不稳定心绞痛患者C反应蛋白与斑块成分关系的探讨

摘要: [摘要] 目的 分析冠心病不稳定心绞痛患者血清 C 反应蛋白( C-reactiveprotein , CRP )浓度与血管内超声发
现的斑块成分的相关性。方法 回顾性研究不稳定心绞痛患者 43 例。应用血管内超声及虚拟组织学分析罪犯血
管处斑块成分,并据此分为薄纤维帽斑块组(
n =19 )及稳定斑块组( n =24 )。采用酶联免疫吸附测定法检测血清
CRP ,研究 CRP 与斑块成分的相关性。结果 与稳定斑块组比较,薄纤维帽斑块组最小管腔直径、外弹力膜面积、最
小管腔面积差异无统计学意义;但薄纤维帽组斑块负荷较稳定斑块组显著增大( P <0.05 )。虚拟组织学发现,薄纤
维帽组纤维组织比例较稳定斑块组减少( P <0.05 );而薄纤维帽组坏死核心比例较稳定斑块组明显增加(
P <
0.05 )。 Spearman 相关分析显示, CRP 水平与纤维组织比例呈负相关( r s =-0. 411 , P =0.009 ), CRP 水平与坏死
核心比例呈正相关(
r s =0.421 , P =0.005 )。结论 不稳定心绞痛患者中,薄纤维帽斑块的形成伴随着 CRP 的增
高; CRP 水平与纤维组织比例呈负相关、与坏死核心比例呈正相关。

Abstract: Abstract] ObjectiveTo analyze the correlation between the concentration of serum C reactive protein(CRP) and plaque composition in patients with unstable angina pectoris. MethodsRetrospective analyzed 43 patients with unstable angina pectoris were performed. Intravascular ultrasoundvirtual histology(VHIVUS) examination in a culprit lesion was used. Patients were divided into thincap fibroatheroma(TCFA) group or stable plague group according to VHIVUS. Enzyme linked immunosorbent assay was used to detect serum CRP and to study the correlation between CRP and plaque composition. ResultsThere were no diffirence in minimal luman area, minimal luman diameter, external elastic membrane between the two groups. TCFA group had larger plague burden than stable plague group(P<0.05). Virtual histology analysis showed TCFA group had lower fibrous ratio than stable plague group(P<0.05); and TCFA group had higher necrotic core ratio than stable plague group(P<0.05). Spearman correlation analysis showed that CRP  was positively correlated with necrotic core (rs=0.421, P=0.005) and negatively correlated with fibrous tissue(rs=-0.411, P=0.009). ConclusionThe formation of thin fibrous cap was accompanied with the increase of CRP in patients with unstable angina pectoris. serum level of CRP increased in patients with TFCA. The level of CRP was positively correlated with the ratio of necrotic core and negatively correlated with the proportion of fibrous tissue.
