我院鲍曼不动杆菌耐药率与同期抗菌药物用药频度相关性分析

〖HTH〗目的〖HTSS〗〖KG*2〗调查河北医科大学第四医院2013年1月—2015年6月鲍曼不动杆菌的耐药现状和变化趋势,为临床合理使用抗菌药物提供依据。
〖HTH〗方法〖HTSS〗〖KG*2〗回顾性分析本院鲍曼不动杆菌的耐药率和同期常用8种抗菌药物用药频度,并进行相关性分析。
〖HTH〗结果〖HTSS〗〖KG*2〗鲍曼不动杆菌对抗菌药物的耐药率普遍偏高。对美罗培南的耐药率与其用药频度呈正相关(r=0.667,P=0.035);对头孢吡肟的耐药率与其用药频度呈负相关(r=-0.736,P=0.015);对哌拉西林钠/他唑巴坦钠、亚胺培南/西司他丁、头孢哌酮钠/舒巴坦钠、头孢他啶、左氧氟沙星、环丙沙星的耐药率与其用药频度均无相关性(P>0.05)。
〖HTH〗结论〖HTSS〗〖KG*2〗抗菌药物的用药频度和耐药率之间存在一定的相关性,建议根据药敏结果选择抗菌药物,本院使用头孢哌酮钠/舒巴坦钠和头孢吡肟是治疗鲍曼不动杆菌感染的较好选择。

Abstract: [Abstract]Objective〖HTSS〗〓To investigate the drug resistance status and trends of acinetobacter baumannii from our hospitalized patients between January 2013 and June 2015 so as to provide references for guiding reasonable application of antibiotics clinically.
〖HTH〗〖WTHZ〗Methods〖HTSS〗〓The drug resistance rates of acinetobacter baumannii and defined daily dose(DDDs) used eight kinds of antimicrobial drug were retrospectively analyzed. Meanwhile, nonlinear relationship was estimated by regression.
〖HTH〗〖WTHZ〗Results〖HTSS〗〓Acinetobacter baumannii has a high resistance to a variety of commonly used antimicrobial agents. It is significantly positively correlated with the consumption of meropenem(r=0.667, P=0.035) and negatively correlated with the consumption of cefepime(r=-0.736,P=0.015). There was no correlation between the resistance rate of acinetobacter baumannii and DDDs of piperacillin sodium/he azole sodium, imipenem/cilastatin, cefoperazone/sulbactam, ceftazidime, levofloxacin, ciprofloxacin(P>005). 
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗〓There is a certain correlation between the use of antibacterial drugs and the rate of bacterial resistance. Antibiotics should be used rationally in accordance with clinical sensitivity test results as far as possible. The cefoperazone/sulbactam and cefepime are the better options to treat acinetobacter baumannii infection in our hospital.
