Abstract ] Objective Toanalyzethediagnosisandtreatmentoffibrinshellformationof
centralverlousportaccesssystem ( CVPAS ) .Methods There were131casesofcatheter
dysfunctionofCVPAS.Byusingthedigitalsubtractionangiography ( DSA ) andtheexternal
interfaceofthebutterflywingneedleintheportalvein , thecontrastagentwasperformedin
vivo , andtheDSAexpressionwasanalyzed , andthetreatmenteffectwasobserved.Results
Fifty-onecasesoffibrinshellformationwerefoundbyDSA , whichaccountedfor38. 9% ofthe
venousportcatheterdysfunction ( 51 / 131 ) .Amongthose , 43caseswerelocalizedintheendofthe
catheter , 5casesofdiffusefibrousproteininthebody , and3casesofextensivefibroussheath
formation.DSAspecificimagingshowstheline , columnarandirregularhighdensityalongouter
wallofbodycatheterandparcelbodycatheter.Thediameterofparcelcatheterisobvious
thickening , Thecontrastagentrefluxintothevenacava
,
theimplantationsideofCVPASandthe infusionseat peripheralsubcutaneous sofetissue.Fifty one patients are given urokinase
treatment , 48casesCVPAscontinuetouse , 3casesCVPASanticipateextirpation.Thetreatment
successratesoflocal , diffuseandextensivefibrinshellformationgivenurokinasetreatmentare
respectively100. 0% ( 43 / 43 ), 100. 0% ( 5 / 5 ) vs 0. 0% ( 0 / 3 ) .Conclusion Thecausesofcatheter
diffusefibrinshellformationofCVPAScanbeevaluatedaccuratelybymeansoftheDSA.The
maincauseofcatheterdysfunctionisfibrinshellformation.Urokinasehasgoodtherapeuticeffect
onlocalfibrinshellformation.
