3D打印种植手术导板对前牙即刻种植手术精度影响的研究

〖HTH〗目的〖HTSS〗〖KG*2〗对比研究前牙即刻种植手术中3D打印种植手术导板辅助法和常规手术法在手术精度上的差别。
〖HTH〗方法〖HTSS〗〖KG*2〗选取进行前牙即刻种植的患者40例,分为导板辅助组和非导板辅助组各20例,导板辅助组术前进行 CT 扫描,在3D打印种植导板辅助下共植入 20 枚Dentist骨水平种植体,术后再次行 CT 扫描。非导板辅助组术前拍摄锥形束CT(cone beam  CT,CBCT),手术时由种植医师目测决定种植体窝洞预备的方向和角度,共植入Dentist骨水平种植体20枚,术后拍摄CBCT。将2组术前模拟种植体位置和术后实际种植体位置匹配,用Mimics软件分别测量2组实际种植体位置与术前预定种植体位置和角度的差别。
〖HTH〗结果〖HTSS〗〖KG*2〗导板辅助组颈部近远中向偏差距离、颈部颊舌向偏差距离、颈部垂直向偏差距离均较非导板辅助组小,差异有统计学意义(P<005);导板辅助组近远中向角度偏离值、颈部颊舌向偏差距离和颈部垂直向偏差距离均较非导板辅助组小,差异有统计学意义(P<005)。
〖HTH〗结论〖HTSS〗〖KG*2〗3D打印种植手术导板可降低种植体植入的位置和角度误差,提高种植体植入的位置精准度。

Abstract: [Abstract] Objective〖HTSS〗〓To compare the differences of surgical precision between the auxiliary method of 3d printing implant template and the conventional method in immediate implantation of anterior teeth.
〖HTH〗〖WTHZ〗Methods〖HTSS〗〓Forty patients of immediate implantation in anterior teeth were selected and divided into two groups: template auxiliary and nontemplate auxiliary, with respectively 20 cases of the each group. Patients in the group of template auxiliary were taken CT scan before operation. Twenty dentistimplants of bone level with the auxiliary of 3D printing implant template are implanted and CT scan were followed once again after the operation. As for patients in the group of nontemplate auxiliary, the cone beam CT(CBCT) would be taken before operation and 20 dentist implants of bone level were planted after determining the prepared directions and angles of the cavity by doctors′ visual measurement, with CBCT following then. The implant positions of preoperative simulation and postoperative reality in both groups need to be matched. While the differences between the actual and expected positions and angles of the implants in both groups were measured respectively by the application of Mimic.
〖HTH〗〖WTHZ〗Results〖HTSS〗〓The deviation distances of neck mesialdistal, buccolingual and vertical directions in the group of template auxiliary were smaller than those in the group of nontemplate auxiliary, which had the statistical significance. The deviation angles of the mesialdistal, the deviation distances of neck buccolingual and vertical directions in the group of template auxiliary were smaller than those in the group of nontemplate auxiliary,which had the statistical significance.
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗〓3D printing implant template could reduce the deviations from the positions and angles of implantation so as to improve its position precision.
