    Objective This study is to explore the protective role and mechanism of tanshinoneⅡA(Tan ⅡA)for the blood-brain barrier in focal cerebral ischemia of rats. Methods Male Sprague-Dawley rats were subjected to permanent middle cerebral artery occlusion(MCAO). Sprague-Dawley rats (n = 168)were randomly divided into sham-operated group,MCAO group,TanⅡA higher dose group (TanⅡA-H)and TanⅡA lower dose group(TanⅡA-L). Tan ⅡA was administered with intraperitomeal injection immediately after operation,rats were executed at 24h. Western Blot,RT-PCR and confocal microscope were used to measure claudin-5 protein and IgG expression in ischemic brain. And the neurological deficits,brain water content and infarct volume were explored. Results At 24h after focal cerebral ischemia,claudin-5 expression in MCAO group decreased. Claudin-5 in Tan Ⅱ A-H group significantly up-regulated,and also its neurologic deficit,infarct volume and brain water content were alleviated compared with MCAO group( P ﹤ 0. 05). Conclusion Tan Ⅱ A-H protected blood-brain barrier permeability by increasing claudin-5,which might be the mechanism of treating focal cerebral ischemia.

    Objective To determine whether an association exists between obstructive sleep apnea hypopnea syndrome( OASHS)and short-term prognosis of acute cerebral infarction in different genders. Methods Patients with acute cerebral infarction(n = 85)were recruited and divided into pure cerebral infarction group(n = 37),cerebral infarction with mild OSAHS group( n = 23)and cerebral infarction with moderate to severe OSAHS group(n = 25)according to the monitoring results of apnea hyponea index(AHI). The degree of neurologic impairment was assessed by National Institutes of Health Stroke Scale(NIHSS)and modified Rankin Score(mRS)on admission and before discharge. Results The NIHSS scores before discharge,the reduced values of NIHSS and mRS in the cerebral infarction with moderate to severe OSAHS group were significantly different from those in pure cerebral infarction group (P ﹤ 0. 05). Significant difference in reduced values of NIHSS and mRS were also found in male(P ﹤0. 01 or ﹤ 0. 05),but not in female. Conclusion The severity of OASHS is related to the increased risk of poor prognosis in male patients with acute cerebral infarction.

    Objective To observe the protective role of artorvastatin for kidney in rats with renal interstitial fibrosis. Methods Fifty-four male Wistar rats were randomly divided into three groups:sham-operated group(group A,n = 18),unilateral ureteral obstruction(UUO)group(group B,n = 18), UUO with artorvastatin treated group(group C,n = 18). Artorvastatin put to use 3 days before surgery. UUO and sham rats received equal volume physiological salif by daily gastric gavage. Rats were killed after 3,7,14 days of the surgery. The kidney pathological changes were observed under microscope. The expressions of intercellular adhesion molecule-1(ICAM-1)and nuclear factor-кB(NF-кB)were analyzed by routine immunohistochemical method. Results The levels of ICAM-1 and NF-кB in group B were various and significantly higher with the extension of obstruction time. The levels of ICAM-1 and NF-кB in group C were significantly lower than those of group B at the same period(P ﹤ 0. 05). The level of NF-кB was significantly associated with ICAM-1(P ﹤ 0. 05). Conclusion Artorvastatin reduces ICAM-1 and NF-кB expression,resulting in decreased tubular damage and interstitial fibrosis. The effect of artorvastatin in reducing ICAM-1 perhaps implements through the inhibition of NF-кB partly.

    Objective To explore the factors affecting the compliance of dialysis in chronic renal failure patients,and to provide the basis for rational improvement in the care approach of chronic renal failure patients undergoing dialysis. Methods The patients' informations were collected to analyze the relationship between patients' compliance and age,educational condition,psychological and emotional condition,dialysis duration,trust in the medical staff,occupational and economic status. Results In accordance with compliance behaviors,the patients were divided into the following groups based on age ( ﹥ 18 - 40 years,﹥ 40 - 60 years and ﹥ 60 - 80 years),educational background( preliminary school and below,middle school and high school and university and above),Hamilton Dysphoria Questionnaire ( ﹤ 14 scores,≥14 - 28 scores,≥29 scores),Hamilton Depression Questionnaire( ﹤ 14 scores,≥14 -28 scores and ≥29 scores),dialysis duration( ≤1 year,﹥ 1 - 3 years and ﹥ 3 years),trust in the medical staff( good,average and poor),occupation( public servant,white collar and farmer) and economic status( completely satisfied,averagely satisfied and unsatisfied). The patients' compliance behaviors were all satistically different among these groups( P ﹤ 0. 01 ). Logistic regression analysis showed that the compliance of dialysis in chronic renal failure patients was associated with age, educational background,Hamilton Dysphoria Questionnaire,Hamilton Depression Questionnaire,dialysis duration,trust in the medical staff,occupation and economic status( P ﹤ 0. 05). Conclusion Age, educational condition,psychological and emotional condition,dialysis duration,trust in the medical staff, occupational and economic status are all factors that affect a better degree of patient compliance behavior. The formulating of treatment plan and care measures should be individualized,thus improving to patients' compliance behavior,the therapeutic effect.

    Objective This study observes the effect of paclitaxel on liver cancer cell proliferation by inhibiting KIF14 expression in liver cancer cells. Methods Silenced KIF14 expression in liver cancer cells by RNA interference,these cells were delt by paclitaxel and the proliferation ability of liver cancer cells was determined by MTT test. Results Silence KIF14 expression in liver cancer cells enhanced the cells sensitivity to paclitaxel. Conclusion KIF14 is important in the process of drug resistance produced by human hepatocellular carcinoma cells. Further study on its mechanism can provide theoretical basis for seeking the effective ways to overcome drug resistance of liver cancer.

    Objective To explore the mechanical stability of translaminar facet screw( TFS) and pedicle screw( PS)in anterior lumbar interbody fusion( ALIF)and whether the two posterior instrumentations have the same influence on the adjacent segments in two level anterior lumbar interbody fusion. Methods Normal adult human fresh corpses were used as spine samples,16 lumbar spines were randomed into no surgery group( intact),ALIF group,ALIF + TFS group and ALIF + PS group. Under 400N compressive preload and 75N/ m speed,comparisons were performed in the stiffness,motion range of adjacent segments and intervertebral disc pressure among different groups. Results Compared with intact group,ALIF group showed increase in flexion,lateral bending axial rotation,but decrease in extension. Comparisons among ALIF group,ALIF + TFS group and ALIF + PS group,ALIF group also showed increase in flexion,lateral bending,axial rotation,and the fixation stability by TFS and PS were similar, the difference was no significant,but TFS exerted less compressive stress on the adjacent segments of intervertebral discs. Conclusion TFS can provide the stability equivalent to PS in two-segment ALIF, TFS is the ideal choice in the operation.

    Objective To explore the effect of early family oriented intervention on the intellectual development of premature infants. Methods One hundred cases of premature infants were chosen in the study,they were divided into observation group and control group according to randomly number table. The observation group was treated with early family oriented intervention,the control group used with conventional methods of child rearing. Differences were observed between the two groups after the intervention of children with mental development and brain MRI findings. Results After the intervention 3,6,12,18 months,adaptability,gross motor,fine motor,language and social development quotient( DQ)means of two groups increased. There were no statistically significant difference in the scores between two groups 3 months after the intervention( P ﹥ 0. 05),and 6,12,18 months after the intervention,the average of DQ in observation group was significantly higher than that of control group (P ﹤ 0. 01). MRI showed normal 40 cases,abnormalities 10 cases in observation group. For the patients in the control group,MRI was normal in 29 cases,abnormal in 21 cases. The normal proportion of observation group with brain MRI manifestations was higher than that of control group( P ﹤ 0. 01). Conclusion The early implementation of family oriented intervention can improve the intelligence development of premature infants.

    Objective Reverse transcription polymerase chain reaction(RT-PCR)was used for inflammatory factor expression in experimental rat temporomandibular joint cartilage by the occlusal interference. Methods The six week old female SD 9 rats were randomly divided into occlusal disorder group,removed occlusal disorder group and normal control group,3 rats in each group. The occlusal interference animal model was established. After six weeks,occlusal disorder group did not receive any treatment,removed occlusal disorder group was rid of occlusal interference,normal control group served as normal control. After eight weeks,the rats were sacrificed,the right side of the temporomandibular joint was cut out. RT-PCR was used to detect interleukin-1(IL-1),interleukin-6(IL-6),tumor necrosis factorα(TNF-α),and interferon -γ(IFN-γ)expression in the temporomandibular joint cartilage. Results In occlusal interference condition,RT-PCR results showed inflammatory cytokines IL-1,IL-6 and TNF-α in rats of different groups were expressed. However,the inflammatory cytokines expression of the experimental group were higher than those of control group. IL-6 and TNF-α expression in occlusal disorder group were higher than those of removed occlusal disorder group. IFN-γ expression in three groups was not detected. Conclusion Under the occlusal interference condition,the expression of the inflammatory cytokines IL-1,IL-6 and TNF-α in the temporomandibular joint cartilage got enhancement. These results suggested that inflammatory cytokines and temporomandibular joint condylar remodeling in rats activity correlated.

    Objective To explore the relationship between duration of diabetes mellitus and cerebrovascular disease by observing the variation of transcranial Doppler sonograghy(TCD)in different stages of type 2 diabetes mellitus patients. Methods Eighty type 2 diabetes mellitus patients who were hospitalized or confirmed in the department of our hospital were divided to two groups,40 patients in group A(disease course≥10 years)and 40 patients in group B(disease course ﹤ 10 years). The group C including 40 health persons with similar age served as control group( group C). The systolic peak velocity(VP)and pulsatility index(PI)of middle cerebral artery(MCA)of left and right sides and vertebral artery(VA)of left and right sides in three groups were detected by TCD. Results The VP of MCA of left and right sides and VA of left and right sides of group A and group B were significantly higher than those of group C(P ﹤ 0. 01),the VP of MCA of left and right sides and VA of left and right sides of group A were much higher than those of group B(P ﹤ 0. 01),the PI of MCA of left and right sides and VA of left and right sides of group A were much higher than those of group B and group C(P ﹤ 0. 01), and there were no significant statistic differences between the PI of MCA of left and right side and VA of left and right side of group B and group C(P ﹥ 0. 05). Conclusion The cerebrovascular disease may be related to disease course of diabetes mellitus patients and TCD test can provide important information in functional status of cerebral blood vessels.

    Objective To assess the diagnostic value of combined detection of tumor markers carcinoembryonic antigen( CEA),carbohyrate antigen 19-9( CA19-9 ) and inflammatory marker C reactive protein( CRP)in colon cancer. Methods Serum levels of CEA,CA199 and CA125 were determined in 146 patients with colorectal cancer,158 patients with inflammatory bowel disease,colon polyp or acute appendicitis,and 76 healthy persons. And their accuracies and consistencies with the analysis of receive operating characteristic(ROC)and value of Kappa were evaluated. Results Compared with the control group,the serum CRP,CA19-9 and CEA in patients with colon cancer and inflammatory bowel disease were significantly increased. The levels of CEA and CA19-9 in colon cancer group were obviously higher than those of inflammatory bowel disease group and control group. ROC was used to compare the diagnostic value of CEA,CA19-9 and CRP in discriminating benign inflammatory and colon cancer groups,indicating the diagnostic value of CRP was better than that of CEA and CA19-9. The AUC of combined detection of CEA,CA19-9 and CRP was 0. 886. Conclusion Combined detection of CEA, CA199 and CRP can help improve the diagnosis accuracy of colorectal cancer. The diagnostic accuracy was superior to individual item.

    Objective To establish a reversed phase high performance liquid chromatography (RP-HPLC)method for determination of emodin and physcion in Jueming Jiangzhi Tablets. Methods RP-HPLC system was carried out on Kromasil C18(250mm × 4. 6mm,5μm)column with a mobile phase, the standards and samples were separated using a gradient mobile phase consisting of methanol, tetrahydrofuran and 1% phosphoric acid(84: 7: 9). The column temperature was 25℃ and the flow rate was 1. 0mL/ min. The nuciferine was detected at the wavelength of 254 nm. The injection volume was 20μL. Results Emodin and physcion were separated from impurities well and its retention time was at about 6. 9 min,11. 8 min. Regress equation was Y = 3. 31 × 104 X - 4. 63 × 103(R = 0. 999 7). The linear in the range of emodin was 3. 12 - 9. 36mg / L. The average recovery was 102. 1% ,and its RSD rate was 1. 7%(n = 9);Regress equation was Y = 2. 35 × 104 X + 8. 80 × 103(R = 0. 999 8). The linear in the range of physcion was 2. 1 - 10. 5mg / L. The average recovery was 104. 7% ,and its RSD rate was 1. 3%(n = 9 ). Conclusion RP-HPLC is areliable and accurate method,which can be applied for determination of emodin and physcion in Jueming Jiangzhi Tablets.
