Evaluation of the effect of anti-osteoporosis treatment by quantitative CT#br#

目的  探讨应用定量CT检测骨密度评估抗骨质疏松治疗效果的可行性。
方法  选择绝经后骨质疏松患者,均给予双膦酸盐类抗骨吸收药物治疗。于治疗前和治疗6个月后检测血清β胶原降解产物(β-C-terminal telopeptide of type Ⅰ collagen,β-CTX)、双能X线骨密度及定量CT骨密度,依照血清β-CTX变化情况分为治疗有效组和治疗无效组,比较2组骨密度变化情况。
结果  共纳入患者220例。血清β-CTX含量下降超过50%患者211例为治疗有效组,血清β-CTX含量下降未达到50%患者9例为治疗无效组。口服双膦酸盐类药物治疗6个月后,治疗有效组及治疗无效组双能X线吸收测定法骨密度检测均无明显变化,治疗有效组定量CT骨密度检测提示骨密度升高,治疗无效组则无明显变化。
结论  血清β-CTX检测与定量CT骨密度检测可以对绝经后骨质疏松患者抗骨吸收治疗效果进行评估,且2种检测方式互为补充,为绝经后骨质疏松治疗提供指导。

Abstract: [Abstract]Objective〖HTSS〗To explore the feasibility of quantitative CT in evaluating the effect of anti-osteoporosis treatment.
Methods〖HTSS〗The patients with postmenopausal osteoporosis were treated with bisphosphonate. Serum β-C-terminal collagen of type Ⅰ collagen(β-CTX), dual energy X-ray bone density and quantitative CT bone density were measured before and 6 months after treatment. According to the change of serum β-CTX, the patients were divided into two groups: the effective group and the ineffective group.
Results〖HTSS〗A total of 220 patients were included. 211 patients whose serum β-CTX content decreased by more than 50% were regarded as the treatment effective group, 9 patients whose serum β-CTX content decreased less than 50% were regarded as the treatment ineffective group. After 6 months of oral bisphosphonates treatment, there was no significant change in dualeneopausal X-ray absorptiomtry bone mineral density(BMD) in the effective group and the ineffective group. Quantitative CT BMD in the effective group indicated that BMD increased, but there was no significant change in the ineffective group.
Conclusion〖HTSS〗The detection of serum β-CTX and quantitative CT BMD can evaluate the effect of anti bone resorption in postmenopausal osteoporosis patients, and the two detection methods are complementary to each other, providing guidance for the treatment of postmenopausal osteoporosis.
