面肌痉挛显微血管减压术后延迟治愈临床分析#br#

Clinical analysis of delayed cure after microvascular decompression for hemifacial spasm#br#

摘要: [摘要]
目的  探讨面肌痉挛(hemifacial spasm,HFS)患者行显微血管减压术(microvascular decompression,MVD)后延迟治愈的相关影响因素。
方法  回顾性分析220例HFS治愈患者的临床资料,患者均行MVD治疗,对患者术后延迟治愈的相关影响因素进行分析。
结果  术后176例患者立即治愈,44例患者延迟治愈,随访1年,44例延迟治愈患者治愈时间为10 d~12个月。对术后延迟治愈进行单因素分析,结果显示是否延迟治愈与性别、年龄及责任动脉粗细差异均无统计学意义(P>0.05),2组病史、症状严重程度及异常面肌反应监测差异均有统计学意义(P<0.05)。Logistics回归分析显示,病史、症状严重程度是延迟治愈的影响因素。
结论  病史、症状严重程度是延迟治愈的影响因素,病史越长越容易发生延迟治愈,症状越严重越容易发生延迟治愈;HFS患者MVD后出现延迟治愈较为常见,分析MVD后延迟治愈的影响因素可改善患者的临床症状及预后。

Abstract: [Abstract] Objective〖HTSS〗To explore the related factors of delayed cure after microvacular decompression(MVD) in patients with hemifacial spasm(HFS).
Methods〖HTSS〗Clinical data of 220 HFS patients were conducted the retrospective analysis. All patients were treated with MVD. Relevant influencing factors of delaying cure for patients after operation were analyzed.
Results〖HTSS〗After operation, 176 patients were cured immediately, while 44 patients delayed cure. After one-year follow-up visiting, healing time of 44 patients in the delayed cure group was 10 d-12 month.The analysis of the influencing factors of delayed cure after operation showed that there was no significant difference between delayed cure and gender, age and the size of the responsible artery(P>0.05). There was no significant difference in the history, severity of symptoms and monitoring of abnormal facial muscles between two groups(P<0.05). Logistic regression analysis showed that medical history and severity of symptoms were the influencing factors of delayed cure.
Conclusion  The medical history and severity of symptoms are the influencing factors of delayed cure. The longer medical history is, the easier delayed cure will have. The severer spasm symptoms are, the easier delayed cure will have. Delayed cure after MVD in HFS patients is more common. Analysis of the influencing factors of delayed cure after MVD can improve the clinical symptoms and prognosis of patients.
