CT 灌注成像在缺血性脑血管病患者脑-硬膜-动脉贴敷手术中的应用

摘要: 目的:分析缺血性脑血管病患者在行脑-硬膜-动脉贴敷(encephalo-dura-artery sticking,EDAS)术前后 CT 灌注成像(CT perfusion,CTP)的特征,探讨 CTP 在该疾病 EDAS 术中的应用价值。方法对30例临床诊断为缺血性脑血管病的患者,于 EDAS 治疗前后行 CTP 检查,获得脑血容量(relative cerebral blood volume, rCBV)、相对脑血流量(relative cerebral blood flow,rCBF)、相对达峰时间(relative time to peak,rTTP)以及其他灌注参数,计算出 rCBV、rCBF、rTTP 相对灌值,分析患者在手术前后脑血流动力学的变化情况。结果 EDAS 术后狭窄血管供血区的 rTTP 值较术前降低(P <0.05);rCBF、rCBV 在手术前后无明显变化(P >0.05)。狭窄血管非供血区手术前后各灌注参数比较差异无统计学意义(P >0.05)。结论 CTP 可为 EDAS 手术前后提供无创性监测方法,灌注参数中 rTTP 可作为血流动力学变化的一项有效指标。

Abstract: Objective To analyze the CT perfusion(CTP)imaging features in patients with ischemic cerebrovascular disease before and after encephalo-dura-artery sticking (EDAS ) operation,and to explore the clinical value of CTP in the disease.Methods Thirty patients diagnosed ischemic cerebrovascular disease were applied CTP examination before and after EDAS operation,to obtain cerebral blood volume relative cerebral blood volume(rCBV),cerebral blood flow relative cerebral blood flow(rCBF),time to peak relative time to peak(rTTP)and other perfusion parameters.Then,the cerebral hemodynamics changes before and after surgery were analyzed comparatively.Results The rTTP value was reduced as compared with postoperative stenosis vascular blood supply region(P <0.05);while the rCBF and rCBV value did not show significant difference before and after EDAS(P >0.05).Perfusion parameters showed no statistical significance in non stenosis vascular blood supply region before and after surgery (P > 0.05 ). Conclusion CTP can provide non-invasive monitoring method for the patients,and rTTP is an effective indicator of hemodynamic changes before and after EDAS.
