    Objective To compare the stress distribution of posterior pelvic ring fixed with minimally invasive adjustable plate(MIAP)combined with one iliosacral(IS)screw and two IS screws.Methods Five embalmed specimens of adult pelvis were used.The soft tissue was removed from the specimens,and the spines from the fourth lumbar vertebra to the proximal one-third of both femurs were retained.The pubic symphysis,bilateral sacroiliac joints and ligments, bilateral hip joints,bilateral sacrotuberous ligaments and sacrospinous ligaments were intact.The strain gages were adhered to 32 sites distributed symmetrically.The specimens were placed in the biomecanical machine at a standing neutral posture.Vertical load up to 520 N was applied and the strain value of intact pelvic ring was recorded.The Tile C pelvic fractures were made.The symphysis pubis was fixed with plate,and the fracture of posterior pelvic ring was fixed with two kinds of internal fixators.The strain changes were recorded respectively.Results The symmetrical sites of the intact pelvic ring had similar strain changes(P >0.05).And the strain concentration sites ran along the sacrofemoral arch.Compared with the strain value of the intact pelvis,the strain changes of these sites(except site 5,6,10,11)in the pelvic fractures fixed with two kinds of internal fixation were not statistically significant(P > 0.05).Each strain value in the posterior pelvic ring fixed with MIAP combined with one IS screw was close to that in intact pelvis under 500 N vertical load.Conclusion In the intact pelvic ring,the conduction of vertical load runs along the sacrofemoral arch.The posterior pelvic ring fixed with MIAP combined with one IS screw can achieve better restoration of mechanical conduction than that fixed with two IS screws.

    Objective To investigate the relationship between acute myocardial infarction (AMI)patients with percutaneous coronary intervention (PCI)and no reflow.Methods One hundred and sixteen patients with AMI were given two grade coronary heart disease prevention and anticoagulation before operation.The culprit vessel distal occlusion was confirmed by surgery PCI.Fifty-nine patients with AMI in group A:the lesions were firstly dilated by small balloon and low pressure,then planted with stents.Fifty-seven in patients with AMI in group B:the lesions firstly dilated by small balloon and low pressure,then changing bigger balloon and high pressure dilated again,at last planted with stents.Results The incidence of no reflow in group A was 6 cases.The incidence of no reflow in group B 14 cases.Conclusion The balloon dilation number and expansion intensity in patients with AMI increase the incidence of no reflow.The more intensity the balloon dilation,the high the incidence of no reflow.

    Objective To explore the influence of different doses rosuvastatin on peroxidation and proliferation of vascular smooth muscle cells for experimental atheriosclerosis rats.Methods Forty-six male rats were divided randomly into five groups,control group(n =6) was given with ordinary feed,high methionine group(n =10)with 2% methionine on this basis, folic acid group(n =10)with high methionine feed after four weeks,adding folic acid for lavage by 0.8 mg/kg per day,low dose group(n = 10)and high dose group(n = 10)with high methionine feed after four weeks,adding rosuvastatin for lavage by 1 mg/kg per day,2 mg/kg per day, respectively.The rats were executed after last lavage for four hours to get blood and carotid artery.Plasma homocysteine (Hcy),content of plasma malondialdehyde (MDA),activity of glutathione peroxidase (GSH-PX) and inflammatory factors were detected,and the relative thickness of carotid artery walls was measured.Results At 12 weeks after modeling,the level of plasma Hcy was significantly increased than before modeling,the order was high methionine group< folic acid

    Objective To investigate the prevalence and risk factors of fatty liver(FL),and clarify the relationship between FL and other components of metabolic syndrome among medical staffs in the general hospitals of Shijiazhuang.Methods A retrospective study was performed to analyze the prevalence and related risk factors of FL in 1 428 medical stuffs who continuously participated in health examination in the Third Hospital of Hebei Medical University.Background variables for the analysis included gender,age,body mass index (BMI),laboratory tests and abdominal ultrasound examination.Results The FL prevalence was increasing with age before 70 years old and higher in male than female among the participants younger than 60 years old(χ2=185.787,P <0.01).Logistic multivariant regression analysis indicated that the incidence of FL was correlated to gender,age,BMI,systolic pressure,diastolic blood pressure,fasting blood sugar, total cholesterol and triglycerides.Conclusion There is a high incidence of FL in the medical staffs of general hospitals of Shijiazhuang,and BMI,blood glucose and triglycerides correlated with FL prevalence.BMI is an independent predictor for morbidity and recovery of FL.

    Objective To investigate the regulation of interleukin-24 (IL-24 )indouble-stranded RNA-dependent protein kinase(PKR),eukaryotic initiation factor 2α(eIF-2α)-induced apoptosis of glioma cells.Methods Adenovirus Ad5F35-IL24(Ad-IL-24)and the control virus Ad5F35-enhanced green fluorescent protein (Ad-EGFP)were transfected into U87 and U251 glioma cells.Then,U87 and U251 glioma cell apoptosis were tested using flow cytometry,and the expression of PKR,phosphorylation-PKR(pPKR),eIF-2αand phosporylation eIF-2α(peIF-2α)in U87 and U251 of different groups were detected by western blot.Results The cell apoptosis rates were significantly increased to 12.3% and 13.0% in U87 and U251 glioma cells after being transfected with IL-24,and the expression of PKR,pPKR,eIF-2α and peIF-2α increased after being transfected with IL-24 in glioma cells.Conclusion IL-24 may regulate PKR,eIF-2αexpression and activation to promote apoptosis of glioma cells.

    Objective To evaluate the short-term therapeutic effect of pedicle screw internal fixation plus inter-lamina bone grafting with calcium phosphate artificial bone and cancellous bone (CPC).Methods Twenty-six patients with thoracolumbar burst fracture were treated in our hospital.The changes of Cobb angle,loss of correction and postoperative back pain were observed 1 week,3 months,6 months,12 months,15 months after operation.Results All patients were followed up.At five different periods of follow-up,vertebral Cobb angle and anterior vertebral height significantly improved,and the back pain significantly eased(P <0.01).However,when the internal fixation had been removed at 15 months after operation,the vertebral collapse happened again in some patients,accompanied by varying degrees of kyphosis which caused back pain. Conclusion Pedicle screw internal fixation plus inter-lamina bone grafting with calcium phosphate artificial bone and CPC improved the stability of fractured verterbra,restored the height of verterbra.However,vertebral collapse in some patients occurred again after internal fixation were removed,leading to the possibility of delayed thoracic back pain and other long-term complications.A variety of bone grafting methods should be considered to treat thoracolumbar burst fractures.

    Objective To investigate the value of tuberculous infection of T cells spot test (T-SPOT.TB)in the diagnosis of osteoarticular tuberculosis.Methods The study enrolled 80 patients who were confirmed osteoarticular tuberculosis and 44 did not have active tuberculosis. Acid-fast staining,mycobacterial culture,real-time quantitative PCR were respectively performed for the patients.The results were compared and analyzed.Results The sensitivity,specificity, accuracy for osteoarticular tuberculosis diagnosis by T-SPOT.TB assay were 82.5%,75.0% and 79.8%.The positive rate of T-SPOT.TB was significantly higher than that of acid-fast staining, mycobacterial culture(P < 0.05).But,there were no statistical difference in different locations (spine or extremities joint)and courses between groups.Conclusion The results suggested that T-SPOT.TB assay could provide useful aids in the diagnosis of osteoarticular tuberculosis.

    Objective To investigate the risk factors of postoperative hospital infection of gallstone patients.Methods The study enrolled 193 cases of gallstone patients who underwent operation in hospital and the patients fell into two groups according to hospital infection:83 cases of postoperative hospital infection and 110 cases of postoperative non hospital infected at the same time as control.The gender,age,smoking,drinking and operation condition were compared between two groups.Results Compared with non hospital infected group,the age above 60 years old,BMI over 25.0,drinking,smoking,the length of incision ≥ 15 cm,operation time over 2 h, hospital stay over 10 days,blood lose over 200 mL,over 3 kinds of antibiotics being administrated and bilirubin > 17.1 μmol/L were highly closed with postoperative infection.Conclusion Risk factors of gallstone postoperative hospital infection should be prevented and controlled and combined with measures of hospital infection management to reduce the incidence of gallstone postoperative infection.

    Objective To determine the role of translaryngofissure CO 2 laser partial laryngectomy in laryngeal carcinomas.Methods The clinical data of 26 patients with laryngeal carcinoma,whose conditions were not for self-retaining microlaryngoscopic surgery.We did translaryngofissure partial laryngectomy by using CO 2 laser. Results Compared translaryngofissure CO2 laser partial laryngectomy with traditional partial laryngectomy,the average operation time was shortened by 20 minutes,reduced the laryngeal mucosa edema postoperatively.All patients were followed up for five years.The recurrence rate was significantly lower,without implantation metastasis.Conclusion Translaryngofissure CO 2 laser partial laryngectomy in laryngeal carcinoma has many advantages,for example shorter operation time, shorter laryngeal mucosa edema postoperatively.

    Objective To explore the clinical characteristics and curative effect of neonatal aspiration pneumonia.Methods Seventy-eight cases with neonatal aspiration pneumonia were randomly divided into three groups,each group 26 patients,C group with conventional treatment of neonatal aspiration pneumonia,A group with ambroxol hydrochloride on the basis of C group, and B group with budesonide and ambroxol hydrochloride treatment on the basis of C group.The total efficiency,the clinical symptoms and signs disappeared time,hospital stay,the incidence of adverse reactions of three groups were compared after seven days of treatment.Results The total efficiency in A group(92.31%)and B group(96.15%)were higher than that in C group(76.92%). The difference were statistically significant among three groups(P < 0.05).Breathing difficulty disappeared time,Cyanosis disappeared time and lung rale disappeared time of A group and B group ware earlier than those of C group.And the effect of B group was better than C group. Conclusion Ambroxol(A group)or budesonide jointed ambroxol(B group)in the treatment of neonatal aspiration pneumonia has achieved better clinical results,and improved the clinical signs and symptoms,the medication is safe and reliable,which will help the quick recovery of the children.

    Objective To compare the efficacy of lumbar plexus/sciatic nerve block using peripheral nerve stimulator with epidural anesthesia in the unilateral femoral head replacement surgery of elderly patients.Methods Forty patients,ASA physical status ofⅡ - Ⅲ,undergoing the unilateral femoral head replacement surgery were randomly divided into two groups:group N (lumbar plexus/sciatic nerve block group,n = 20);group E(epidural anesthesia group,n = 20). The patients in group N received lumbar plexus/sciatic nerve block using peripheral nerve stimulator.The local anesthetic drug was the mixed solution of 0.3% ropivacaine and 0.3%lidocaine,and the amount of drug for lumber plexus block was 30- 35 mL and 25- 30 mL for sciatic plexus block.The patients in group E received epidural anestheisa and epidural catheter headward.The local anesthetic drug was the mixed solution of 1% lidocaine and 0.5% ropivicaine, 6-8 mL would be given through epidural catheter after test dose.The blood pressure and heart rate were measured before anesthesia,5,10 min after anesthesia,operations beginning and 30 min later(T0 - T4 ).Anesthesia procedure time,adverse reactions and postoperative complications of two groups were recorded.Results The mean artery pressure (MAP) of T2 (10 min after anesthesia)and T3 (operation beginning)in group E were decreased than those of baseline(P <0.01);compared with group N,MAP of T2 and T3 in group E were lower.The heart rates were stable in both groups.Anaesthesia effect of two groups had no significant difference(P >0.05). The complications of group N were less than those of group E (P < 0.01 ).Conclusion The anesthetic strategy of lumbar plexus/sciatic nerve block using peripheral nerve stimulator will lead to precise anesthesia effect.Compared with epidural anesthesia,nerve block had stable hemodynamics and a few complications.

    Objective To investigate the use of carotid ultrasonography in the intensity-modulated radiation therapy(IMRT)of upper esophageal cancer.Methods Sixty-eight patients with upper esophageal cancer were divided into group A(experimental group,34 cases)and group B(control group,34 cases).In group A,the areas were treated by IMRT on condition that lymph nodes metastasis was found.Both groups were performed by IMRT through a 5 field IMRT plan. Results There were no significant differences between two groups in total effective rate(P >0.05).The first and second year′s survival rate in group A and group B were no significant differences(P >0.05).The 3 years'rate in group A was distinctly higher than that in group B(P <0.05).The side effects of radiotherapy were mainly classified into grade 1 and 2.In addition,no significant differences were noted between the two groups.The fatality rate in group A was remarkably lower than that in group B(P < 0.05).Conclusion Carotid ultrasonography can be used as a basis of neck irradiation in IMRT of upper esophageal carcinoma,which can improve the 3 years′ survival rates with satisfactory recent curative effect and several tolerable toxic side effects.

    Objective To evaluate united test significance in the diagnosis of tuberculous meningitis (TBM)with modified acid-fast staining method of cerebrospinal fluid and real-time fluorescence quantitative polymerase chain reaction (FQ-PCR).Methods Cerebrospinal fluid samples from 81 patients with TBM were detected for Mycobacterium tuberculosis respectively with centrifugal modified acid-fast staining method and real-time FQ-PCR.Results The positive rate of modified acid-fast staining 66.7%(54/81)was significantly higher than that of FQ-PCR 50.6%(41/81),there was significant difference between the two methods(P <0.05).Conclusion Modified acid-fast staining method is significantly better than FQ-PCR in the diagnosis of TBM.

    Objective To investigate the bacteria distribution of neonatal purulent meningitis,antibiotic sensitivity and resistance,and provide laboratory evidence for clinical diagnosis and treatment.Methods Twenty-eight cases diagnosed purulent meningitis were recruited into this study.The bacterial culture,drug resistance and sensitivity of cerebrospinal fluid from the patients were analyzed.Results The pathogen distribution in the cerebrospinal fluid of 28 cases with neonatal purulent meningitis:27 cases with cultured single strain,just 1 case with two strains.In this findings,there were 20 gram-negative bacteria strains (68.96%),8 gram-positive bacteria strains (27.59%),1 candida albicans strain (3.45%).The drug resistant rate of gram-negative bacteria to ampicillin,cotrimoxazole,piperacillin was more than 80.00%;on the other hand,it was up to 60.00% to cefazolin,tetracycline,cefotaxime.Eight strains of extended spectrum beta lactamase bacteria were tested in this study.Gram-negative bacteria resistant to meropenem and amikacin were not detected.Most gram-positive bacteria showed drug resistance to ampicillin,cotrimoxazole,erythromycin,cefoxitin.There was none detected in gram-positive bacteria resistant to vancomycin.Conclusion Gram-negative bacteria was the major pathogens in neonatal purulent meningitis,and they were resistant to multiple antibiotics.

    Objective To understand the composition of the common pathogens in blood culture and a variety of pathogens to antimicrobial agents in the hospital.To offer the basis for the rational use of antimicrobial drugs to solve the patients'pain.Methods The collection involved 1 764 blood samples.France bioMérieux BacT/Alert3D fully automatic blood culture instrument, supporting medium,identification system were used to identify the pathogens and susceptibility test.Results A total of 1 764 blood samples showed the culture positive rate 10.1%,and there were 179 isolated positive bacteria,among which gram-positive bacteria were 68 (38.0%),gram-negative bacteria 111 (62.0%),at the first place of pathogens as E.coli followed by coagulase-negative bacteria,Klebsiella pneumonia,Pseudomonas aeruginosa.The susceptibility results showed that most of the detected pathogens were resistant to different anti-bacteria agents. Conclusion Clinicians should pay attention to early blood culture of patients,and strictly stick to the rational use of antimicrobial agents based on susceptibility results.

    Objective To investigate the situation of obesity in the children with the age 7 to 13 years from Yuhua district,Shijiazhuang city to provide the basis for prevention and control of obesity.Methods The study enrolled 11 475 students with the age 7 to 13 years(1 year as a year age group)from 19 primary schools in Yuhua district as population,randomized by cluster random sampling methods and based on “BMI standard category for Chinese school children, adolescent used to screen obesity and overweight”to investigate and summarize the obesity situation within children.Results There were significant difference in different age groups(P <0.05).In the age groups(7 to 12,six groups),the obesity rate of male children was respectively higher than that of female children in the age groups(P <0.05).Conclusion There is high obesity rate for the male children with 7 to 12 years from Yuhua district,Shijiazhang city,more attention should be concerned and intervented as soon as possible.
