Objective To explore the effects of pitavastain on the role of oxidative stress in a rat model of myocardial ischemia-reperfusion,which will provide a theoretical basis for pitavastain clinical application in myocardial ischemia-reperfusion.Methods A total of 42 male SD rats were randomly assigned to three groups:Sham group,control group (IR group)and experimental group(Statin group).In Sham group and control group,2 mL saline was irrigated into stomach daily for four weeks,0.42 mg/kg pitavastain dissolved in 2 mL saline was given in experimental group.In surgery,we wore line under the coronary artery without ligation in Sham group,and ligated the line for 30 minutes and then released it for 2 hours in IR group and Statin group.The levels of CK-MB were discerned after operation.Some myocardium tissue specimens under the line for ligation were used to detect the activity of total superoxide dismutase (T-SOD).The myocardial infarction size and risk area of each group were analyzed through comparing the differentiation of myocardium tissue stained by triphenyl tetrazolium chloride(TTC)and Evansblue by computer software system.Results The levels of CKMB elevated and TSOD contentdecreased significantly in control group and experimental group as compared with Sham group(P <0.01).In IR group,the level of CKMB was significantly higher than that of statin group,butlower when comes to TSOD content(P <0.01).There were no statistical significance betweenstatin group and IR group compared with area at risk(P > 0.05).Statin group significantlyreduced infarct size compared with IR group(P <0.01).Conclusion Pitavastatin could increaseantioxidant capacity and it took a positive role in ischemiareperfusion myocardial injuryprevention.

Objective To study the predictive value of prealbumin(PA)and albumin level for prognosis of patients with severe chronic obstructive pulmonary disease(COPD).Methods The study selected 107 patients with severe COPD,the serum protein of initial and 72 hours in intensive care unit was detected.According to the initial albumin levels,the patients were divided into albumin < 30 g/L group and albumin ≥ 30 g/L group,comparison was made in acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score,initial albumin level,the time of mechanical ventilation,and the case fatality rate of 28 days of two groups.After 28 days,outcome was divided into death group and alive group,observation was performed on the dynamic change of serum protein level.Results The APACHE Ⅱ score,mechanical ventilation time and case fatality rate of 28 days in albumin ≥ 30 g/L group were significantly lower than those in albumin <30 g/L group(P <0.01).Initial PA level of albumin ≥30 g/L group was significantly higher than that of albumin <30 g/L group(P <0.01).PA level of survival group was higher than that of death group at 0 h and 72 h(P <0.01).PA level of survival group at 72 h was above the 0 h(P < 0.01).The albumin level of survival group at 72 h showed no significant difference as compared with that of 0 h(P >0.05).PA and albumin level of death group at 72 h also showed no significant difference as compared with those of 0 h(P >0.05).Conclusion Serum protein level is an important predictor of severity and prognosis for severe COPD patients.

Objective To assess fluid responsiveness in mechanically ventilated patients with septic shock by central venous pressure(CVP)and global end diastolic volume index(GEDVI). Methods Fifty-five mechanically ventilated patients with septic shock underwent 55 liquid load tests(LLT),within 30 minutes,the subjects received an intravenous infusion of 6 ml/kg of 6%hydroxyethyl starch(200/0.5).The CVP was monitored through a catheter introduced via the right internal jugular vein and the subclavian vein.The cardiac index(CI)and GEDVI before and after fluid loading were monitored by pulse-induced contour cardiac output(PiCCO).Positive fluid responsiveness was defined as an increase in CI(△CI)by 15% or more.Hemodynamic changes before and after LLT were observed and the correlations between CVP,GEDVI and △CI were assessed.Results The change rate of CVP after volume treatment of responders was higher than that of non-responders,but there was no significant difference (P > 0.05 ).The change rate of GEDVI after volume treatment of responders was higher than that of non-responders,there was significant difference(P <0.01).CVP and GEDVI before the resuscitation showed increase after the resuscitation.There was no significant correlation between △CVP and △CI,while there was positive correlation between △GEDVI and △CI(r =0.816,P <0.01).Conclusion Compared with CVP,GEDVI can better reflect cardiac preload in septic shock patients,and predict fluid responsiveness.

Objective The study shows that puerarin regulates pathway of metabolism in osteoblasts and provides the theoretical foundation for treating methods of postmenopausal osteoporosis.Methods Culture of primary calvarial osteoblasts in rats were digested,and used in experiments.Then,the cells were treated with different concentrations of puerarin(5,10,25,50, 100 μmol/L)in different groups,respectively.The methylation status of CpG islands in estrogen receptor α(ERα)gene promoter region of genome DNA was analyzed by nested-methylation-specific PCR;ER-α gene mRNA expression was examined by semiquantitative reverse transcription PCR analysis.The proliferation and viability of osteoblasts were assayed by MTT assay using the spectrophotometric method.Furthermore,the differentiation of osteoblast-like cells by p-nitrophenyl phosphate were identified.Results The cultured osteoblasts treated by puerarin with different concentrations reduced in de novo methylation in promoter region of ERαgene with a concentration-treating manner,and a dose-dependent raising effect on osteoblasts proliferation and differentiation (P < 0.01 ).Meanwhile,ER-α gene mRNA expressions in osteoblasts treated by puerarin with different concentrations became stronger and stronger,until in the 25 μmol/L treating group,their expressions were the stongest(P < 0.01 ).Conclusion Puerarin reduces DNA methylation in promoter region of ERα in osteoblasts and enhences ERαgene mRNA expressions in osteoblasts.Above results possible influence its proliferation and osteoblastic differentiation.

Objective To evaluate the correlation of hemoglobin A1 c (HbA1 c),fasting blood glucose(FBG),age and serum CA199 levels in diabetes mellitus (DM) patients.Methods According to the normal range of CA199 levels(CA199<27 kU/L),508 DM patients in the Third Hospital of Tangshan were divided into two groups,normal group and elevated group.The indexes of two groups were compared.Single factor correlation analysis Pearson was performed for the correlation of the continuous variables.Multiple liner regression analysis was performed for the quantitative relationship between CA199 and risk factors.Results The age,HbA1 c,FBG and 2 hour blood glucose of elevated group were higher compared with those of normal group (P <0.05).Single factor correlation analysis Pearson showed that serum CA199 was positive correlated with HbA1 c,FBG and age(r =0.152,0.120,0.150,P <0.01).Multiple linear stepwise regression analysis showed that HbA1 c,FBG and age were risk factors of serum CA199.Conclusion Serum CA199 levels of DM patients were closely related with poor glucose control and age.

Objective To study strontium change of patients with chronic kidney disease (CKD).Methods The serum strontium was measured in 254 CKD patients and 40 healthy volunteers.Results According to creatinine clearance rate,CKD patients were divided into four groups,the serum strontium levels of different groups of CKD patients were significantly higher than that of control group(P <0.01),but there were no significant difference between the groups of CKD patients(P >0.05).ROC curve was 40.25 μg/L for serum strontium to predict the critical value of CKD.Conclusion Patients with CKD can lead to the accumulation of strontium;Strontium level may have a positive meaning for the early screening of CKD.

Objective To investigate the effect of initiation time on renal replacement therapy(RRT) for clinical prognosis of patients with non diabetic end stage renal disease (NDNESRD).Methods A total of 89 NDNESRD patients who started the first dialysis were classified into early-start group or late-start group according to estimated glomerular filtration rate(eGFR).The primary outcome was patient survival,and the secondary outcomes were hospitalization,cardiovascular events,vascular access complications,change of dialysis modality, and peritonitis.The follow-up period was five years.Results The survival rate showed no significant difference in the first,the third and the fifth years between the two groups(P =0.166, 0.088,0.067 ).Hospitalization (35.7% vs 38.3%,P = 0.667 ),cardiovascular events (14.3% vs 10.6%,P = 0.089),vascular access complications (21.2% vs 28.1%,P = 0.682)and peritonitis (22.2% vs 26.7%,P =0.541)were no difference between the two groups.Conclusion There was no significant clinical benefit comparing early versus late initiation of RRT.

Objective To explore the effect of health education path in the patients with resection of giant pituitary neoplasms via transsphenoidal approach with endoscope.Methods Sixty-two patients were randomly and equally divided into two groups.Control group received conventional method,while experimental group received health education path.Before discharge, between the two groups postoperative complications and satisfaction were compared.Results Before discharge,the complications occurrence rate of experimental group was lower than that in the control group,the satisfaction of experimental group was higher than that of control group (P <0.05).Conclusion Health education path is a practical way for the giant pituitary neoplasms patients with endoscopic surgery.It can effectively reduce the complications incidence and improve the patient satisfaction levels.

Objective To investigate the safety,cancer radical and early efficacy after laparoscopic gastric cancer resection D2.Methods Seventy-two cases of gastric cancer patients were randomly divided into two groups according to the admission order,each group 36 cases.The control group was given traditional open gastric resection,while the observation group took laparoscopic gastric D2 resection.The early efficacy,tumor radical and surgical safety of two groups were compared.Results The operation time of observation group was significantly longer than that of control group.The incision length,blood loss,recovery time of gastrointestinal function,ambulation,and the average hospital stay time of observation group were significantly less than those of control group(P <0.05 or <0.01).The number of lymphadenectomy,the length of near and far cutting edge showed no significant difference between two groups(P >0.05).The postoperative complication rate of observation group 11.11%(4/36)was significantly lower than that of control group 27.78%(10/36)(P <0.05).Conclusion Laparoscopic gastric D2 resection has good early effect,safe and small surgical trauma,which are conducive to the recovery of patients,the method has good clinical application value.

Objective To examine the effect of continuous irrigation and drainage through self-made double pelvic drainage tube in treatment of anastomotic fistula for reference in clinical research.Methods The self-made double pelvic drainage tube was positioned in advance under the anastomotic to prevent fistula in patients undergoing laparoscopic low anterior resection.Timely and continuous irrigated the pelvic when the fistula happened.The observation mainly included fluids,quantity,clinical symptom,the time of fistula and recovery.Results Under the continuous irrigation and drainage through self-made double pelvic drainage tube,9 of 198 patients after laparoscopic low anterior resection all recovered.Compared with the conventional intervention,the method shortened the healing time of fistula,and the mortality decreased.Conclusion The continuous irrigation and drainage through self-made double pelvic drainage tube in treatment of anastomotic fistula is effective and economical.But,there is still lacking of support by randomized controlled trial.

Objective To explore the clinical application value of early microecological treatment with golden bifid on the applying of intubation-surfactant-extubation(INSURE)for premature infants.Methods Eighty-six premature infants with respiratory distress syndrome (RDS)treated by applying INSURE were randomly divided into two groups.Patients of both groups were subjected to routine treatments.Besides,patients of the treatment group were given live combined Bifidobacterium and Lactobacillus tablets,0.5 g,3 times/day.Results There were significant differences in the incidence of feeding intolerance(χ2 =4.693,P <0.05),enough feeding time(t = 2.385,P < 0.05 ),the number of warm brine enema(t = 2.925,P < 0.05 ),birth mass recovery time(t =4.541,P <0.05),and incidence of cholestasis(χ2 =4.671,P < 0.05).But,there were no significant differences in the incidence of necrotizing enterocolitis.The probiotics was not related to the occurrence of sepsis.Conclusion The premature infants with RDS treated by INSURE strategy benefit the implementation of early microecological treatment over to the full amount feeding by gastrointestinal tract,better growth weight,reduce cholestasis occurrence,no side effect was found.It is worth clinical promotion.

Objective To observe the effect of low-temperature plasma cutter combined with pidotimod for pediatric laryngeal papilloma.Methods Twenty-one cases(69 times)of pediatric laryngeal papilloma who had complete follow-up medical records were retrospectively analyzed. According to the treatment method,the patients were divided into CO 2 laser microsurgery combined with pidotimod(group A,n = 34)and low-temperature plasma cutter combined with pidotimod group (group B,n = 35 ).The tumor recurrence and the time to recurrence were compared after treatment between two groups.Results There was no crossing in relapse curve between two groups,which proved comparable.The recurrence rate of group A was significantly higher than that of group B (P <0.05),the tumor recurrence time of group A was significantly shorter than that of group B(P < 0.01).Conclusion Low-temperature plasma cutter combined with pidotimod for pediatric laryngeal papilloma can effectively extend tumor recurrence time and acquire good treatment effect and it is worth promotion in clinical application.

Objective To analyze the effect of sufentanil combined with small doses ropivacaine in spinal anesthesia of elderly patients.Methods Seventy-five patients who were endured with benign prostatic hyperplasia were randomly divided into A,B,C group.Group A was treated with 0.75% ropivacaine 7.5 mg,group B with 0.75% ropivacaine 7.5 mg+sufentanil 5 μg, group C with 0.75% ropivacaine 5.0 mg + sufentanil 5 μg.Anesthesia,circulation,the extent of movements and complications were evaluated during the spinal anesthesia of different groups. Results All the patients obtained successful spinal anesthesia,and there were no patients withdrawn from the study.Group C obtained the most stable MAP value(P < 0.05 ).Group C obtained the most stable HR value (P < 0.05 ).SpO 2 value among the time points showed significant difference(P <0.05),but inter-group,the time point interaction were not statistically significant(P > 0.05 ).VAS among three groups of patients showed statistic significance (P <0.05),inter-group and inter-group interaction · point showed no significant difference(P >0.05). Group C was superior to group A and B,group B was better than group A in the average time to reach the plane,pain retreating T12 time than the duration of sensory block (P <0.01).Conclusion Sufentanil combined with small dose ropivacaine for spinal anesthesia in elderly patients produced effect in short time,the dose is small,the analgesic effect is obvious,and there are no significant side effects.

Objective To compare and observe the variation of pressure on two left-sided double-lumen endobronchial tubes caused by shift off from the ideal position.Methods A total of 41 cases of American Society of Anesthesiologists(ASA)Ⅰ - Ⅲ patients(gender not limited) aged between 29 and 81 years old who had been scheduled for elective thoracic surgery were randomly divided into two groups:Tuoren double-lumen endobronchial tube group(group T,n =21) and Smith double-lumen endobronchial tube group (group B,n = 20 ).After anesthesia induction,group T was inserted orally with the common type of left-sided double-lumen endobronchial tube made by Tuoren Medical Instrument Co.,Ltd.,whereas group B was inserted orally with the corresponding type of Portex? Blue Line? left-sided double-lumen endobronchial tube.Under the visual assistance of fiberoptic bronchoscope,the top edge of the bronchial cuff of double-lumen endobronchial tube was just located under the ridge of the carina,where was also the “0.0 cm position”designed in the present study.The indication cuff was connected with a disposable pressure sensor via three-way stopcock.The bronchial cuff was filled with air via a three-way stopcock in order to achieve a pressure up to 30 cmH 2 O.Then,the tube was retracted outward in 0.5 cm,until to the 2.0 cm position.Meanwhile,the pressure of bronchial cuff at each position was respectively recorded.Results The pressure on the 2.0 cm position was lower than that on the initial position(30 cmH2 O)in both groups(P <0.01).In both groups,when the tube was moved from 0.0 cm to 2.0 cm position,a decrease tendency of the pressure in bronchial cuff was exhibited (P < 0.01 ).Moreover,the amount and rate of decrease in overall pressure of bronchial cuff in group B were more than those in group T(P <0.01).Conclusion When the two kinds of double-lumen endobronchial tubes were shifted from the ideal position,the pressure of bronchial cuff was accordingly reduced.But,the amount and rate of reduction in the pressure of bronchial cuff of Smith double-lumen endobronchial tube were more than those of Tuoren double-lumen endobronchial tube.

Objective To explore the blood supply of rabbit liver metastasis(LM)based on its model by using CT perfusion and immunohistochemical method.Methods VX2 tumor strains were injected in the spleen of 35 New Zealand white rabbits.The LM was confirmed by using CT scan,and then the CT perfusion was performed.The rabbit LM was separated.The immunohistochemical staining was used to observe the expressions of CD34,intercellular cell adhesion moleculer-1(ICAM-1)and α-smooth muscle actin(α-SMA).Results When the average size of LM neoplasm reached approximately to(315.76±24.78)μm,no expressions of ICAM-1, CD34 andα-SMA could be observed,and the ICAM-1 positive cells in the tumor were connected with the normal sinusoidal.CD34 and ICAM-1 positive cells and someα-SMA positive cells could be seen in the size of LM(632.58± 35.46)μm neoplasm,and the ICAM-1 positive cells in the tumor were connected with the normal sinusoidal.The CD34 and α-SMA positively expressed in the size of LM(1 056.32± 124.67)μm neoplasm,and the ICAM-1 positive cells were further reduced;only CD34 and α-SMA positive cells expressed in the average size of LM(2 183.15± 167.32)and LM(5 326.00± 172.51)μm neoplasms,ICAM-1 staining disappeared or only a few ICAM-1 positive cells could be found around the sinusoidal tumor or normal sinusoidal;the size of 1.0 cm metastases expressed the CD34,α-SMA and ICAM-1,and the staining was similar to the previous stage.LM showed ring staining.CT perfusion of LM of hepatic arterial fraction,hepatic arterial perfusion,hepatic arterial index were greater than the perlesion liver tissue,while portal vein perfusion and portal venous perfusion index were less than the perlesion liver tissue. Conclusion The early stage of rabbit LM was supplied by portal vein blood,with the increase of LM,the hepatic artery increased gradually,the portal vein blood supply decreased gradually.When LM developed to a certain stage,only the hepatic artery,portal vein does not participate in the blood supply.

Objective To analyze the cell count of cerebrospinal fluid in tuberculous meningitis(TBM)and explore the association of test positive for Mycobacterium tuberculosis by real time fluorescent quantitative PCR(FQ-PCR)and cell count.Methods The study selected 81 patients with tuberculous meningitis.Cerebrospinal fluid cell was counted by automatic blood-body fluid cytoanalyzer,cell smears of cerebrospinal fluid were made by centrifugal machine and cells were classified by image analysis system of optical microscope.Mycobacterium tuberculosis in cerebrospinal fluid was detected by FQ-PCR and its correlation with cell count was analyzed. Results The positive rate of M.tuberculosis DNA(TB-DNA)by FQ-PCR method was 50.6%(41/81).The leukocyte count of FQ-PCR positive group was higher than that of the negative group(P <0.05).The leukocyte number was positively correlated with the copy number of FQ-PCR.The characteristics of cerebrospinal fluid cytology in FQ-PCR positive group indicated that the mixed cellular reaction was dominant,and the lymphocytes ratio was 0.590,while neutrophils ratio was 0.366.Conclusion The cerebrospinal fluid cytology of tuberculous meningitis is mostly lymphocyte reaction,cerebrospinal fluid leukocyte count in combination with PCR can reflect the severity of disease and has certain significance for the early diagnosis of tuberculous meningitis.
