    Observation on the effect of transcranial direct current stimulation combined with mirror neuron therapy on motor aphasia after ischemic stroke

    Objective To explore the effect of transcranial direct current stimulation combined with mirror neuron therapy on motor aphasia after ischemic stroke. 
    Methods Eighty patients with motor aphasia after ischemic stroke admitted to the hospital were selected and divided into control group(n=40) and experimental group(n=40) according to random number table. Both groups were given basic treatment, the control group was given mirror neuron therapy, and the experimental group was treated with transcranial direct current stimulation on the basis of the control group. Both groups were treated for 20 d. Before and after treatment, the two groups were assessed using the Boston Diagnostic Aphasia Examination(BDAE), Western Aphasia Battery(WAB) and the National Institutes of Health Stroke Scale(NIHSS), and the clinical efficacy, and incidence of adverse reactions were compared between two groups. 
    Results After treatment, the difference in BDAE grade distribution between two groups was statistically significant(P<0.05). After treatment, there was significant difference in the distribution of curative effects between two groups(P<0.05), and the total effective rate of the experimental group was higher than that of the control group(P<0.05). After treatment, the WAB scores of both groups were increased(P<0.05), which were higher in experimental group(P<0.05), and the NIHSS scores of two groups were decreased(P<0.05), and which were lower in experimental group(P<0.05). There was no significant difference in the incidence of adverse reactions between two groups(P>0.05). 
    Conclusion Transcranial direct current stimulation combined with mirror neuron therapy can improve the language function of patients with motor aphasia after ischemic stroke, with a definite effect, good safe and reliability.

    Value of clinical features and laboratory indicators in predicting the risk of spontaneous recanalization of infarct-related arteries in patients with acute ST-segment elevation myocardial infarction

    Objective To study the value of clinical features and laboratory indicators in predicting the risk of spontaneous recanalization of infarct-related artery(IRA) in patients with acute ST-segment elevation myocardial infarction(STEMI). 
    Methods In total, 375 patients with acute STEMI treated in our hospital were selected as the research subjects. After admission, patients underwent emergency coronary angiography. Patients with IRA TIMI grade 2-3 were included in spontaneous recanalization group(n=114), and patients with IRA TIMI grade 0-1 were included in non-spontaneous recanalization group(n=261). Clinical data of patients were collected, including age, sex, body mass index(BMI), smoking, hypertension or diabetes, and preinfarction angina. Heart rate, number of lesions and infarct sites were recorded. Serum fasting blood glucose(FBG), fibrinogen(FIB), albumin(Alb), creatine kinase isoenzyme(CK-MB), high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C), lipoprotein α[Lp(α)] levels were detected, and FAR values were calculated. ROC curve was used to analyze the value of FIB, CK-MB, FAR and Lp(α) in predicting the risk of spontaneous recanalization of IRA in patients with acute STEMI. Unconditional Logistic stepwise regression analysis was used to analyze the independent predictors of spontaneous recanalization of IRA in patients with acute STEMI. Consistency analysis was used to evaluate the value of multiple predictors in predicting the risk of spontaneous recanalization of IRA in patients with acute STEMI. 
    Results Univariate analysis showed that compared with non-spontaneous recanalization group, the proportion of smoking patients in spontaneous recanalization group was lower, the proportion of patients with preinfarction angina was higher, and the levels of FIB, CK-MB,FAR and Lp(α) in spontaneous recanalization group were lower(all P<0.05). ROC analysis confirmed that FIB level ≤ 359.440 mg/L, CK-MB level ≤73.460 U/L, FAR value ≤ 9.585, and Lp(α)≤150.947 nmol/L were the optimal cut-off values of IRA spontaneous recanalization in patients with acute STEMI(all P<0.05). The results of multivariate Logistic regression analysis showed that smoking, preinfarction angina, FIB level ≤359.440 mg/L, CK MB level ≤73.460 U/L, FAR value ≤9.585, and Lp(α)≤150.947 nmol/L were independent predictors of spontaneous recanalization of IRA in patients with acute STEMI(all P<0.05). By consistency analysis, the sensitivity, specificity, accuracy and Kappa of multiple predictors in predicting spontaneous recanalization of IRA in patients with acute STEMI were 0.930, 0.931, 0.931 and 0.840, respectively. 
    Conclusion The FIB level ≤ 359.440 mg/L, CK-MB level ≤73.460 U/L, FAR value ≤9.585, and Lp(α)≤150.947 nmol/L were the optimal cut-off values of IRA spontaneous recanalization in patients with acute STEMI. Smoking and preinfarction angina may be the influencing factors of IRA spontaneous recanalization. Multiple factors combined to predict the risk of IRA spontaneous recanalization in patients with acute STEMI has a higher accuracy.

    Effect of bi-level positive airway pressure on prognosis and vascular endothelial function in patients with OSAHS

    Objective To analyze the effect of bi-level positive airway pressure(BiPAP) on the prognosis and vascular endothelial function of patients with obstructive sleep apnea hypopnea syndrome(OSAHS). 
    Methods In total, 100 patients with OSAHS were included in the study, and randomly divided into observation group(n=50) and control group(n=50).The observation group was treated with BiPAP, while the control group was given routine treatment.Polysomnography(PSG) indexes(hypoventilation index, apnea index, apnea and hypopnea time, times of oxygen saturation below 90%, time of oxygen saturation below 90%), cardiopulmonary function brain natriuretic peptide(BNP), Tei index of the right ventricle, Tei index of the left ventricle, forced expiratory volume in one second/forced vital capacity(FEV1/FVC), forced expiratory volume in one second as a percentage of expected value(FEV1%pred), vascular endothelial function [endothelin-1(ET-1), nitric oxide(NO), antithrombinⅢ(AT-Ⅲ)] and quality of life were compared between two groups. 
    Results The hypoventilation index, apnea index, apnea and hypopnea time, times of blood oxygen saturation below 90% and time indexes of blood oxygen saturation below 90% in the observation group were significantly lower than those in the control group(P<0.05). After treatment, BNP and Tei index in the observation group were significantly lower than those before treatment and after treatment in the control group. FEV1/FVC and FEV1%pred in the observation group were significantly higher than those before and after treatment in the control group(P<0.05). After treatment, ET-1 and AT-Ⅲ in the observation group were lower than those before treatment and after treatment in the control group, while NO in the observation group was higher than that before treatment and after treatment in the control group(P<0.05). The scores of quality of life in the observation group were significantly higher than those in the control group(P<0.05). 
    Conclusion BiPAP treatment can improve the prognosis and vascular endothelial function of patients with OSAHS, showing definite curative effect.

    Study on the relationship between melatonin, AOPP levels and osteoporosis in elderly patients with chronic renal failure undergoing maintenance hemodialysis

    Objective To analyze the relationship between the levels of melatonin(MT), advanced oxidized protein product(AOPP) and osteoporosis in elderly patients with chronic renal failure(CRF) undergoing maintenance hemodialysis.
    Methods A total of 105 elderly patients with CRF undergoing maintenance hemodialysis in our hospital were enrolled. The incidence of osteoporosis was calculated, and the serum MT, AOPP levels and bone mineral density between osteoporosis patients and non-osteoporosis patients were compared. Pearson correlation analysis was performed on the relationship between serum MT and AOPP levels and bone mineral density in elderly patients with CRF undergoing maintenance hemodialysis. Logistic regression analysis was performed on the factors affecting osteoporosis in elderly patients with CRF undergoing maintenance hemodialysis. The receiver operating characteristic(ROC) curve was used to analyze the efficacy of serum MT and AOPP levels in predicting osteoporosis in elderly patients with CRF undergoing maintenance hemodialysis. 
    Results The incidence of osteoporosis in patients with CRF undergoing maintenance hemodialysis was 45.71%. The serum AOPP level of osteoporosis patients was higher than that of non-osteoporosis patients(P<0.05), and the serum MT level and bone density(lumbar spine L1-4 and bilateral femoral neck and Ward′s area) were lower than those of non-osteoporosis patients(P<0.05). The results of Pearson correlation analysis showed that serum AOPP levels in elderly patients with CRF undergoing maintenance hemodialysis were negatively correlated with bone mineral density in different areas(P<0.05), and serum MT levels were positively correlated with bone mineral density in different areas(P<0.05). Compared with non-osteoporotic patients, the serum osteocalcin levels and dialysis age of osteoporosis patients had statistically significant differences(P<0.05). Logistic multivariate regression analysis showed that serum dialysis age, AOPP, MT,and gender were independent risk factors for osteoporosis in elderly patients with CRF undergoing maintenance hemodialysis(P<0.05). ROC analysis showed that the optimal cut-off points of MT and AOPP levels for predicting osteoporosis in elderly patients with CRF undergoing maintenance hemodialysis were 90.99 μmol/L and 21.66 ng/L respectively, and the area under the curve(AUC) was 0.859 and 0.713 respectively. 
    Conclusion Elderly patients with CRF undergoing maintenance hemodialysis have abnormally increased serum AOPP levels, and abnormally decreased serum MT levels and bone mineral density. Serum AOPP and MT levels are correlated with bone mineral density in elderly patients with CRF undergoing maintenance hemodialysis, which can be used as sensitive indicators to predict osteoporosis in CRF patients. 

    Expression and clinical significance of SPARC protein in breast cancer

    Objective Secreted proteinacidic and rich in cysteine(SPARC) plays an important role in tumor growth, invasion and metastasis. This article explores the expression of SPARC in breast cancer tissues and its relationship with prognosis. 
    Methods In total,122 postoperative specimens of primary breast cancer patients from the Fourth Hospital of Hebei Medical University and 38 normal breast tissue specimens were randomly collected. Chi-square test was used to analyze the relationship of SPARC with the clinicopathological data of patients. Kaplan-Meier method, Log-rank test and COX regression model were used to analyze survival and risk factors. 
    Results The positive rate of SPARC in breast cancer tissues was higher than that in normal breast tissues(24.6% vs. 7.9%), and the difference was statistically significant(P<0.05). The positive rates of interstitial cells in the two groups were 54.9% and 68.4% respectively, and the difference was not statistically significant(P>0.05). The staining of epithelial cells in tumor tissue was defined as positive. The expression of SPARC in breast cancer tissues was not related to patient age, tumor diameter, lymph node status, TNM stage, vascular tumor thrombus, histological grade, molecular typing, estrogen receptor(ER), and human epidermal growth factor receptor 2(HER2) status(all P>0.05), but related to pathological type and progesterone receptor(PR) status(P<0.05). The difference in overall survival(OS) of breast cancer patients with positive and negative SPARC protein expression was statistically significant(P<0.05), and disease-free survival(DFS) was not statistically different(P>0.05). SPARC protein expression was not an independent prognostic factor for OS in patients[P=0.052, HR=0.142(0.019-1.060)]. 
    Conclusion The expression of SPARC in epithelial cells of breast cancer tissue is significantly higher than normal breast tissue and is related to pathological type and PR status. The expression of SPARC is related to the patient′s OS, which may be an independent prognostic factor that affects the patient′s death and can reduce the patient′s risk of death.

    Analysis of SIRT7 expression and its correlation with immune infiltration level in pancreatic cancer

    Objective To investigate the expression of silent information regulator 2 homolog protein 7(SIRT7) and its correlation with immune infiltration level in pancreatic cancer. 
    Methods The mRNA expression of SIRT7 in pancreatic cancer tissues was collected from Oncomine and GEPIA databases. A total of 27 clinical tissue specimens were collected by immunohistochemical staining, and SIRT7 protein expression was detected. The relationship between SIRT7 expression and immune infiltration level in pancreatic cancer was analyzed. SIRT7 gene mutation landscape was analyzed by cBioPortal based on the RNA sequencing data of pancreatic cancer samples. Proteins interacting with SIRT7 were predicted using the STRING database and gene set enrichment analysis was performed. 
    Results Analyzing data from Oncomine and GEPIA database and the immunohistochemical results from clinical tissue specimens revealed that transcription and protein levels of SIRT7 were both highly expressed in pancreatic cancer tissues compared with adjacent tissues. SIRT7 expression was negatively correlated with the expression of effector memory CD8+ T cells, B cells, nature killer cells, nature killer T cells in pancreatic cancer,but positively correlated with the level of T helper cells 17. SIRT7 gene mutation occurred in 13 of 749 pancreatic cancer samples from cBioPortal, with a total mutation rate of 1.73%. Histone deacetylase 1/2/4/6/8/11(HDAC 1/2/4/6/8/11), tumor protein P53(TP53), forkhead box O3(FOXO3) had obvious interactions with SIRT 7. These proteins participated in numbers of biological process such as histone deacetylation and DNA transcription regulation, and were involved in many cancer-related pathways such as cell cycle and Notch. 
    Conclusion SIRT7 is overexpressed in pancreatic cancer tissue, which is closely related to tumor immune infiltration. The proteins interacted with it are involved in numbers of pancreatic cancer-related pathways. SIRT7 may be a potential target for pancreatic cancer diagnosis, treatment and evaluation of immune therapy effect.

    Expression and significance of Hh signaling pathway-related factors Shh, Ptch-1 and Gli-1 in prostate cancer

    Objective To investigate the expression and significance of Hedgehog(Hh) signaling pathway-related factors Sonic hedgehog(Shh), Patched-1(Ptch-1) and glioma-related oncogene homology-1(Gli-1) in prostate cancer. 
    Methods Sixty cases of prostate cancer and normal tissue specimens adjacent to prostate cancer were collected. The positive expressions of Shh, PTCH-1 and GLI-1 in each sample were detected by immunohistochemistry. The mRNA expression of Shh, Ptch-1 and Gli-1 in each specimen was detected by reverse transcription polymerase chain reaction(RT-PCR). The correlation between Shh, PTCH-1, GLI-1 and clinical stage, differentiation degree and serum prostate-specific antigen(PSA) level of prostate cancer  was analyzed. 
    Results The positive signal of Shh protein was mainly expressed in cytoplasm or membrane, the positive signal of Ptch-1 protein was mainly expressed in cytoplasm, and the positive signal of Gli-1 protein was mainly expressed in the nucleus or cytoplasm. The positive expression rates of Shh, Ptch-L and Gli-L in prostate cancer tissues were significantly higher than those in adjacent tissues(P<0.05). There was a positive correlation between Shh and Ptch-1 in prostate cancer tissue specimens(P<0.05), but no correlation between Shh and Gli-1, as well as between Ptch-1 and Gli-1(P>0.05). There were significant differences in mRNA expression levels of Shh, Ptch-1 and Gli-1 in prostate cancer tissues with different clinical stages, degrees of differentiation and serum PSA level(P<0.05). The mRNA expressions of Shh, Ptch-1 and Gli-1 in prostate cancer tissues were positively correlated with clinical stage, degree of differentiation and serum PSA level(P<0.05). 
    Conclusion The up-regulated expression levels of Shh, Ptch-1 and Gli-1 in prostate cancer tissues are correlated with the clinical stage and differentiation degree of prostate cancer, suggesting that Shh, PTCH-1 and GLI-1 are closely associated with the occurrence and development of prostate cancer. Therefore, inhibition of Hh signaling pathway is of great significance for the diagnosis and treatment of prostate cancer.

    Analysis of risk factors related to non-curable resection of early gastric cancer by endoscopic submucosal dissection

    Objective To evaluate the efficacy and safety of endoscopic submucosal dissection(ESD) in the treatment of early gastric cancer(EGC), and to understand the related risk factors and prognosis of non-curative resection(nCuR). 
    Methods The relevant clinical data and follow-up prognosis of EGC patients who received ESD were collected, and they were divided into curative resection(CuR) and nCuR groups. 
    Results Among the 99 patients included,the number of cases aged 40 years or older increased significantly, and reached a peak around the age of 65. The diameter of the lesion was (2.32±1.73) cm, and the lesions were mainly located in the upper third of the stomach(70.7%). En-bloc resection was performed in 99 cases(100.0%), complete resection in 79 cases(79.8%), and curative resection in 71 cases(71.7%). There were statistical differences in lesion diameter, postoperative pathology and depth of invasion in the CuR and nCuR groups. Logistic regression analysis showed that postoperative pathology and depth of invasion were risk factors for nCuR(P<0.05).According to the follow-up after ESD, among the patients who underwent stratification of eCura scoring system, 8 had additional surgery, 3 were in the low-risk group, and 5 were in the moderate-risk group; 1 patient who was found to have local cancer residue and regional lymph node metastasis was classified as moderate risk. 
    Conclusion In preoperative evaluation of EGC patients, more attention should be paid to the judgment of lesion diameter, preoperative pathological type and depth of invasion, and the indication standard should be fully understood. The treatment plan for patients with nCuR may refer to the eCura scoring system. 
    Clinical effect of vagus nerve stimulation in the treatment of refractory epilepsy and its impact on cognitive function

    Objective To investigate the clinical effect of vagus nerve stimulation in the treatment of refractory epilepsy and its impact on cognitive function. 
    Methods A total of 12 patients with intractable epilepsy treated by vagus nerve stimulation were selected. According to the McHugh rating scale, the therapeutic effect was evaluated at 1 month before operation and at 3, 6, 9 and 12 months after operation. Raven′s reasoning test was used to evaluate IQ, and cognitive function was assessed by mini-mental state examination(MMSE), and the Beijing version of the Montreal cognitive assessment(MoCA). 
    Results With the extension of treatment time, the frequency of attack gradually decreased, and the difference was statistically significant(P<0.05). The incidence of remission more than 50% was 33.3% in 3 months, 66.7% in 6 months, 75.0% in 9 months and 83.33% in 12 months. IQ, MMSE score and Beijing version of MoCA score at 12 months after surgery were higher than those at 1 month before surgery, and the differences were statistically significant(P<0.05). 
    Conclusion Vagal nerve stimulation is safe and effective in the treatment of refractory epilepsy. It can not only reduce the frequency of epileptic seizures, but also improve the IQ level and cognitive ability of epileptic patients.

    Expression of miR-205-5p, PDCD5 and Beclin1 in endometrial cancer tissues and their correlation

    Objective To explore the expression of microRNA-205-5p(miR-205-5p), programmed cell necrosis factor 5(PDCD5), and Beclin1 in endometrial cancer tissues and their correlation. 
    Methods In total, 75 cases of endometrial cancer tissues and adjacent tissues 5 cm away from the edge of the cancerous tissue of endometrial cancer were selected. The expressions of miR-205-5p, PDCD5, and Beclin1 were detected by immunohistochemistry and real-time fluorescence quantification methods, and the correlation of MiR-205-5p, PDCD5 and Beclin1 and their diagnostic value for endometrial cancer were analyzed. 
    Results The expression of miR-205-5p in cancer tissues was higher than that in adjacent tissues, while the expression of PDCD5 and Beclin1 were lower than that in adjacent tissues(P<0.05). The expressions of miR-205-5p, PDCD5 and Beclin1 in endometrial cancer patients with different histological grade, clinical stage, lymph node metastasis and muscle invasion were significantly different(P<0.05). The expressions of miR-205-5p were increased in endometrial cancer patients with stage Ⅲ-Ⅳ, moderate-to-low differentiation, lymph node metastasis and muscle invasion≥1/2 , while the expression of PDCD5 and Beclin1 were decreased, and the difference was statistically significant(P<0.05). There were no significant differences in the expressions of miR-205-5p, PDCD5 and Beclin1 in endometrial cancer patients with different ages and pathological types(P>0.05). There was negative correlation between miR-205-5p and PDCD5, negative correlation between miR-205-5p and Beclin1, and positive correlation between PDCD5 and Beclin1(P<0.05). The value of combined detection of the three in the diagnosis of endometrial cancer was higher than that of miR-205-5p, PDCD5 and Beclin1 alone(P<0.05). 
    Conclusion The increased expression of miR-205-5p and the decreased expression of PDCD5 and Beclin1 promote the development of endometrial cancer and participate in the evolution of endometrial cancer. The endometrial cancer can be predicted in clinical practice according to the above indicators.

    Comparison of evaluation effects of different ovarian response predictors on ovarian response function in infertile women

    Objective To explore the effects of age,antral follicle count(AFC), basal serum follicle-stimulating hormone, anti-müllerian hormone(AMH), basal follicle-stimulating hormone(FSH)/ luteinizing hormone(LH), and ovarian response prediction index(ORPI) on the evaluation of ovarian response function. 
    Methods A total of 490 patients who were treated with in vitro fertilization and intracytoplasmic sperm injection for the first time in the Department of Reproductive Medicine of our hospital were selected as the research subjects. The above indicators were measured and calculated. The receiver operating characteristic(ROC)curve was used to analyze the prediction accuracy of variables, and the cutoff value and the corresponding sensitivity and specificity were calculated. 
    Results Among the 490 infertile patients, 61 cases(12.45%) had low ovarian response, 352 cases(71.84%) had normal ovarian response, and 77 cases(15.71%) had high ovarian response. The area under the ROC curve(AUC) of age, AFC, basal FSH, AMH, and FSH/LH and ORPI of the patients was 0.674,0.964, 0.674, 0.842, 0.716 and 0.915, respectively, the sensitivity was 93.5%, 92.2%, 84.4%, 79.2%, 63.6% and 83.1%, respectively, and the specificity was 60.5%, 97.7%, 44.1%, 77.0%, 73.8% and 89.5%, respectively. The cutoff values were 35.5 years of age, 15, 7.75 U/L, 4.325 mg/L, 1.633 and 1.985, respectively. The AUC values in the ovarian hyporesponse group were 0.616, 0.844, 0.589, 0.789, 0.573, and 0.834, respectively. The sensitivity was 57.4%, 75.4%, 44.3%, 70.5%, 62.3%, and 78.7%, the specificity was 69.0%, 99.8%, 55.7%, 84.8%, 59.2%, and 85.3%, respectively, and the cutoff values were 35.5 years of age, 4, 8.945 U/L, 1.085 mg/L, 2.252, and 0.207, respectively. 
    Conclusion For infertile women, AFC and ORPI are better than other variables in the evaluation of ovarian response, and show a better effect in the prediction of ovarian hyperresponse. Further exploration of more predictive indicators is needed in the prediction of ovarian hyporesponse.

    Objective To explore the relationship of umbilical cord blood adiponectin with glucose and lipid metabolism, fetal growth and development in infants of mothers with gestational diabetes mellitus(GDM). 
    Methods In total, 148 infants of GDM mothers were selected as the research subjects. According to fetal birth weight and gestational age, they were divided into small for gestational age(SGA) group(n=48), appropriate gestational age(AGA) group(n=59), and large than gestational age(LGA) group(n=41). The serum adiponectin, blood glucose and blood lipid index levels in the cord blood were detected, and the relationship between the adiponectin level in the cord blood and the glucose and lipid metabolism, fetal growth and development indicators was analyzed. 
    Results There were significant differences in gestational age, birth weight, birth weight index, cord blood adiponectin, cord blood glycated hemoglobin(HbA1c), cord blood insulin, and maternal blood insulin among the three groups(P<0.05). No significant difference was found in sex, cord blood glucose, cord blood triglyceride, cord blood cholesterol, cord blood low-density lipoprotein cholesterol, cord blood high-density lipoprotein cholesterol, and cord blood homeostasis model assessment-insulin resistance(HOMA-IR)(P>0.05). GDM maternal age, pre-pregnancy body weight, body weight at delivery, pre-pregnancy body mass index(BMI), increase in BMI during pregnancy, maternal blood glucose, maternal blood HbA1c, and maternal blood HOMA-IR were not statistically significant(P>0.05). The results of Pearson correlation analysis showed that adiponectin in fetal cord blood was positively correlated with neonatal gestational age, neonatal birth weight, neonatal birth mass index, cord blood HOMA-IR, but negatively correlated with cord blood HbA1c and cord blood insulin(P<0.05). The results of multiple linear regression analysis showed that neonatal birth weight index, cord blood adiponectin, cord blood HbA1c, and cord blood insulin were the influencing factors of fetal growth and development(P<0.05). 
    Conclusion Umbilical blood adiponectin has a positive correlation with fetal glucose and lipid metabolism and growth and development, which can reflect fetal glucose and lipid metabolism and growth and development, thereby providing guidance for promoting normal fetal growth and development and reducing fetal insulin resistance.

    Objective To evaluate the correlation of inflammatory cytokine levels and main indexes reflecting coagulation function in children with sepsis. 
    Methods A total of 80 children with severe sepsis/septic shock were enrolled retrospectively and then divided into two groups based on Chinese disseminated intravascular coagulation(DIC) score: DIC group and non-DIC group. The basic information, routine blood test, C-reactive protein(CRP), procalcitonin(PCT) and inflammation cytokines [interleukin(IL)-2, IL-4, IL-6, IL-10, tumor necrosis factor-α(TNF-α), interferon-γ(IFN-γ)], coagulation tests [prothrombin time(PT), activated partial thromboplastin time(APTT), thrombin time(TT), fibrinogen(FIB), D-dimer(DD)] and thrombelastogram(TEG) in both groups were collected. The correlation between cytokines and CDSS score, four items of blood coagulation, DD and TEG was analyzed. 
    Results The white blood cells in DIC group were lower while CRP and IL-6 were significantly higher than those in non-DIC group(P<0.05). There were no significant differences in gender, age, absolute lymphoid value, PCT, IL-2, IL-4, IL-10, TNF-α and IFN-γ between two groups(P>0.05). PT, APTT, international normalized ratio(INR), TT, DD and R value in DIC group were higher, while FIB, Angle, MA value and CI value in  DIC group were lower, as compared with those in non-DIC group(P<0.05). Correlation analysis showed that CDSS score was positively correlated with IL-6, CRP, PCT, R value, PT, INR, APTT, and DD, and negatively correlated with MA value, CI value and FIB(P<0.05). Also, PCT was positively correlated with APTT, and CRP was positively correlated with R value(P<0.05). 
    Conclusion There is correlation between CDSS with TEG, four items of blood coagulation and inflammatory indexes in children with sepsis, and IL-6 showed a strongest correlation with CDSS, which may be one of the potential mechanisms for the occurrence and development of DIC. 

    The prognostic value of albumin-related ratio in children with sepsis

    Objective To evaluate the predictive value of albumin-related ratio in the prognosis of children with sepsis. 
    Methods In total, 193 children with sepsis were selected as the research subjects. According to the 28-day survival, they were divided into survival group(n=132) and death group(n=61). The white blood cell count(WBC), neutrophil count(NEU), C-reactive protein(CRP), procalcitonin(PCT), blood lactic acid(Lac) and serum albumin(ALB) were collected within 24 h after admission. The pediatric risk of mortality Ⅲ(PRISM Ⅲ) was recorded within 24 h after PICU admission. Risk factors of death in children with sepsis, and the correlation between ALB and other indicators was analyzed; albumin-related ratio, including NEU/ALB, CRP/ALB, Lac/ALB were calculated. Receiver operating characteristic(ROC) curve was used to evaluate the predictive value of ALB, NEU/ALB, CRP/ALB, Lac/ALB, and PRISM Ⅲ in the prognosis of children with sepsis, and Kaplan Meier survival curve was drawn. 
    Results PRISM Ⅲ score of death group was higher than that of survival group, while ALB was lower than that of survival group, and the difference was statistically significant(P<0.05). ALB was positively correlated with PRISM Ⅲ, NEU, CRP and Lac(P<0.05). NEU/ALB, CRP/ALB, and Lac/ALB of death group was higher than those of survival group, and the difference was statistically significant(P<0.05). Lac/ALB had the highest predictive value in 28-day mortality of children with sepsis, its sensitivity was the highest, and its specificity was second only to CRP/ALB. CRP/ALB had a lower predictive value in 28-day mortality of children with sepsis than Lac/ALB, and its sensitivity was lower. With 28-day prognosis of children with sepsis as the outcome, survival rate of patients in ALB, PRISM Ⅲ score, NEU/ALB, CRP/ALB, and Lac/ALB of normal level group was higher than that in the corresponding indexes of abnormal level group, and the difference was statistically significant(P<0.05). 
    Conclusion NEU/ALB, CRP/ALB, and Lac/ALB are easily obtained prognostic indicators, which can identify septic children with poor prognosis. Lac/ALB improves the predictive value and sensitivity of prognosis in children with sepsis.

    Objective To explore the effect of sufentanil combined with sevoflurane anesthesia on hemodynamics and postoperative agitation in pediatric adenoid hypertrophy resection. 
    Methods A prospective study of 171 children undergoing adenoid hypertrophy resection was performed. According to the random number table method, they were divided into the observation group(n=86) and the control group(n=85). The observation group was given sufentanil combined with sevoflurane anesthesia, and the control group was given propofol combined with remifentanil anesthesia. The perioperative hemodynamics, postoperative agitation rate, facial expression scale(FPS) score, and observer's assessment of alertness/sedation(OAAS) score were compared between two groups. The incidence of adverse reactions was calculated. 
    Results Pulse oxygen saturation(SpO2) and mean arterial pressure(MAP) in the two groups showed a trend of decreasing initially and then increasing, and heart rate(HR) showed a trend of increasing initially and then decreasing. The difference of interaction between groups, time points, and time points between groups was statistically significant with respect to fluctuation range of SpO2, MAP and HR(P<0.05). The OAAS score of the two groups showed a trend of initial decreasing and then increasing, and the FPS score was increasing initially and then decreasing. The OAAS score of the observation group was higher than that of the control group, while the FPS score was lower than that of the control group. The difference in interaction between groups, time points, and time points between groups was statistically significant(P<0.05). The agitation rates of the observation group at 6 h, 24 h and 48 h after operation were lower than those in the control group, and the differences were statistically significant(P<0.05). The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). 
    Conclusion Compared with propofol combined with remifentanil anesthesia, sufentanil combined with sevoflurane anesthesia for adenoid hypertrophy resection can stabilize hemodynamics, and reduce postoperative agitation rate, which is safer and more reliable.

    Objective To analyze expression and significance of interferon-γ(IFN-γ),interleukin-6(IL-6) and interleukin-4(IL-4) in patients with multiple myeloma(MM) complicated with herpes zoster infection caused by bortezomib-containing regimen. 
    Methods In total, 60 patients with MM complicated with herpes zoster infection caused by bortezomib-containing regimen were selected as MM complicated with herpes zoster infection group, and 60 patients with simple MM were selected as MM group, and 60 healthy physical examinees were selected as healthy group. The levels of IFN-γ, IL-6 and IL-4 of three groups were detected by enzyme linked immunosorbent assay(ELISA). The relationship between the expression of IFN-γ, IL-6 and IL-4 in MM patients with herpes zoster infection and the severity of MM complicated with herpes zoster infection was analyzed. 
    Results The levels of IFN-γ in MM group and MM complicated with herpes zoster infection group were lower than those in healthy group, while the levels of IL-6 and IL-4 were higher than those in healthy group; the level of IFN-γ in MM complicated with herpes zoster infection group was lower than that in MM group, while the levels of IL-6 and IL-4 were higher than those in MM group, suggesting significant differences(P<0.05). The level of IFN-γ in patients with severe and moderate herpes zoster was lower than that in patients with mild herpes zoster infection, while the levels of IL-6 and IL-4 were higher than those in patients with mild herpes zoster infection; The level of IFN-γ in patients with severe herpes zoster was lower than that in patients with moderate herpes zoster, while the levels of IL-6 and IL-4 were higher than those in patients with moderate herpes zoster, and the differences were statistically significant(P<0.05). Correlation analysis showed that IFN-γ was negatively correlated with the severity of MM complicated with herpes zoste rinfection, while IL-6 and IL-4 were positively correlated with the severity of MM complicated with herpes zoster infection(P<0.05). Logistic regression model analysis showed that IFN-γ, IL-6 and IL-4 were the risk factors of MM complicated with herpes zoster infection. IFN-γ was negatively correlated with IL-6 and positively correlated with IL-4, and IL-6 was positively correlated with IL-4(P<0.05). The diagnostic value of the three in combination in MM complicated with herpes zoster infection was higher than that of IFN-γ, IL-6 and IL-4 alone(P<0.05). 
    Conclusion The decreased expression of IFN-γ and increased expression of IL-6 and IL-4 can aggravate the abnormal immune function and inflammatory response of MM patients with herpes zoster infection. In clinical practice, changes in the levels of IFN-γ, IL-6 and IL-4 can be used to judge the severity of MM complicated with herpes zoster infection.

    Resveratrol ameliorates oxidative stress damage of human granulosa cells induced by cyclophosphamide by regulating SIRT1

    Objective To investigate the role of resveratrol(RES) in ameliorating 4-hydroperoxy cyclophosphamide(4-HC)-induced oxidative stress damage in human granulosa cells(GCs). 
    Methods The GCs were collected from patients undergoing in vitro fertilization and embryo transfer(IVF-ET) for in vitro culture. Hematoxylin-eosin staining(HE) and immunochemical staining were performed to identify GCs. Cell Counting Kit-8(CCK-8) was used to detect the toxicity of 4-HC-induced GCs and the improvement effect of RES on the toxicity of 4-HC-induced GCs. The GCs were divided into three groups: the control group, 4-HC group and RES+4-HC group. The CCK-8 was used to measure survival rate of GCs, and DCFH-DA fluorescent probe was used to detect reactive oxygen species(ROS) in GCs. Water-soluble tetrazolium-1(WST-1) was used to detect superoxide dismutase(SOD), and ammonium molybdate method was used to detect catalase(CAT). The protein expression of Silent Information Regulator 2-related enzyme 1(SIRT1) was determined by Western Blot. 
    Results GCs were treated with 4-HC( 0, 1.0, 2.5, 5.0, 7.5 μmol/L). CCK-8 results showed the survival rate of GCs decreased with the increase of 4-HC, which showed significant differences, as compared with the control group(P<0.05). 2.5 μmol/L 4-HC was selected as the subsequent concentration of 4-HC model. RES(0, 5, 10, 25, 50, 75 μmol/L) was used to treat GCs for 2 h and then added with 2.5 μmol/L 4-HC. The results showed that RES improved the survival rate of GCs, suggesting significant differences compared with the 4-HC group(P<0.05). 50.0 μmol/L RES was selected as the subsequent improvement concentration. The detection results of GCs after treatment in the three groups showed that survival rate of GCs, SOD and CAT decreased, and ROS increased in 4-HC group, compared with CON group(P<0.05). Compared with the 4-HC group, the survival rate of GCs, SOD and CAT in the RES+4-HC group significantly increased, and the ROS significantly decreased. Western blot showed that SIRT1 in the RES+4-HC group significantly increased, suggesting significant difference, compared with the other two groups(P<0.05). 
    Conclusion RES possibly ameliorates 4-HC-induced GCs damage by increasing SIRT1.
