正畸治疗对牙龈蛋白R变化的影响及引发牙龈炎症的风险分析

〖HTH〗目的〖HTSS〗分探讨正畸治疗对牙龈蛋白R(gingival protein R,Rgp)变化的影响作用,分析牙龈炎发生的风险。
〖HTH〗方法〖HTSS〗随机抽取进行正畸治疗的118例青少年患者,分别取矫正前及矫正后第1,2,3,6 个月的龈沟液,采用16S rDNA PCR 技术检测牙龈卟啉单胞菌(Porphyromonas gingivalis,P.g)和Rgp;比较各组间的P.g、Rgp检出率,分析牙龈炎与P.g、Rgp检出率之间的关系。
〖HTH〗结果〖HTSS〗 118例患者矫正前和矫正后1,2,3,6个月P.g检出阳性率依次为29.66%、53.39%、72.03%、60.17%、38.14%;与矫正前比,矫正后1,2个月P.g检出阳性率逐渐升高,矫正后3个月开始下降(P<0.05)。矫正前和矫正后1,2,3,6个月P.g检出阳性中Rgp检出阳性率依次为34.29%、53.97%、69.41%、67.61%、42.22%;与矫正前比,矫正后2,3个月Rgp检出阳性率呈升高趋势(P<0.05)。牙龈指数与P.g、Rgp检出阳性率之间均存在正相关(rs=0.352、0.461,P<0.05)。
〖HTH〗结论〖HTSS〗正畸治疗早期,矫治器可使P.g、Rgp增加,第3个月开始下降,第6个月略高于矫正前,且在正畸治疗期间应当重视口腔卫生,有利于预防牙龈炎的发生。

Abstract: [Abstract] Objective〖HTSS〗To investigate the effect of orthodontic treatment on the changes of gingival protein R(Rgp) and to analyze the risk of gingivitis.
〖HTH〗〖WTHZ〗Methods〖HTSS〗A total of 118 adolescent patients who underwent orthodontic treatment were randomly selected. The gingival crevicular fluid was taken before, after, and at 1, 2, 3, and 6 months after correction. The 16S rDNA PCR technique was used to detect Porphyromonas gingivalis(P.g) and Rgp. Chi-square test was used to compare the detection rates of P.g and Rgp between groups, and Spearman rank correlation analysis was used to analyze the relationship between gingivitis and P.g and Rgp detection rates.
〖HTH〗〖WTHZ〗Results〖HTSS〗Among the 118 subjects, the positive rate of P.g detection before, after, 1, 2, 3, and 6 months after correction was 29.66%, 53.39%, 72.03%, 60.17%, 38.14%, compared with the pre-correction, after correction. The positive rate of P.g detection increased gradually in the first month and 2 months, and decreased in the third month(P<0.05). The positive rate of Rgp in the positive detection of P.g before and after correction, 1, 2, 3, and 6 months after correction were 34.29%, 53.97%, 69.41%, 67.61%, 42.22%. Compared with the pre-correction, the positive rate of Rgp detection increased gradually after 2 and 3 months of correction(P<0.05). There was a positive correlation between gingival index and detection time, P.g. and positive rate of Rgp(rs=0.352, 0.461, P<0.05).
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗In the early stage of orthodontic treatment, the appliance can increase P.g and Rgp, and begin to decline in the third month. The sixth month is slightly higher than before correction, and oral hygiene should be emphasized during orthodontic treatment, which is conducive to the prevention of gingivitis.
