〖HTH〗目的〖HTSS〗〖KG*2〗探讨CT肝容量测量联合肝功能 CTP(ChildTurcottePugh)分级用于评价原发性肝癌合并肝纤维化患者肝储备功能的临床价值。
〖HTH〗方法〖HTSS〗〖KG*2〗选择38例原发性肝癌合并肝纤维化患者为试验组,选择同期腹部CT检查结果正常的38例健康志愿者为对照组。对试验组进行CTP分级,对2组行CT肝容量测量,对比2组及试验组不同CTP分级患者肝容量变化,分析CT测量肝容量与CTP分级的相关性。
〖HTH〗结果〖HTSS〗〖KG*2〗与对照组相比,试验组肝左外叶、肝方叶、肝右叶、肝尾叶和肝脏总体积均缩小(P<005);原发性肝癌合并肝纤维化患者肝右叶体积、肝脏总体积随CTP分级增加逐渐缩小(P<005);患者CTP分级与患者肝右叶体积和肝脏总体积呈负相关(P<005)。
〖HTH〗结论〖HTSS〗〖KG*2〗 CT肝容量测量与CTP分级有机结合能够准确评价肝叶大小,有效评估患者肝脏储备功能,对于原发性肝癌合并肝纤维硬化患者治疗方案的制定具有指导意义,值得推广应用。

Abstract: [Abstract] Objective〖HTSS〗〓To investigate the clinical value of liver volume measurement by CT combined CTP grading in evaluating the liver reserve function in patients with primary hepatic carcinoma combined liver fibrosis.
〖HTH〗〖WTHZ〗Methods〖HTSS〗〓Thirtyeight cases of hepatic carcinoma with hepatic fibrosis in our hospital were selected as experimental group, 38 healthy volunteers with normal abdominal CT findings in the same period were selected as control group. In the experimental group, the CTP grading was used and CT liver volume measurement was conducted in the two groups. Liver volume changes between two groups of patients and experimental group was compared, and the correlation between the measured liver volume and CTP grading was analyzed.
〖HTH〗〖WTHZ〗Results〖HTSS〗〓Compared with the control group, the volume of right lobe and caudate lobe were decreased(P<005), the volume of quadrate lobe and the total liver volume were decreased(P<005). The volume of right lobe and the total liver volume in patients with primary liver cancer were decreased(P<005). The CTP grading was negatively correlated with the volume of right lobe and liver volume(P<005).
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗〓The CT liver volume measurement combined with the CTP grading can accurately evaluate the liver lobe size, effectively assess the patient′s liver reserve function, and has guiding significance for the development of treatment scheme for patients with primary liver cancer and  hepatic fibrosis, which is worthy of popularizing.
