肺部感染评分、降钙素原及C反应蛋白对特发性肺纤维化并发感染的诊断意义

摘要: [摘要] 目的 探讨简化临床肺部感染评分( clinicalpulmonaryinfectionscore , CPIS )、降钙素原( procalcitonin ,
PCT )及 C 反应蛋白( C-reactiveprotein , CRP )对特发性肺纤维化( idiopathicpulmonaryfibrosis , IPF )并发肺部细菌
感染的诊断意义、相关性以及联合使用的临床诊断价值。方法 选择疑似 IPF 伴肺部细菌感染患者 98 例,检测
PCT 、 CRP ,计算简化 CPIS ,统计 PCT 、 CRP 、简化 CPIS 诊断 IPF 并发肺部细菌感染的敏感度、特异度、准确度。分
析 PCT 与 CPIS 的相关性。结果 简化 CPIS 、 PCT 诊断 IPF 伴肺部感染的敏感度、特异度、准确度高于 CRP , PCT
联合简化 CPIS 诊断 IPF 伴肺部感染特异度、准确度高于单独简化 CPIS 和 PCT ,而敏感度未提高。常规炎症指标
中, PCT 与 CPIS 呈显著正相关(
r =0. 450 , P =0.000 )。结论 简化 CPIS 联合 PCT 检测有助于提高 IPF 并发肺部
细菌感染早期诊断特异度,且敏感度未提高。

Abstract: [ Abstract ] Objective To explorethe diagnosticsignificance , correlation and clinical
diagnosisvalueofthesimplifiedclinicalpulmonaryinfectionscore ( CPIS ), procalcitonin ( PCT )
andC-reactiveprotein ( CRP ) inidiopathicpulmonaryfibrosis ( IPF ) complicatedwithpulmonary
bacterialinfection.Methods Atotalof98adultpatients , whowereclinicallydiagnosedasIPF
complicatedwithpulmonarybacterialinfection , wereenrolledinthisprospectivediagnostictest ,
monitoringvariousinflammatoryindicatorssuchasCRP , PCT , calculatingthesimplifiedCPIS
onthefirstdayoftheadmission.Thesensitivity , specificity , accuracyofPCT , CRPandCPIS
forthediagnosisofIPFcombinedwithpulmonarybacterialinfectionswerecalculatedafterthe
experiment.Results Thesensitivity , specificityandaccuracyofPCTandCPISinthediagnosis
ofIPFwithpulmonaryinfectionwashigherthanthatofCRP.Thespecificityandaccuracyof
PCTcombinedwithsimplifiedCPISindiagnosisofIPFwithpulmonaryinfectionwashigherthan
thatofCPISandPCT , butthesensitivitywasnotimproved.ThePCTandCPISwerepositively
correlatedwiththeconventionalinflammatorymarkers ( r =0.450 , P =0. 000 ) .Conclusion The
simplified CPIScombined with PCT detection washelpfultoimprovetheearly diagnostic
specificityofIPFcomplicated withpulmonarybacterialinfection , andthesensitivitywasnot
improved.
