摘要: [摘要] 目的 观察右美托咪定对单肺通气( one-lungventilation , OLV )患者肺组织核因子 κB ( nuclearfactor-
κB , NF-κB )表达及血浆肿瘤坏死因子 α ( tumornecrosisfactor , TNF-α )和细胞间黏附分子 1 ( intercellularadhesion
molecule-1 , ICAM-1 )水平的影响,探讨右美托咪定对 OLV 患者肺保护作用的机制。方法 择期行肺癌根治术患者
40 例,采用随机数字表法分为对照组和右美托咪定组各 20 例 。麻醉诱导 10min 后右美托咪定组经静脉输注右美
托咪定 1 μ g /
kg ,随后以
0. 5 μ g ·
kg
-1 · h -1 的速率输注至手术结束前 30min ;对照组采用同样的方法静脉输注等
容量的生理盐水。于 OLV 前即刻( T 1 )、 OLV60min ( T 2 )、恢复双肺通气后 30min ( T 3 )、术后 2h ( T 4 )和术后 24h
( T 5 )取桡动脉血,测定血气指标计算氧合指数(
oxygenationindex , OI )、呼吸指数( respiratoryindex , RI )及动脉肺泡
氧分压比( PaO 2 / PAO 2 ),测定血浆 TNF-α 和 ICAM-1 浓度。于肺叶离体时在距肿瘤 >5cm 处取肺组织,测定
NF-
κB 的表达。结果 2 组 OI 和 PaO 2 / PAO 2 在 T 2 ~T 4 时较 T 1 时降低,右美托咪定组在各时间点均高于对照组( P <
0.05 )。 2 组 RI 值在 T 2 ~T 4 时较 T 1 时升高,右美托咪定组在各时间点均低于对照组( P <0.05 )。 2 组血浆 TNF-
α 和 ICAM-1 浓度呈升升降降趋势,右美托咪定组在 T 2 ~T 4 时均低于对照组( P <0.05 )。右美托咪定组肺组织
NF-κB 表达较对照组升高( P <0.05 )。结论 给予 1 μ g /

的右美托咪定负荷量,随后以 0.

率持续输注,可以抑制肺组织 NF-κB 的激活,减轻 OLV 导致的全身炎症反应,改善患者的氧合功能,发挥肺保护

operation ( T 4 ) and24hafteroperation ( T5 ) toobtainarterialbloodgasanalysisdata , oxygen
index ( OI ), respiratoryindex ( RI ), PaO 2 / PAO 2 andtodeterminelevelsofplasmaTNF-αand
ICAM-1.Lungtissueswereobtainedmorethan5cmawayfromtumoratthetimeoflobectomy
oflungsinordertodetermineNF-κBexpression.Results ThevaluesofOIandPaO 2 / PAO 2 were
decreasedatT 2 ~T 4 thanthatofatT 1 intwogroups.ThevaluesofOIandPaO 2 / PAO 2 were
increasedindexmedetomidinegroupcomparedtothatincontrolgroupatT 2 ~T 4 ( P <0.05 ) .The
valuesofRIwereincreasedatT 2 ~T 4 thanthatofatT 1 intwogroups , andweredecreasedin
dexmedetomidinegroupcomparedtothatincontrolgroupatT 2 ~T 4 ( P <0.05 ) .PlasmaTNF-α
and ICAM-1 levels showed ascending and descending trends , and were decreased in
dexmedetomidinegroupcomparedtothatincontrolgroupatT 2 ~T 4 ( P <0.05 ) .Theexpression
ofNF-κBwasincreasedindexmedetomidinegroupcomparedtothatincontrolgroup ( P <0.05 ) .
Conclusion Dexmedetomidinewith1 μ g / kgloadingdoseand0.5 μ g ·
kg
-1 · h -1 continuing
infusiondose during operation caninhibitactivation of NF-κB and reduce OLV induced
inflammatoryresponseaswellasimproveoxygenationfunctionandshowitslungprotection
roles.
