匹伐他汀对大鼠心肌缺血再灌注后氧化应激作用的影响

摘要: 目的:探讨匹伐他汀对大鼠急性心肌缺血再灌注损伤(myocardial ischemia reperfusion inj ury,MIRI)的保护作用,为他汀类药物在缺血再灌注(ischemia reperfusion,IR)心肌的临床应用提供理论依据。方法将成年雄性 SD 大鼠42只随机分为假手术组(Sham 组)、IR 组及实验组(Statin 组)各14只。Sham 组与 IR 组术前给予生理盐水2 mL/d灌胃4周,Statin 组给予匹伐他汀0.42 mg·kg-1·d-1,溶解成2 mL灌胃4周。制作大鼠心肌 IR 模型:Sham 组冠状动脉下只穿线,不结扎;IR 组、Statin 组结扎冠状动脉左前降支30 min,再松解120 min。术后检测血清中肌酸激酶同工酶(creatine kinase isoenzyme-MB,CK-MB)水平,留取结扎线下部分心肌组织用黄嘌呤氧化酶法测心肌总超氧化物岐化酶(total superoxide dismutase,T-SOD)活性,用氯化三苯基四氮唑(triphenyltetrazolium chloride,TTC)、伊文思蓝双染色法测定各实验组心肌梗死面积及缺血区面积。结果与 Sham 组相比,IR 组、Statin组的血清 CK-MB 升高,T-SOD 含量下降(P <0.01);Statin 组 CK-MB 升高程度不及 IR 组(P <0.01),T-SOD 含量下降程度不及 IR 组(P <0.01)。IR 组和 Statin 组危险区面积差异无统计学意义(P >0.05);与 IR 组相比,Statin组梗死面积明显减小(P <0.01)。结论匹伐他汀可以提高机体抗氧化能力,减轻再灌注后心肌坏死程度,减轻MIRI。

Abstract: Objective To explore the effects of pitavastain on the role of oxidative stress in a rat model of myocardial ischemia-reperfusion,which will provide a theoretical basis for pitavastain clinical application in myocardial ischemia-reperfusion.Methods A total of 42 male SD rats were randomly assigned to three groups:Sham group,control group (IR group)and experimental group(Statin group).In Sham group and control group,2 mL saline was irrigated into stomach daily for four weeks,0.42 mg/kg pitavastain dissolved in 2 mL saline was given in experimental group.In surgery,we wore line under the coronary artery without ligation in Sham group,and ligated the line for 30 minutes and then released it for 2 hours in IR group and Statin group.The levels of CK-MB were discerned after operation.Some myocardium tissue specimens under the line for ligation were used to detect the activity of total superoxide dismutase (T-SOD).The myocardial infarction size and risk area of each group were analyzed through comparing the differentiation of myocardium tissue stained by triphenyl tetrazolium chloride(TTC)and Evansblue by computer software system.Results The levels of CKMB elevated and TSOD contentdecreased significantly in control group and experimental group as compared with Sham group(P <0.01).In IR group,the level of CKMB was significantly higher than that of statin group,butlower when comes to TSOD content(P <0.01).There were no statistical significance betweenstatin group and IR group compared with area at risk(P > 0.05).Statin group significantlyreduced infarct size compared with IR group(P <0.01).Conclusion Pitavastatin could increaseantioxidant capacity and it took a positive role in ischemiareperfusion myocardial injuryprevention.
