乳腺癌FDG摄取与VEGF、p53表达在淋巴结转移中的临床意义

〖HTH〗目的〖HTSS〗〖KG*2〗探讨乳腺癌脱氧葡萄糖(fluorodeoxyglucose,FDG)摄取与血管内皮生长因子(vascular endothelial growth factor,VEGF)、p53表达在淋巴结转移中的关系。
〖HTH〗方法〖HTSS〗〖KG*2〗选取经手术证实的乳腺癌患者92例,术前均进行正电子发射型计算机断层显像/CT全身检查,并记录所有肿瘤组织的最大标准化摄取值(maximal standard uptake value,SUVmax)。术后切除的乳腺癌组织均进行VEGF和p53表达检测,计算不同临床病理特征乳腺癌组织的VEGF和p53表达阳性率。分析乳腺癌FDG摄取与VEGF、p53表达在淋巴结转移中的关系。
〖HTH〗结果〖HTSS〗〖KG*2〗有淋巴结转移的乳腺癌组织中VEGF和p53的阳性表达率明显高于无淋巴结转移的乳腺癌组织(P<005)。有淋巴结转移的乳腺癌组织中VEGF与p53的表达有相关性(rs=0.563,P=0.000),无淋巴结转移的乳腺癌组织中VEGF与p53的表达有相关性(rs=0.430,P=0.000)。随VEGF和p53阳性表达增强,乳腺癌组织摄取FDG的SUVmax逐渐增高(P<005)。在乳腺癌组织中,SUVmax与VEGF、p53表达呈正相关(rs=0.730,P=0.019,rs=0.610,P=0.035)。
〖HTH〗结论〖HTSS〗〖KG*2〗VEGF、p53表达与乳腺癌淋巴结转移有关,两者可能有协同作用,乳腺癌FDG摄取可反映淋巴结的生成及肿瘤的侵袭能力,并反映肿瘤更多的生物学特性。

Abstract: [Abstract]  Objective〖HTSS〗〓To evaluate the relationship between the uptake of fluorodeoxyglucose(FDG) and the expression of vascular endothelial growth factor(VEGF) as well as  p53 in breast cancer with lymph node metastasis.
〖HTH〗〖WTHZ〗Methods〖HTSS〗〓The selected 92 cases with breast cancer had been  performed positron emission computed tomography before operation,and recorded the maximal standard uptake value. The expression of VEGF or p53 of breast cancer tissue were tested after operation, and the positive expression rates of VEGF or p53were calculated. The relationship were analyzed between the uptake of FDG and the expression of VEGF and p53 in breast cancer with lymph node metastasis.
〖HTH〗〖WTHZ〗Results〖HTSS〗〓The positive expression rates of p53 and VEGF with lymph node metastasis were obviously higher than that without lymph node metastasis in breast cancer(P<0.05). The expression of VEGF was correlated with the expression of p53 in breast cancer tissue with lymph node metastasis(rs=0.563, P=0.000). The expression of VEGF was correlated with the expression of p53 in breast cancer tissue with no lymph node metastasis(rs=0.430, P=0.000). When the positive expression of VEGF or p53 became stronger, the SUVmax values was increased gradually(P<0.05). The SUVmax value showed positive correlation with the expression of VEGF or p53 in breast cancer tissue(rs=0.730, P=0.019, rs=0.610, P=0.035).
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗〓The  expressions of VEGF and p53 are associated with lymph node metastasis of breast cancer, and the SUVmax values are related to the expressions intensity of VEGF and p53. Maybe there is a synergistic effect between VEGF and p53. And the uptake of FDG maybe reflect the ability of lymph node metastasis and tumor invasion. It provides the basis for further clinical guidance for rational treatment.
