MSCTV对下肢静脉血栓性疾病的诊断价值

The diagnosis value of direct multi-slice computed tomography venography in lower extremity thrombosis

摘要: 目的  探讨多层螺旋CT直接法静脉造影(multi-slice computed tomography venography,MSCTV)对下肢静脉血栓性疾病的诊断价值及临床意义。
方法  对临床高度怀疑为下肢静脉血栓性疾病患者48例行MSCTV检查,将原始数据传送到工作站做多种图像后处理,寻找病变的特征性影像学表现,并与数字减影血管造影(digital subtraction angiograpithy,DSA)结果进行比较。
结果   48例患者中,下肢深静脉血栓36例,双下肢同时受累3例,左下肢受累21例,右下肢受累12例;发现髂静脉压迫综合征27例,伴有深静脉血栓18例。以DSA结果做为金标准, MSCTV对下肢深静脉血栓诊断的敏感度为97.2%,特异度为100.0%。
结论  MSCTV可以作为临床诊断静脉血栓性病变的首选方法。

Abstract: Objective  To explore the value and clinical significance of direct multi-slice computed tomography venography(MSCTV) in diagnosing venous obstructive diseases. 
Methods  Forty-eight patients with high clinical suspicion of lower extremity venous thrombotic disease were examined by MSCTV. The original data were sent to the workstation for multiple image post-processing to find out the characteristic imaging manifestations of the disease, and compared with the digital subtraction angiography(DSA) results. 
Results  Among the 48 patients, 36 had deep vein thrombosis of lower extremities, 3 had simultaneous involvement of both lower extremities, 21 had left lower extremity involvement, and 12 had right lower extremity involvement. 27 had iliac vein compression syndrome and 18 had accompanied with deep vein thrombosis.With DSA results as the gold standard, the sensitivity and specificity of MSCTV for the diagnosis of deep venous thrombosis of lower extremities were 97.2% and 100.0%. 
Conclusion  MSCTV may be used as the first choice in diagnosing venous thrombosis of the lower extremity.
