Analysis of risk factors of contrastinduced nephropathy in patients undergoing coronary angiography or nonemergent percutaneous coronary intervention

Abstract ] Objective Toobservetherenalfunctionalvariancebeforeandaftercoronary
angiography ( CAG ) ornon-emergentpercutaneouscoronaryintervention (
PCI ) inpatientswith
unstableangina , todeterminetheincidencerateofcontrast-inducednephropathy ( CIN ) andto
discusstheriskfactors.Methods Fivehundredpatientswithunstableanginaenrolledintothe
study.CystatinC ( CysC ), serumcreatinine ( SCr ) andestimatedglomerularfiltrationrate ( eGFR )
werecollectedbeforeandafterCAGornon-emergentPCIatdifferenttimepointsonthebasisof
thetreatmentofhydrationtoestimatetheincidenceofCIN.ThepatientsweredividedintoCIN
groupandnon-CINgroup , andthechangesofrenalfunctionbeforeandafteroperationingroup
CINwereevaluated.Theclinicalcharacteristicsofthetwogroupswerecompared , andtherisk
factorsoftheoccurrenceofCIN wereanalyzedbyCINriskscore ( Mehranscoringsystem ) .
Results CINoccurredin72of500patients , andtheincidencewas14. 4%.Thefactorsofage ,
baselineCysClevel , baselineSCrlevel , thedosageofcontrastmedium , hypertensionhistory diabetichistory , severityofcoronarylesions , MehranscoresofCINgroupwerehigherthanthat
ofnon-CIN group , andbaselineeGFRlevelwerelowerthanthatofnon-CIN group.The
differencewasstatisticallysignificant ( P <0.05 ) .ThelevelofCysCandSCrinpatientswithCIN
afteroperationshowedatrendoffirstincreasingandthendecreasingandtheeGFRlevelwas
decreasedandthenincreased.Thedifferencewasstatisticallysignificant ( P <0.05 ) .Theresults
ofmultivariateLogisticregressionanalysisshowedthat : baselineCysClevel , baselineeGFR
level , diabetichistory , thedosageofcontrastmedium , andMehranscoresweretheriskfactors
ofCIN.Conclusion TheincidenceofCINisstillhigh , even withthepreventiveuseofthe
treatmentofhydration.ThedetectionratewillbeincreasedifCysCandSCrarecombined.
Diabeteshistory , baselinerenaldysfunction , thedosageofcontrastmediumandMehranscoreare
independentriskfactorsforCIN , andthemostrelevantriskfactorisMehranscore , whichcanbe
appliedbeforeoperation.
