显微血管减压术硬膜严密缝合技术改良及疗效分析#br#

Improvement and curative effect analysis of method of dural suturing in microvaseular decompression

〖HTH〗目的〖HTSS〗〖KG*2〗探讨显微血管减压术(microvascular decompression,MVD)开关颅方法的改良对术后并发症发生率的影响。
〖HTH〗方法〖HTSS〗〖KG*2〗回顾性分析行MVD治疗的235例面肌痉挛患者的临床资料。115例采用传统技术(传统技术组)手术;120例采用改良技术(技术改良组)手术,分别在骨瓣成形、硬膜剪开方法、硬膜缝合、骨性结构处理及皮肤缝合等方面进行了改良。比较2组手术疗效、手术时间(开颅时间、颅内操作时间、关颅时间)和术后并发症发生率。
〖HTH〗结果〖HTSS〗〖KG*2〗技术改良组开颅时间和颅内操作时间短于传统技术组,关颅时间多于传统手术组,差异均有统计学意义(P<005)。传统技术组术后并发症发生率20.9%(脑脊液漏4例,听力下降10例,耳鸣8例,面瘫2例),技术改良组术后并发症发生率8.3%(听力下降5例,耳鸣4例,面瘫1例),差异有统计学意义(P<005)。
〖HTH〗结论〖HTSS〗〖KG*2〗显微血管减压术治疗面肌痉挛技术的改良可以更好地显露术野,减少手术操作时间,减少术后并发症的发生。

Abstract: [Abstract] Objective〖HTSS〗〓To investigate the effect of improved microvascular decompression(MVD) on the incidence of postoperative complications.
〖HTH〗〖WTHZ〗Methods〖HTSS〗〓The clinical data of 235 patients undergoing MVD were analyzed retrospectively. Among them, traditional method of skull closing was used in 115 patients and 120 patients received modified method of skull closing. The modifications involved aspects including forming bone window, duradic cutting method,suturing of dual matter, skull reconstruction and suturing of skin. To study the effect, the operation time(craniotomy time, intracranial operation time, closed cranial time) and other complications were compared between the traditional and modified groups.
〖HTH〗〖WTHZ〗Results〖HTSS〗〓 The craniotomy time and intracranial operation time of modified group were shorter than those of traditional group, while the craniotomy time was longer than that of traditional group, and the difference was statistically significant(P<005).The incidence of postoperative complications in traditional group was 20.9%, which included 4 cases of cerebrospinal fluid leakage, 10 cases of hearing loss, 8 cases of tinnitusand 2 cases of facial paralysis. The incidence of postoperative complications in modified group was 8.3%, which included 5 cases of hearing loss,4 cases of tinnitus and1 cases of facial paralysis. There was significant difference in incidence of postoperative complications between two groups(P<005).
〖HTH〗〖WTHZ〗Conclusion〖HTSS〗〓The improvement of microvascular decompression for hemifacial spasm can reveal the surgical perspective, reduce the operation time and the postoperative complications.
