食管癌患者术后应用不同剂量ω3多不饱和脂肪酸的临床效果观察

Clinical effect of different dose of ω3 polyunsaturated fatty acids in postoperative patients with esophagus cancer

摘要: [摘要] 目的 探讨术后早期应用不同剂量 ω-3 多不饱和脂肪酸对食管癌患者炎症因子、营养状况以及术后
并发症的影响。方法 将 92 例食管癌患者按随机数字表法分为对照组 32 例、实验 1 组 30 例和实验 2 组 30 例。所
有患者术后均给予 6d 肠外营养支持治疗,胃肠功能恢复后给予流食。对照组另外给予生理盐水 100mL / d ,实验
1 组给予 ω-3 鱼油脂肪乳注射液 50mL / d 和生理盐水 50mL / d ,实验 2 组给予 ω-3 鱼油脂肪乳注射液 100mL / d ,
疗程均为 1 周。收集并比较 3 组术前 1d 、术后 1d 、术后 7d 降钙素原(
procalicitonin , PCT )、 C 反应蛋白( C-reactive
protein , CRP )、白细胞介素 6 (
interleukin-6 , IL-6 )、白蛋白( albumin , Alb )、淋巴细胞( lymphocyte , LYM )、前白蛋白
(
prealbumin , PA )、转铁蛋白( transferrin , TFN )含量,记录药物不良反应及术后并发症发生情况。结果 术后 7d
时,实验 1 组和实验组 2 组 PCT 、 CRP 和 IL-6 水平均低于对照组( P <0.05 ), 3 组 PCT 、 CRP 和 IL-6 组间、时点间、
组间·时点间交互作用差异均有统计学意义( P <0.05 ),实验 2 组 PCT 、 CRP 和 IL-6 水平均低于实验 1 组(
P <
0.05 ),实验 1 组和实验组 2 组 LYM 高于对照组( P <0.05 ),实验 2 组 LYM 高于实验 1 组( P <0.05 )。对照组发
生切口感染 3 例,肺部感染 3 例,吻合口瘘 1 例;实验 1 组发生切口感染 3 例,肺部感染 3 例;实验 2 组发生切口感
染 1 例。实验 2 组并发症发生率 3.

Abstract: [ Abstract ] Objective Toinvestigatetheeffectofdifferentdosesofω-3polyunsaturatedfatty

esophagealcarcinoma.Methods Ninety-twopatients wtihesophaguscancer wererandomly
dividedintoacontrolgroup ( 32cases ), experimentalgroup1 ( 30cases ) andtheexperimental
group2 ( 30cases ) .Allpatientsweregiven6daysofparenteralnutritionsupporttreatment
, and
thenweregivenliquiddietafterrecoveryofgastrointestinalfunction.Controlgroupwasgiven
saline100 mL / d , the experimentalgroup 1 weretreated with saline 50 mL / d and ω-3
polyunsaturatedfatty acid 50 mL / d , the experimental group 2 were treated with ω-3
polyunsaturatedfattyacid100 mL / d , allthetreatmentcourse wasone week.Procalicitonin

PCT ), C-reactiveprotein ( CRP ) andinterleukin-6 ( IL-6 ), albumin ( Alb ), lymphocyte ( LYM ),

experimentalgroup1 ( P <0.05 ), theLYM ofexperimentalgroup1andexperimentalgroup2
werehigherthanthatofconrolgroup ( P <0.05 ), theLYMofexperimentalgroup2werehigher
thanthatofexperimentalgroup1 ( P <0.05 ) .Therewere3casesofwoundinfection , 2casesof
pulmonaryinfection , 1casesofanastomoticleakageincontrolgroup , 3casesofwoundinfection
inexperimentalgroup1 , 3casesofpulmonaryinfectionand1casesofincisioninfectionin
experimentalgroup2.Theincidenceofcomplicationswas3. 33% ( 1 / 30 ) inexperimentalgroup2 ,
whichwaslowerthanthatincontrolgroup21. 88% ( 7 / 32 ) andtheexperimentalgroup2 ( P <
0.05 ) .Theincidenceofcomplicationswerenotstatisticallysignificant ( P >0.05 ) .Conclusion ω-
3polyunsaturatedfattyacidscanreducetheinflammatoryreactioninpatientswithesophageal
cancer , andimprovethenutritionalstatusofpatientsaftersurgery.Theapplicationoflargedose
ismoreobvious , andcanreducetheincidenceofpostoperativecomplications.
