐动脉/大脑中动脉 >1. 00 和 S / D 脐动脉/大脑中动脉 >0. 72 作为临界
值,预测胎儿缺氧的敏感度、特异度及 Youden 指数均较高。结论 胎儿脐动脉、大脑中动脉的各项阻力指标及其比
值在胎儿缺氧预测中具有重要价值,能够预测胎儿缺氧以及其他不良的妊娠现象,应及早发现异常并指导临床干
预,从而降低围产期新生儿病死率,提高新生儿质量。

Abstract ] Objective Todiscusstheapplicationvalueoffetalumbilicalarteryandmiddle

womeninhospitalwasselectedanddividedintocontrolgroupandobservationgroup , thecontrol

abnormalitiestoreduceneonatalmortalityrate , guidetheclinicalintervention , improvethe
qualityofnewbornbirth.
