〖HTH〗目的〖HTSS〗〖KG*2〗对膝关节骨关节炎患者关节滑液中的磨粒进行铁谱分析,以早期评价骨关节炎的病情。
〖HTH〗方法〖HTSS〗〖KG*2〗将368例疑为骨关节炎患者根据KellgrenLawrence(KL)分级分为正常关节组46例和骨关节炎组322例,无菌抽取关节液,将其沉淀物磁化处理后,在铁谱显微镜下观察磨粒形态,对比正常关节组和骨节炎组以及骨节炎组不同分级的磨粒的长度、面积、伸长率和熵。
〖HTH〗结果〖HTSS〗〖KG*2〗骨关节炎组磨粒的长度、面积、伸长率和熵均明显大于正常关节组,差异有统计学意义(P<005);骨关节炎组不同分级随着分级的升高其磨粒的长度、面积和伸长率逐渐减小,熵逐渐升高,差异均有统计学意义(P<005);磨粒长度与分级和熵呈正相关,与磨粒面积和伸长率呈负相关(P<005)。
〖HTH〗结论〖HTSS〗〖KG*2〗 磨粒的形态和数量与关节磨损的速率和机制相关。磨粒的研究具有较高的敏感性和客观性,为骨关节炎的早期诊断提供了一种可靠选择。

Abstract: [Abstract] Objective〖HTSS〗〓To perform ferrography analysis of wear debris in synovial fluid of patients with knee osteoarthritis and to make an early evaluation of the condition of osteoarthritis.
〖HTH〗〖WTHZ〗Methods〖HTSS〗〓Three hundreds and sixtyeight patients with suspected knee osteoarthritis were collected and divided into the normal group(46 cases) and the osteoarthritis group(322 cases) according to KellgrenLawrence(KL) classification. After aseptically extracting the joint fluid and magnetizing its sediment, the morphology of the abrasive particles was observed under the ferrography microscope. The length, area, elongation and entropy of abrasive particles were compared between normal group and osteoarthritis group.
〖HTH〗〖WTHZ〗Results〖HTSS〗〓The length, area, elongation and entropy of abrasive particles in the osteoarthritis group were significantly higher than those in the normal control group, and the difference was statistically significant(P<005). The abrasive particle length was positively correlated with the grading and entropy, the abrasive particle length was negatively correlated with the abrasive particle area and elongation rate(P<005).
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗〓The shape and quantity of abrasive particles are related to the rate and mechanism of joint wear.The study of abrasive particles has high sensitivity and objectivity, which provides a reliable choice for the early diagnosis of osteoarthritis.
