    Objective To compare the safety and efficacy of biphasic insulin aspart 50 (BIAsp 50 )and premixed human insulin 50 (BHI 50 )on type 2 diabetes mellitus (T2DM) patients.Methods A total of 76 T2DM patients were randomly divided into BIAsp 50 group(38 cases)and BHI 50 group(38 cases).The fasting blood glucose (FBG),2-hour postprandial blood glucose (2hPG)and glycated hemoglobin(HbA1 c)after 12 weeks′insulin aspart 50 treatment, the insulin dosage and hypoglycemic episodes were compared.Results After 12 weeks′treatment,the decrease of 2hPG in BIAsp 50 group was more than that in BHI 50 group (P <0.05).Besides,in BIAsp 50 group,the incidence of hypoglycemic episodes was reduced,the insulin dosage was smaller,both showing significant differences (P <0.05).Conclusion BIAsp 50 could improve treatment efficiency and patients′ compliance.It is economic and suitable for widely clinical application.

    Objective To explore the quantitative training with theory and practice in j unior nurses about insulin knowledge and inj ection skills.Methods The research was taken in 148 junior medical nurses who started work within two years,including the experimental group in which nurses were given the quantitative training combined theory and practice,and the control group in which 120 non-medical junior nurses accepted the routine training in hospital.Results After the training,two groups obviously improved on the aspects of basic knowledge of insulin, inj ection skill score,insulin theory examination scores,skills simulated evaluation,and practice in ward grade.But the medical nurses group were has increased much more than non-medical nurses, especially in matter of insulin basic knowledge and inj ection skill score.After the training,medical nurses group were obviously better than non-medical nurses group in all scores (P<0.01).The training was very efficient to avoiding the nursing risk of insulin.Conclusion The quantitative training combined theory and practice is obviously superior to the routine training.The method raised the insulin knowledge and satisfaction of diabetes inpatients,enhanced the professional image and value of nurses.

    Objective To evaluate the effects of intensive first aid training on improving the first aid techniques among community health workers.Methods The study enrolled 180 community health workers who were randomly divided into traditional training group and re-training group,each group with 90 members.Physicians in both groups were given a series of training including theory teaching,operational practice and scene simulation exercise.The re-training group accepted the same training once again after six months.Assessments on both theory and practice were conducted before the training,immediately after the training,3 and 12 months after the training separately to compare the training effects.Results The assessment results between the two groups showed no significant differences in the emergency theories, cardio-pulmonary resuscitation,electric defibrillation,trachea cannula,emergency treatment of trauma and the total scores before training (P > 0.05).All workers′ techniques in both groups significantly improved after the training immediately,then the assessment scores slightly decreased 3 months later.The traditional training group had more obviously decline than the re-training group after one year.Significant differences were found between two groups on every time point after training (P <0.01).The qualified workers in the re-training group were much more than the traditional training group on first aid knowledge as well as practical skills (P <0.01). Conclusion The first aid technology among community physicians needs urgent improving,and the re-training method proved to be more effective.

    Objective Different types of tension-free hernia repair were used to treat various types of inguinal hernia,and curative effect was evaluated for a standardized treatment in clinical practice.Methods The clinical data of 150 patients with inguinal hernia treated with tension-free hernia repair were retrospectively analyzed.Results All cases were operated successfully.The mean operation time was 35-45min.Wound healing in all patients discharged from hospital,without serous hematoma and infection.Seven days after surgery,the incidence of no pain and mild pain of experimental group (98.7%)was significantly higher than that in the control group (93.4%).The patients were followed up 2-24 months after operation,no case recurred.Conclusion Tension-free hernia repair in inguinal hernia individually is an effective surgical method.The individual method is safe and effective,the post-operation recovery is quick and the result is satisfactory.Individual application of tension-free hernia repair in inguinal hernia contributes to standardized treatment.

    Objective To systematically evaluate the safety and feasibility of fast track surgery in perioperative period of hepatectomy.Methods Literatures of PubMed,Embase, ScinceDirect,Google Scholar,WanFang and CNKI published before March 2014 were searched. The methodological quality including RCTs and CCTs were assessed.Results Seven RCTs and two CCTs involving 1 072 patients met the inclusion criteria.Compared with those by conventional way the patients managed by fast track surgery had less complications (RR=-0.01,95%CI=-0.06 to -0.03,P=0.58),faster bowel function recovery (MD=-0.90,95%CI=-1.28 to -0.52,P<0.01)and shorter postoperative hospital stay (MD=-2.45,95%CI=-3.99 to -0.91,P<0.01),but not statistically increased severe complications (bile leak,pleural effusion,intraperitoneal abscess)(RR=0.93,95%CI=0.60 to 1.45,P=0.75).Conclusion Fast track surgery is safe and feasible in perioperative period of hepatectomy.

    Objective To investigate the effect and clinical significance of coenzyme Q10 on postoperative fatigue syndrome(POFS)in patients with rectal cancer after radical operation. Methods Seventy-six cases with radical resection of rectal carcinoma were randomly divided into experimental group(38 cases)and control group (38 cases).Intravenous drip of coenzyme Q10 (10mg,once daily)was given after anaesthesia induction and 1-3d postoperation in experimental group,and control group received normal saline instead.After operation,the patients of two groups were treated with the same therapeutic regimen.General data were observed in two groups.Visual analogue scale(VAS)was measured preoperation and 1st,3rd,7th d postoperation. Body mass index (BMI ),triceps skinfold thickness (TSF ) were also tested in two groups preoperation and 7th d postoperation.Serum total protein (TP),albumin (ALB),transferrin (TRF),adiponectin(APN)were detected at the same time.Results The evacuation time and hospitalization time of experimental group were shorter than those of control group (P<0.05). VAS scores increased in two groups after operation,whereas VAS score in experimental group was lower than that in control group 3rd,7th d postoperation (P<0.05).No significant difference was found in preoperative BMI,TSF,TP,ALB,TRF between two groups (P>0.05 ),and all these indicators decreased after operation,whereas decline degree in experimental group was less than that in control group (all P<0.05 ).Conclusion Obvious POFS existed in patients with rectal cancer after radical operation.And coenzyme Q10 can alleviate POFS,promote early rehabilitation.

    Objective To compare the pregnancy complications and neonatal outcomes of patients with gestational diabetes mellitus who received insulin treatment with continuous glucose monitoring system (CGMS ) and peripheral blood monitor.Methods A randomized control experimental study was conducted in the hospitalized diabetes mellitus patients and gestational diabetes mellitus patients with insulin treatment in Beijing Obstetrics and Gynecology Hospital. The patients were randomly divided into two groups,the patients of experiment group were monitored using CGMS,those of control group referred to the use of peripheral blood glucose monitoring.All patients in two groups were observed about glycemia control,pregnancy complications and neonatal outcomes.Results Higher rate of patients with hypoglycemia was detected during pregnancy in the experimental group compared to the control group (P<0.05). The incidence rates of macrosomia in experimental group were lower than those in control group (P<0.05 ).Conclusion CGMS revealed higher detective rate of gestational hypoglycemia, compared with that of peripheral blood monitoring.The study provides a reliable basis for clinical treatment,while reduces the occurrence of adverse pregnancy outcomes and neonatal complications.

    Objective To investigate the effect of lipid combined with uterine artery blood flow detection in diagnosis of preeclampsia.Methods Retrospective analysis was made on 200 preeclampsia women and 600 normal pregnant women who took prenatal tests in the Fourth Hospital of Hebei Medical University.The lipid levels during the first,second and third trimester were compared between normal and preeclampsia women.A total of 588 pregnant women were randomly selected,all women were detected the lipid level during first trimester and uterine artery blood flow during second trimester.The triglyceride level and uterine artery blood flow were all normal in group A.The triglyceride level was high but uterine artery blood flow was normal in group B.The uterine artery blood flow was high but the triglyceride level was normal in group C. The triglyceride level and uterine artery blood flow were all high in group D.The incidence of preeclampsia for the four groups were compared.Results For preeclampsia women,triglyceride during first trimester was significantly higher than normal pregnant women (P <0.05);During second and third trimester,triglyceride,cholesterol and low density lipoprotein were much higher (P <0.05).The incidence of preeclampsia for group D was significantly higher than other three groups (P < 0.05 ).Conclusion Preeclampsia women had metabolic abnormalities of lipid, especially triglyceride level was high from first trimester and continued high state.Lipid combined with uterine artery blood flow detection would have higher predictive value in diagnosis of preeclampsia.

    Objective To explore the inhibitory effect of arsenic troixide(AS2 O 3 )on the growth of nude mice bearing human endometrial carcinoma transplanted subcutaneously and its mechanisms.Methods Human endometrial cancer xenograft models were established in nude mice using HEC-1-A cell,then were divided into five groups as following,group A(AS2 O 3 4mg· kg-1 ·d-1 ),group B(AS2 O3 6mg· kg-1 · d-1 ),group C(AS2 O 3 8mg· kg-1 · d-1 ),group D (cisplatin 3mg·kg-1 ·d-1 ),and group E(saline group).The drugs were given intraperitoneally for 14 d,then the inhibition rate of tumor volume and tumor mass were calculated.The change of body mass of nude mice after treatment was measured.Results The tumor volume was inhibited by 50.97%,75.58%,56.92%,52.23%,and the tumor mass was inhibited by 10.15%,29.33%, 16.67%,14.69%,respectively,in group A,group B,group C,group D,as compared with the saline group,the difference were statistically significant,respectively (P <0.05).AS2 O 3 had no effect on body mass,while the change of nude mice body mass was significant after treatment in group D. Conclusion AS2 O3 could inhibit the growth of human endometrial cancer xenograft in nude mice. Furthermore,AS2 O3 has stronger inhibitive effect than clisplatin,with lighter side effect.Thus, AS2 O3 is a new drug for endometrial cancer treatment.

    Objective We examined typical changes in hippocampus volume and metabolite ratios in the workers chronically exposed to lead,and compared with those of control individuals, using magnetic resonance imaging (MRI ) and proton magnetic resonance spectroscopy (1 H-MRS),then,investigated whether an abnormality in brain capacity or metabolism associated with memory changes.Methods The subj ects included 8 workers chronically exposed to lead (j ob years 6-37)and 8 gender age and matched healthy people,MRS was used in the related region of the brain to detect N-acetyl aspartate(NAA),lipid(Lip),choline(Cho),glutamate and glutamine (Glx),Lactate(Lac)and myoinositol(mI)levels,and each item was expressed respectively as a ratio to creatine and phosphocreatine (Cr).We also assessed the hippocampus volume on 3D T1-weighted MRI.Neurobehavioral tests were also performed to define memory status.Results NAA/Cr ratio in the frontal lobe,basal ganglia and hippocampus,MI/Cr ratio in basal ganglia and hippocampus,and the hippocampal volume were significantly different to those of control subj ects (P < 0.05 ).For the workers conformed lead poisoned,the blood lead concentration was significantly correlated with MI/Cr ratio,NAA/Cr ratio in the hippocampus as well as hippocampus volume (P<0.05 ).Conclusion Occupational exposure to lead may cause subtle structural and functional alteration in human brains,which may be one reason of memory lost.

    Objective To investigate the effect of PC cell-derived growth factor(PCDGF) RNA on hepatocellular carcinoma cells HepG2 in vitro .Methods The PCDGF-shRNA expression vector was transfected into hepatocellular carcinoma cells HepG2 by lipofection technology.The expression of PCDGF mRNA and protein were detected by reverse transcription-polymerase chain reaction(RT-PCR)and Western Blot,respectively.The ability of tumor cell proliferation was determined by MTT assay.The effect of PCDGF expression on invasion was also identified by transwell chamber system.Results The expression of PCDGF mRNA and protein were both decreased in the transfection group than in the non-transfection group,liposome group and negative vector group(P <0.01).The MTT showed cell proliferation activity was inhibited in the transfection group (P <0.01).Transwell experiments showed that cell invasion ability was lower in the transfection group compared with that in the non-transfection,liposome and negative vector groups(P <0.01).Conclusion PCDGF RNA interference can inhibit the proliferation and invasion abilities of hepatocellular carcinoma cells in vitro .PCDGF gene may be the new target of gene therapy.

    Objective To investigate the effect of memantine on learning and memory disorder induced by cerebral ischemia/reperfusion (I/R)of rats.Methods Model of cerebral ischemia was established by the four-vessel occlusion method.Memantine was intraperitoneally inj ected for five days after the ischemia.Cresyl violet staining was used to observe the survival neurons in pyramidal cell layer of the hippocamal CA1 region and Morris water maze test was performed.Results Compared with those of saline control group, the escape latency of the orientation navigation experiment in sham group and memantine group was significantly shortened(P<0.01);the escape latency of I/R group was significantly longer than that of sham group and memantine group(P<0.01),but the escape latency of I/R group had no significant difference compared with that saline control group(P>0.05);Compared with those of control group,the crossing platform time,time percent of swimming in northwest and neuronal cell loss in hippocamal CA1 region in sham group and memantine group significantly raised(P<0.01),but the three items of I/R group significantly reduced in comparison with the above two groups(P<0.01),while they still had no significant differences compared with those of saline group(P>0.05).Conclusion Memantine can improve the learning and memory disorder induced by cerebral ischemia/reperfusion in rats through decreasing neuronal cell death.

    Objective A high efficiency liquid chromatography method was established for the determination of principal component crude drug josamycin.Methods A octadecyl bonded silica column was used as the stationary phase,with the mobile phase of mixture of perchloric acid solution-acetonitrile (60∶40)at the detection wavelength of 231nm.The column temperature was 40℃ and the injection volume was 20μL.Results The principal component of josamycin showed a linear range within 0.125 5- 0.878 5g/L (r = 1.000 0).The average recovery was 99.7%.The quantitation and detection limits were 2.51ng and 0.84ng,respectively.Conclusion The method was rapid,simple,accurate,sensitive,and suitable for the determination of principal component in crude drug josamycin.
