ω-3脂肪酸肠内营养制剂对神经重症患者C反应蛋白和营养代谢的影响

摘要: [摘要] 目的 ω-3 脂肪酸肠内营养制剂对缓解神经重症 C 反应蛋白和营养状态的影响。方法 选择神经重症
患者 40 例配对设计分为试验组和对照组各 20 例。试验组给予 ω-3 脂肪酸肠内营养,对照组给予普通肠内营养,在
治疗前和治疗后 1d 、 4d 、 7d 、 14d 测量 2 组患者 C 反应蛋白、白蛋白、前白蛋白、转铁蛋白水平。结果 治疗后 2 组
C 反应蛋白水平先升高后降低,试验组低于对照组,白蛋白水平试验组先降低后升高,对照组降低后处于平台期,
治疗后 7d 试验组白蛋白高于对照组,组间、不同时点间、组间·不同时点间交互作用差异均有统计学意义( P <
0.05 )。前白蛋白、转铁蛋白水平持续低于治疗前,治疗后 4d 试验组水平高于对照组,组间、不同时点间差异有统
计学意义( P <0.05 ),但组间·不同时点间交互作用差异无统计学意义(
P >0.05 )。结论 ω-3 脂肪酸肠内营养能
明显降低神经重症患者 C 反应蛋白水平,在提升营养状态、缓解负氮平衡方面优于普通营养。

Abstract ] Objective Toobservetheeffectsofenteralnutritionω-3fattyacidonreducing
thelevelofC-reactiveproteinandimprovingnutritional metabolisminsevereneurosurgical
patients.Methods The patients who had severe neurosurgery disease were dividedinto
experimentalgroupandcontrolgroupbypaireddesignmethod.Eachgroupwasconsistedof20
patients.Theexperimentalgroupistreatedwithω-3fattyacidsenteralnutrition , thecontrol
groupistreatedwithtraditionalenteralnutrition.Beforetreatmentandaftertreatmentatday1 ,
day4 , day7 , day14 , C-reactiveprotein , albumin , prealbuminandtransferrinlevels were
measuredin2groups.Results Afternutritionaltherapy , thelevelofC-reactiveproteinintwo
groupswererisingfirstthengoingdownlater.Thelevelinexperimentalgroupwaslowerthan
thatincontrolgroup.Thelevelofalbumininexperimentalgroupwasgoingdownfirstandrising
slowlylater , thelevelincontrolgroupwasgoingdownandneverrisingagainin2weeks , andthe
levelinexperimentalwashigherthanlevelincontrolgroupafter1week , Thereweresignificant
differencesintheinteractionofthetimepointsbetweenthetwogroups , thegroups , andthe
groupsvstimepoints ( P <0.05 ) .Thelevelofprealbuminandtransferrinwerelowerthanthat
beforenutritionaltherapy , thelevelinexperimentalgroupwashigherthanthatincontrolgroup inthefirst4daysoftreatment.Thereweresignificantdifferencesinthetimepointsandgroups

P <0.05 ) .Therewerenotsignificantdifferencesintheinteractionofthetimepointsvstime
points ( P >0.05 ) .Conclusion ω-3Fattyacidsenteralnutritioncanobviouslyreducethelevelof
C-reactiveproteinandimprovenutritionalmetabolisminsevereneurosurgicalpatients , which
wassuperiortogeneralnutritioninimprovingthenutritionalstatusandalleviatingthenegative
nitrogenbalance.
