Abstract ] Objective Toevaluatethediagnosticandprognosticperformanceofheart-type
fattyacidbindingprotein ( H-FABP ) intheearlyofacutecoronarysyndrome ( ACS ) .Methods A
prospectivestudywasusedin173patientswithchestpainwithin12h.H-FABP , cardiactroponin
I ( cTnI ) andmyoglobin ( MYO ), creatinekinase ( CK ), creatinekinase-MB ( CK-MB ) andother
biochemicalindicatorsweremeasured.Theincidenceofmajoradversecardiacevents ( MACE ) was
observedinpatientsduringhospitalization.Therelevantinformationwascollectedandanalized.
Results ①ThepositiverateofH-FABPinacutemyocardialinfarction ( AMI ) wassignificantly
higherthancTnI , MYO , CKandCK-MBinthechestpainonset0-3h ( P <0.05 ) .The sensitivity , positivepredictivevalue

PPV ), accuracyandYoudenindexofH-FABPwerehigher
thancTnI , MYO , CK , CK-MB.②ThepositiverateofH-FABPinAMIwassimilarwithcTnI
andMYO ( P >0.05 ), butsignificantlyhigherthanCK , CK-MB ( P <0.05 ) in>3-6hours.The
positivepredictivevalue , accuracyandYoudenindexofH-FABPweresimilarwithcTnI , but
higherthan MYO , CK , CK-MB.③ThepositiverateandsensitivityofH-FABPinAMIwas
similarwithcTnI ( P >0.05 ) in >6-12hours.Thepositivepredictivevalue , accuracyand
YoudenindexofH-FABPweresimilarwithcTnI , CK , CK-MB , buthigherthanMYO.④The
specificityofH-FABP wassimilarwithcTnI , buthigherthan MYO , CK , CK-MBin0-12
hours.⑤ThediagnosticvalueofH-FABPwashigherthanotherindexesin0-3hours.⑥The
negativepredictivevalue ( NPV ) ofH-FABPandH-FABP+cTnIwerehigherthancTnI , MYO ,
CK , CK-MB.⑦Accordingtotheincidenceofmajoradversecardiacevents ( MACE ) ingroup
AMIinaweek , the OR ofH-FABPis2.761 , 95% CI is0. 704-0.902.Conclusion ①The
sensitivity , accuracyandspecificityofH-FABPishigherthancTnI
, MYO , CK , andCK-MB.
ThereforeH-FABPcanbeusedasasensitiveindicatorforearly AMI , particularly within3
