The correlations between acute lung injury of aortic dissection with serum CRP, MCP-1, and MMP-9

Abstract:

Objective  To investigate the correlations between acute lung injury(ALI) in aortic dissection(AD) with serum C-reactive protein(CRP), monocyte chemoattractant protein-1(MCP-1), matrix metalloproteinase-9(MMP9). 

Methods  A total of 92 aortic dissection patients were selected for the study, among them, 42 were ALI patients(ALI group), and 50 were non-ALI patients(non-ALI group), 50 healthy persons(control group) were selected for physical examination during the same period, the levels of serum CRP, MCP-1, and MMP-9 were detected by enzyme-linked immunosorbent assay(ELISA), the correlations of the expressions of CRP, MCP-1, and MMP-9 and their predictive values for ALI were also studied. 
Results  The levels of serum CRP, MCP-1 and MMP9 in non ALI group and ALI group were higher than those in control group, and those in ALI group were higher than those in non ALI group(P<0.05). The levels of serum interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) in non ALI group and ALI group were higher than those in control group, and those in ALI group were higher than those in non ALI group; Serum IL-10 in non ALI group and ALI group was lower than that in control group, and the difference was statistically significant(P<0.05). Pearson correlation analysis showed that serum CRP was positively correlated with MCP-1 and MMP9(r=0.827, P<0.001; r=0.868, P<0.001), and MCP-1 was positively correlated with MMP9(r=0.741, P<0.001). The AUC of CRP was 0.856, the diagnostic sensitivity was 66.70%, the specificity was 88.00%, the AUC of MCP-1 was 0.769, the diagnostic sensitivity was 69.00%, the specificity was 76.00%, the AUC of MMP9 was 0.884, the diagnostic sensitivity was 81.00%, the specificity was 90.00%. The AUC of combined detection of CRP and MCP-1 was 0.930, the diagnostic sensitivity was 83.30%, and the specificity was 90.00%. 
Conclusion  The levels of CRP, MCP-1, and MMP9 in serum of patients with acute lung injury caused by aortic dissection are increased, and they are positively correlated, the combined diagnosis of CRP, MCP-1, and MMP9 has high sensitivity and specificity, it has high diagnostic value for acute lung injury caused by aortic dissection.
