    Objective To observe the change of serum and urine biochemical indexes in kidney damage rats with severe acute pancreatitis (SAP).Methods A total of 240 healthy male Sprague-Dawley rats were selected as the research obj ects,and the rats were randomly divided into eight groups (each group,n=30)by random number table,named as SAP different time group (SAP 1h,3h,6h,9h,12h,18h,24h group)and the control group.The serum amylase (AMY),serum creatinine (SCr),blood urea nitrogen (BUN)and blood uric acid (UA),the urine creatinine (UCr),renal injury molecules (KIM)-1 andβ-N-acetyl amino glycosidase enzymes (NAG)of different groups were compared.Results The levels of SCr,BUN and UA of SAP 1h, 3h,6h,9h,12h,18h,24h group were higher than those of the control group,the differences were statistically significant (P<0.05).The UCr levels of SAP 1h,3h,6h,9h,12h group were lower than those of the control group,the levels of KIM-1 and NAG were higher than those of the control group,the differences were statistically significant (P<0.05).Conclusion In SAP kidney damage,the renal tubular inj ury is earlier than the capillary damage.The KIM-1 and NAG are the renal tubular damage j udge indexes,the sensitivity and specificity were better than Cr,BUN and UA for early diagnosis of SAP kidney damage.

    Objective To investigate the expression of human mut-s homologue 2 (hMSH2)and phosphatase and tensin homology deleted on chromosome ten (PTEN)in sporadic gastric carcinoma,and thereby to elucidate their relationship and clinical significance.Methods The expression levels of hMSH2 and PTEN were detected using immunohistochemistry in 42 cases with sporadic gastric carcinoma,its corresponding adjacent tissues (3cm away from cancerous tissues),distal tissues (from cancer>10cm)and 15 cases of normal gastric tissues.The expression levels of hMSH2 and PTEN protein were analyzed using Western Blot in 42 patients with sporadic gastric carcinoma and its corresponding distal controls.Results Loss rate of expression of hMSH2 and PTEN in gastric cancer were higher than that of proximal tumor-adj acent gastric carcinoma tissues,distal tumor-adj acent gastric carcinoma tissues and normal gastric tissues (P<0.05 or P<0.01).PTEN loss rate showed negative correlation with the degree of tumor differentiation (rs=-0.707,P<0.05),but positive correlation with TNM staging, invasive depth,and lymph node metastasis (P<0.05).hMSH2 and PTEN were positive correlated in gastric cancer.There was a decreasing expression in expression losses of mismatch repair gene and hMSH2.Conclusion PTEN protein expression is correlated with clinic and pathological features.The expression deletion of mismatch repair protein hMSH2 was accompanied by down-regulated PTEN protein expression in sporadic gastric carcinoma.

    Objective To investigate the spatial relationship between adult acusticus externus and the pineal gland,cornu frontale ventriculi lateralis by using the mathematical methods of coordinate transformation.Methods The original 3-dimensional coordinate data of 30 patients were obtained on thinner CT scan from CT workstation,including meatus acusticus externus,angulus oculi lateralis,the pineal gland,cornu frontale ventriculi lateralis.Then,the original cordinate values were transformed to those in the new cordinate system (line between angulus oculi lateralis and meatus acusticus externus,meatus acusticus externus as origin,OM line and contralaterally meatus acusticus externus as flat)and statistics data were processed. Results The values of the pineal gland on X,Y,Z axis and cornu frontale ventriculi lateralis on X axis had significant difference between male and female(P<0.01).The values of cornu frontale ventriculi lateralis on Y,Z axis had no significant difference between male and female(P>0.05). Conclusion In this new cordinate system,the pineal gland and cornu frontale ventriculi lateralis in male and female adults had relatively stable spatial values,and had significant difference on OM line,which can be reference for operations.

    Objective To compare the sensitivity of interferon gamma release test (IGRA) with suptum smear,sputum culture and tuberculin slcintest (TST ) in the diagnosis of extrapulmonary tuberculosis.To investigate the value of IGRA method in extrapulmonary tuberculosis.Methods Fresh peripheral blood samples were collected from 75 patients of clinically diagnosed extrapulmonary tuberculosis.The result was compared with those of traditional gold standard test methods (sputum smear and sputum culture,TST ),and their clinical application value further were analyzed.Results In 75 cases of extrapulmonary tuberculosis,the positive rate by IGRA reagent test was 84.0%,sputum smear 8.0%,sputum culture 5.3%,TST test 52.0%.The positive rate of IGRA was significantly higher than those of other common methods,and the differences were statistically significant (P<0.01).Conclusion In the diagnosis of extrapulmonary tuberculosis,the sensitivity of IGRA reagent is significantly higher than those of the traditional test methods,IGRA has a good clinical value.

    Obj ective To explore the safety of laparoscopic appendectomy with percutaneous thread loop.Methods Acute appendicitis patients 74 cases treated with laparoscopic appendectomy were enrolled.The patients were randomly divided into wire loop fixation group (n=38)and non fixation group.The two group adopted the same operation procedure except the fixation.Comparison was made between two groups of patients in operation time,post-operation hospitalization time and post-operation complications (incision infection and fever).Results Compared with non fixation group,the mean operation time and post-operation hospital stay in loop group were significantly decreased (P<0.01),but there were no significant difference in the post-operation complications between two groups.Conclusion Laparoscopic appendectomy with percutaneous thread loop did not increase operative incision in the clinical use,the method is safe and effective,and also it reduced the operation time,but did not raise any post-operation complications.

    Objective To establish drug-resistant cell line SKOV3/DDP through ovarian cancer cell line SKOV3 by induced cisplatin (DDP ),and to study its relationship with the apoptosis pathway proteins.Methods The inverted microscope was applied to observe DDP influence on the cell morphology.Methyl thiazolyl tetrazolium (MTT)assay was used to detect proliferation inhibition rate affected by DDP on SKOV3 and SKOV3/DDP cells.Flow cytometry (FCM)was used to detect the influence of cell apoptosis and the expression of X-linked inhibitor-of-apoptosis protein (XIAP ),cysteine-aspartic acid protease-3 (Caspase-3 ),B cell lymphoma/lewkmia-2(Bcl-2)and Survivin proteins.Results MTT assay showed that proliferation inhibition rate of SKOV3/DDP cells decreased significantly compared with SKOV3 cells after DDP influence (P<0.01),and depended on the concentration and time (P<0.01).The resistance index of DDP to SKOV3/DDP and SKOV3 cells in 24h,48h and 72h were respectively 2.434,2.950 and 3.780 by comparison between 50% concentration of inhibition values.FCM revealed that the apoptosis rates of SKOV3/DDP cells to DDP decreased obviously after 48h than those of SKOV3 cells (P<0.01).Compared with SKOV3 cells,the expression of XIAP,Bcl-2 and Survivin proteins in SKOV3/DDP cells were higher,while the expression of Caspase-3 protein was lower,and their differences were statistically significant (P<0.01).Conclusion Drug-resistant cell line SKOV3/DDP for ovarian cancer is successfully established and its resistance mechanism is closely related with the abnormal expression of XIAP,Caspase-3,Bcl-2 and Survivin proteins.These proteins might be involved in the drug resistance of ovarian cancer.

    Objective To explore the expression of forkhead box(FoxO)gene and follicle-stimulating hormone receptor (FSHR)in human ovarian granulosa cells and their relationship. Methods One hundred and twenty-six unfertility women treated with in vitro fertilization and embryo transplantation (IVF-ET ) were recruited.According to the pregnancy outcome,the patients were divided into pregnancy groups 63 cases and non-pregnancy group 63 cases.The expressions of FoxO1 mRNA,FoxO3a mRNA and FSHR mRNA on human ovarian granulosa cells were detected by real time fluorescence quantitative PCR.The correlation analysis of FoxO1, FoxO3a,FSHR with serum sex hormones including luteotropic hormone(LH),estradiol(E2 ), progesterone(P)were mevaluated.Results High quality embryo number and serum E2 level in pregnancy group were significantly higher than those of non-pregnancy group (P<0.01 ),but LH,P level and number of eggs showed no significant difference between two groups (P>0.05). The expression FoxO1 mRNA in pregnancy group was higher than that of non-pregnancy group (P<0.01).The expression of FoxO3a and FSHR mRNA were no statistical difference between pregnancy group and non-pregnancy group (P>0.05).The correlation coefficients of FoxO1 and FoxO3a with FSHR were 0.881,0.999 (P<0.01 ).Conclusion FoxO1 and FoxO3a genes participate in the regulation of oocyte quality and their functions have closely connected with FSHR.

    Objective To investigate the value of cardiac troponin I (CTnI)in children of severe pneumonia with myocardial damage.Methods A total of 200 children with severe pneumonia in our hospital were collected as observation group,180 children with mild pneumonia in our hospital in the same period were collected as mild group,200 cases of healthy children in the same period were collected as control group,CTnI level,creatine kinase MB(CK-MB)level of three groups were analyzed and compared.Results The cTnI,CK-MB,creatine Kinase (CK), lactate dehydrogenase(LDH)and aspartate amino-transferase,AST(AST)levels of observation group were significantly higher than those of mild group (P<0.05),and cTnI positive rate of observation group was significantly higher than that of mild group (P<0.05).The difference of CK-MB level between the two groups was not significant(P>0.05).Conclusion Most children of severe pneumonia are associated with varying degrees of myocardial damage,cTnI can be used to monitor the condition of the children as an important marker of myocardial inj ury.Early detection of cTnI is favorable to the early diagnosis and treatment of severe pneumonia with myocardial damage,thereby preventing the occurrence of cardiac failure in the children.

    Objective To identify the utility of eicosapentaenoic acid (EPA)on diabetic retinopathy (DR)in vivo.Methods Proliferative diabetic retinopathy (PDR)Sprague-Dawley rat (n=24)was induced by application of streptozotocin associated with vascular endothelial growth factor (VEGF).Sprague-Dawley rats were randomly divided into D group (n=1 2 )representing medication administration group at background DR stage,E group (n = 8 ) representing medication administration group at pre-proliferation DR stage,F group (n=4 ) representing medication administration group at proliferation DR stage,then EPA was inj ected intravitreously. Fundus pathological changes were observed by fluorescence fundus angiography(FFA),CD105 positive retinal vascular endothelial cells were observed by microscope with immunohistochemistry technique.The results were compared with diabetic rat models established in our prior study.Results FFA demonstrated the best effect in E group in two weeks after EPA inj ection,whereas effective and augmented time-dependently in D group during eight weeks after EPA inj ection.In six weeks after EPA inj ection in E group,the effect decreased obviously as compared with that 2,4,8 weeks after EPA injection in D group.As compared with non-medication group,retinal neovascularization was not detected at the same course of this disease. Retinal neovascularization was also detected and diabetic retinopathy was not relieved in EPA injected eyes in F group.CD105 immunohistochemistry described there were no CD105 positive vascular endothelial cells at the same course of disease in D and E group.But CD105 positive cells were still observed in F group.Conclusion EPA can suppress DR development by blocking VEGF receptor flk-1 .

    Objective Optical coherence tomography (OCT)was used to quantitatively measure of peripapillary retinal nerve fiber layer (RNFL)thickness and the change of macular retinal thickness,understanding of correlation between the variation characteristics of patients′RNFL thickness and macular retinal thickness and visual outcome following Phacoemulsification (Phaco)of cataract patients.Methods Forty nine patients (70 eyes)diagnosed with age-related cataract,in Xuanwu Hospital,Capital Medical University,were divided into two groups,patients with diabetes in the diabetes group,20 cases (32 eyes),not with diabetes in normal group,29 cases (38 eyes),the cataract combined with phaco intraocular lens implantation operation was successfully implemented,the peripapillary RNFL thickness and macular retinal thickness changes were analyzed by means of OCT after Phaco operation at different time points (including 3 days,2 weeks,1 month,3 months,6 months after operation),to discuss correlation between the above indexes and postoperative best corrected visual acuity(CVA).Results In normal group,the peripapillary RNFL thickness showed statisticly negative correlation with CVA at the time 2 weeks,1 month,3 months after operation(r-0.466,-0.490,-0.511,P<0.01);In normal group,the macular thickness showed statisticly negative correlation with CVA at the time 2 weeks,1 month after operation(r=-0.309,-0.315,P<0.05)respectively.In diabetic group,the peripapillary RNFL thickness showed statisticly negative correlation with CVA in 2 weeks to 6 months following surgery(r=-0.365,-0.447,-0.512,-0.499,P<0.01);In diabetic group,the macular thickness showed statisticly negative correlation with CVA in 3 days to 6 months following surgery(r=-0.427,-0.490,-0.653,-0.451,-0.353,P<0.01).Conclusion The macular retinal thickness in Phaco patients after cataract is the main reason that affects the vision,at the same time cataract after Phaco peripapillary RNFL thickness has impact on visual acuity at different time and different degrees.

    Objective To compare the diagnosis of MRI and 99 Tcm-MDP bone imaging in bone metastasis.Methods Fifteen patients of diagnosed bone metastatic tumor underwent MRI and 99 Tcm-MDP bone imaging,the positive rate was compared between the two methods.Results MRI showed positive examination in 15 cases,and a total 95 lesions.99 Tcm-MDP bone imaging showed positive examination in 1 3 cases and a total 6 1 lesions.The positive rate of MRI in detection lesions was higher than that of 99 Tcm-MDP bone imaging,the difference had statistical significance(χ2=23.684,P<0.01).Conclusion Post-contrasted T1 WI and DWI made MRI more sensitive than 99 Tcm-MDP bone imaging in examination of bone metastasis.MRI and bone scintigraphy are considered as the complementary diagnostic methods in the detection of bone metastasis.

    Objective To investigate the expression of T-box protein 2 (TBX2)and anti-oncogene FEZ1 in normal gastric muscosa,intraepithelial tumor and gastric adenocarcinoma and the roles of their clinic pathology.Methods The exprssion of TBX2 and FEZ1 were detected by immunohistiochemistry in normal gatromucosa,intraepithelial tumor and gastric carcinoma. Statistic soft ware SPSS1 5 .0 was used to analyze the clinicopathological significance and its relationship to prognosis.Results The positive expression rate of TBX2 protein was higher in low differentiated group than in high differrentiated one (P<0.05),and TBX2 in group with stageⅣwas also higher than in group with stageⅠ(P<0.05),and also in group with over one half deeply muscle infiltration was significantly higher than in group with under one-half infiltration (P<0.05).The positive expression rate of FEZ1 protein was lower in low differentiated group than in high differrentiated one (P<0.05),so was it in group with tumor diameter over 4cm than in group under 4cm (P<0.05 );the expression rate of FEZ1 was lower in group with over 1/2 muscle infiltration than in group with under1/2 infiltrattion,so was it in group with lymph nodule metastasis than in group without lymph nodule metastasis(P<0.05);also it was higher in group with the age over 60 years old than in group under 60 years old (P<0.05).Conclusion TBX2 and FEZ1 were of great importance in the development and prognosis of gastric adenocarcinoma. The former played an important role of oncogene with the latter anti-oncogene.

    Objective To look for the scientific method which could apply to research the relationships between typhoid fever incidence and meteorological factors,and to discuss the meteorological epidemiology characteristics of typhoid fever.Methods The data of typhoid fever, meteorological parameters and population in 1991-2010 in Handan were collected and analyzed. Results ①Collinearity diagnosis of meteorological parameters showed that the minimal tolerance was 0.014 and the maximal variance inflation factor was 73.264.②Monthly incidence of typhoid fever had significant positive correlation with monthly mean temperature,monthly extreme minimum temperature,monthly extreme maximum temperature,monthly small evaporation, monthly total precipitation,monthly sunshine hours and monthly average relative humidity,and significant negative correlation with monthly average air pressure by using Spearman correlation analysis (P<0.05 or <0.01 ).The correlation coefficient of typhoid fever and monthly mean temperature was the biggest(rs=0.592).③The result of KMO and Bartlett showed that test of meteorological parameters was very suitable for factor analysis.Conclusion The problem of multicollinearity in 10 meteorological parameters was very serious.Monthly mean temperature was the main meteorological factor influencing typhoid fever incidence.The non natural water factor was the main factor affecting typhoid fever incidence.

    Objective To investigate efflux pump drug-resistance and gene expression in multidrug resistant and pandrug resistant Acinetobacter baumannii.Methods The multidrug resistant Acinetobacter baumannii efflux pump phenotypes were detected by joining carbonyl cyanidem-chloropheny hydrazone (CCCP)pump inhibitors,the minimum inhibitory concentration (MIC)variation was observed in 9 6 strains of Acinetobacter baumannii resistant to tetracycline, norfloxacin,gentamicin,cefotaxime,34 strains of efflux phenotype positive Acinetobacter baumannii were screened out.The efflux pump protein gene adeB in 34 Acinetobacter baumannii were detected and sequenced with the polymerase chain reaction amplification.Results The initial selection involoed 34 strains (35.42%)whose MIC reduced by 4 times and above.In 34 stains of efflux pump phenotype positive Acinetobacter baumannii,33 strains were detected adeB gene,the positive rate was 97.06%.When the 33 strains Aacinetobacter baumannii adeB efflux pump gene were sequenced,the sequence homology was 100.00% compared in Genebank.Conclusion Pump inhibitor CCCP plays an important role in the reversal of Acinetobacter baumannii resistant to tetracycline,norfloxacin,gentamicin and cefotaxime,active efflux pump genes play an important role in multidrug resistant Acinetobacter baumannii.
