    Objective To research the expression of protein insulin receptor substrate( IRS ) and the level of tyrosine phosphorylation in the endometrium of the patients with polycystic ovary syndrome( PCOS)and investigate the correlations between the expressions and the resistance of insulin. Methods Thirty patients with PCOS served as experimental group according to the diagnostic criteria, and 30 females hospitalized synchronously with regular menstrual cycle,normal serum sex hormone, fasting plasma glucose and fasting insulin levels as control group. According to the homeostasis model assessment for insulin resistance,the PCOS group was divided into two groups,insulin resistance group ( group IR)17 cases and non insulin resistance group( group NIR )13 cases. The differences in the expression of IRS-1 and IRS-2 by immunohistochemistry among group IR,group NIR and control group, and the levels of tyrosine phosphorylation of IRS-1 and IRS-2 among the three groups were compared. Results The expressions of protein IRS-1 and IRS-2 and the levels of tyrosine phosphorylation of IRS-1 and IRS-2 in PCOS group were significantly weaker than those in control group ( P ﹤0 . 05 ). The expressions of protein IRS-1 and IRS-2 and the levels of tyrosine phosphorylation of IRS-1 and IRS-2 in the PCOS group with IR were obviously weaker than those in the PCOS group without IR( P﹤0 . 05 ). Conclusion It is obvious that the expressions of protein IRS-1 and IRS-2 declined and the degrees of tyrosine phosphorylation descended in the patients with PCOS. The abnormality is quite distinct in the patients of PCOS group with IR. It speculates that there is a close relationship between the abnormal expression of IRS,the descent of the degree of tyrosine phosphorylation and the genesis of IR in the endometrium of the patients with PCOS.

    Objective To explore effective treatments for patients with obesity hypoventilation syndrome complicated by heart failure. Methods Retrospective analysis was performed on 120 patients with obesity hypoventilation syndrome complicated by heart failure. Noninvasive positive pressure ventilation( NPPV)group(60 cases)was treated with NPPV and medications,and the control group(60 cases)only with medications. The related indicators and curative effects before treatment,6 months after treatment were compared between two groups. Results Arterial blood gas analysis,brain natriuretic peptide( BNP ),echocardiography and polysomnography( PSG ) the control group after treatment all worsened(P﹤0. 05 or ﹤0. 01). Body mass index(BMI)decreased,arterial blood gas analysis,BNP, echocardiography and PSG of NPPV group after treatment all improved significantly( P ﹤ 0 . 01 ). Compared with control group after treatment,NPPV group improved significantly in BMI,arterial blood gas analysis,BNP,echocardiography and PSG( P ﹤0. 01 ). Conclusion NPPV is an effective method for patients with obesity hypoventilation syndrome complicated by heart failure.

    Objective To investigate the value of humane care in autologous peripheral blood stem cells( APBSC)gathers. Methods Eighty-six cases of autologous APBSC gathers were randomly divided into two groups,43 cases in each group. The control group was given routine care,and the observation group was treated with humane nursing,and psychological condition and acquisition results in two groups were statistically analyzed. Results APBSC once puncture success rate of observation group was 58 . 14%,which was significantly higher than that of control group( 30 . 23%)( P ﹤0 . 01 ),the collecting effects and time effects in observation group were better than those in control group( P﹤0 . 05 or ﹤ 0 . 01 ). SCL-90 analysis showed that psychological conditions in observation group improved significantly,comparing with those of control group( P ﹤ 0. 01 ). Conclusion Humane care can significantly improve APBSC puncture success rate,improve collection result,enhance the psychological status of gathers,shorten acquisition time,which is worthy of learning and clinical application.

    Objective To explore the effect of butylphthalide on the brain edema of rats following traumatic brain injury and the expression of cortical matrix metalloproteinase-9( MMP-9 )and occludin around the injury area. Methods One hundred and forty Sprague-Dawley male rats were divided into control group and butylphthalide group(40mg/kg,once a day). The model was made by Marmarou method. After injury 6h,12h,24h,72h and 144h,brain water content was measured to observe the changes of the brain edema,the immunohistochemistry and Western blot were used to observe the expression of MMP-9 and occludin around the injury area. Results Compared with the control group,the brain water content of the butylphthalide group was less,the content of MMP-9 around the injury area decreased significantly( P﹤0. 05 ). And the content of occludin around the injury area increased( P﹤0. 05 ). Conclusion Butylphthalide could reduce the expression of MMP-9 following traumatic brain injury,increase the expression of MMP-9,and reduce the brain edema.

    Objective To investigate the relationship between KIF14 expression and hepatocellular carcinoma( HCC ). Methods This study explored the correlation between KIF14 expression and hepatocellular carcinoma by determining KIF14 expression in HCC tissues and normal liver tissues. Real-time quantitative polymerase chain reaction was applied to test KIF14 in 50 cases of HCC tissues and 50 cases of normal liver tissues. Results Compared with the normal liver tissues,KIF14 mRNA in HCC tissues was overexpressed( P ﹤0 . 01 ). KIF14 DNA copies in HCC tissues were also significantly higher than those in normal liver tissues( P﹤0. 01 ). Conclusion KIF14 overexpression is very important in HCC tumorigenesis and may serve as a potential therapeutic target for HCC.

    Objective To explore the clinical efficacy of RevoLix 2μm continuous wave laser for treatment of glandular cystitis. Methods A total of 33 patients with glandular cystitis underwent 2μm continuous wave laser transurethrally resection. The therapeutic results were evaluated by using following variables:operative time,blood loss,indwelling catheterization and the efficacy of operation. Results All cases were accomplished successfully without serious complications. The operative time was 15-60min, mean( 36. 88 ± 11. 13 )min. The blood loss was 5 -40mL,mean( 13. 28 ± 7. 99 )mL. The mean catheterization time was(4. 31 ± 0. 97)d,range 2 -6d. After a follow-up between 3 and 24 months,27 cases cured and 6 cases improved. Conclusion 2μm continuous wave laser was a superior micro-invasive surgery method with high safety,convenient operation and infrequent complications.

    Objective To observe the effects of the regular nursing intervention to burn patients after leaving the hospital. Methods One hundred and twenty patients were randomly divided into the control group and intervention group,60 cases in each group. The control group received routine discharge instructions and a return visit. The intervention group got 1-on-1 specific nursing interventions,and their self-control ability was strengthened and the control effect was overseen. Complete and objective experimental data were expressed in the form of a questionnaire according to the comparison analysis. Results Compared with the control group( all P﹤0 . 01 ),the pass rate of the special care intervention group was better in the third and sixth month and the number of the patients who got the best in the knowledge of mastery,self-management capacity assessment and compliance behavior was also predominant obviously. Conclusion The nursing intervention through regular intensive training can improve the self-management and control capability of the burn patients,effectively improving the anti-burn scar effects of the later treatment.

    Objective To explore the method and the clinical efficacy of modified golf plate for A2 distal humeral fractures in adults. Methods We operated 12 patients with modified golf plate for distal humeral fractures of type A2 according to AO classification. Through posterior approach,the triceps were split and exposed the fractures. After reduction of the fracture,the modified golf plate was placed in the posterior-lateral column of humerus,then screws were inserted for fixation. Early mobilization to the functional restoration should be ensured. Results Twelve patients were followed up from 8 to 21 months, averaging 12. 6 months. All the fractures obtained satisfactory heal in the first stage. The patients were evaluated using the criteria of Mayo elbow performance score:excellent in 9 cases,good in 2 cases,fair in 1 case. Conclusion Modified golf plate is effective to treat distal humeral fractures of type A2.

    Objective To observe the influence of psychological intervention that was guided by Maslow hierarchy of needs theory in combination with Tongyang Lishi decoction on cervical spondylotic radiculopathy patients with depressive episode and clinical signs and symptoms. Methods One hundred and eighty cases of cervical spondylotic radiculopathy patients were divided into three groups:the control group,the treatment group 1 and the treatment group 2. Patients in control group received routine nursing, patients in the treatment group 1 were given routine nursing and psychological intervention by reference to Marlow hierarchy of needs theory,patients in the treatment group 2 received Tongyang Lishi decoction with dialectical nursing based on the treatment group 1. Depression scores(17-item Hamilton Depression Rating Scale,HAMD-17 )and symptom scores were compared before and after the intervention among three groups. Results There ware significant differences among the three groups in depression and symptom scores( P﹤0 . 05 or ﹤0 . 01 ),the treatment group 2 was better than treatment group 1 ,the treatment group 1 was better than control group. Conclusion Compared with routine nursing or using psychological intervention by reference to Maslow hierarchy of needs theory individually,psychological intervention that was guided by Maslow hierarchy of needs theory with Tongyang Lishi decoction based on dialectical nursing has a better curative effect on cervical spondylotic radiculopathy patients with depression.

    Objective To investigate the impact of the signal changes of lumbar endplate and vertebral body,namely Modic change,to the curative effect of fenestration with excision of nucleus pulposus to lumbar intervertebral disc protrusion. Methods We collected 78 patients of lumbar intervertebral disc protrusion who were treated by fenestration with excision of nucleus in our hospital,the patients were divided into two groups according to Modic changes or not on MRI of lumbar spinal,47 cases in group A,MRI showed no Modic changes,and 31 cases in group B,including 11 cases of Modic type Ⅰ changes on the endplate and vertebral body showed by MRI,17 cases of Modic type Ⅱchanges, and 3 patients of Modic type Ⅲ changes,patients' visual analogue scale( VAS)and Oswestry disability index(ODI)before operation and one year after operation were analyzed statistically. Results The preoperative VAS score and ODI score of patients in group A and B had no significant difference,and these scores were improved significantly one year after surgery. At follow-up of one year after surgery,VAS score of group A was(0. 99 ± 0. 23)scores,and the score of group B was(2. 21 ± 0. 69)scores,the score of group A was better than that of group B(P﹤0. 01);ODI score of group A was(6. 13 ± 1. 12)scores, and the score of group B was(9. 05 ± 2. 54)scores,the score of group A was better than that of group B (P﹤0. 01). At the preoperative and one year after surgery,there were no significant difference in patients with different types of Modic changes on VAS scores and ODI scores. Conclusion Fenestration with excision of nucleus pulposus was one of the effective ways of treatment to lumbar intervertebral disc protrusion. Modic changes on the endplate and vertebral body may be one of the reasons affecting the relief of lumbar pain after surgery.

    Objective To investigate the effect of glutamine on prevention and treatment of neonate upper gastrointestinal hemorrhage( UGH). Methods There were 171 neonates with risk factors for upper digestive tract hemorrhage were randomly grouped,89 cases as treatment group intravenous omeprazole and oral glutamine were given,82 cases as control group intravenous omeprazole given only. The incidence of UGH and the time of UGH between two groups were compared. Results The incidence of upper gastrointestinal bleeding in treatment group and control group showed significant difference (χ2 =3 . 917 ,P﹤0 . 05 ),gastrointestinal bleeding cases showed significant difference in hemostasia time ( t=2 . 836 ,P﹤0 . 05 ). Conclusion Oral glutamine has effect both on prevention and treatment of UGH with safety.

    Objective To compare the prognosis between sudden hearing loss patients with and without anxiety. Methods The sudden hearing loss patients were assessed according to State-Trait Anxiety Inventory( STAI). The selection involved 30 patients sudden hearing loss patients treated in our hospital with anxiety compared with 30 patients without anxiety,the treatment effect between two groups was compared. Results The treatment effect of sudden hearing loss patients with anxiety was significantly reduced compared with that of controls( P﹤0 . 05 ). Conclusion The treatment effect of sudden hearing loss patients with anxiety was significantly reduced compared with that of controls. We should strengthen psychological treatment to increase the recovery rate and the quality of life.

    Objective To compare and evaluate the diagnostic value of cervical lesion screening by two methods used in the clinical application. Methods Analysis was performed on the results of 1 350 cases of gynecological patients who were examined by two methods at the same time:line production technology smear layer type liquid thin cells and traditional pap smear method for uterine cervical cytology. Results The rate of positive cases based on the cervical liquid-based cytology was higher than that of conventional pap smear method. The difference was statistically significant(χ2 =64. 299,P ﹤0. 01). Conclusion Compared to the conventional papanicolaou smear method,cervical liquid-based cytology method has more sensitivity and accuracy,improves the detection rate and reduces false detection rate,which is a more efficient way to diagnose cervical carcinoma and precancerous lesion.

    Objective To discuss the clinical efficiency of epidural anesthesia chloroprocaine combined with different doses of fentanyl in patients undergoing anorectal surgery. Methods One hundred and twenty patients undergoing anorectal surgery were randomly divided into three groups,forty patients in each group,group A,B and C respectively received epidural anesthesia with chloroprocaine combined with fentanyl 1,2,3mg/L. The onset time of anesthesia,the disappeared time of pain sensation, the duration of pain sensation block,the duration of motor block,the degree of muscle relaxation,the visual analogue scale( VAS)scores at 5 min after surgery,the operation time,the discharge time and the incidence rate of adverse reactions were observed and recorded in three groups. Results Compared with group A,the onset time of anesthesia and the disappeared time of pain sensation in group B and group C were shorter;compared with group B,the disappeared time of pain sensation of group C was shorter. The duration of sensory block in group B and group C was longer than that of group A( P﹤0 . 05 ). The VAS score at 5 min after surgery was lower in group B and group C,group C compared with group B was lower ( P﹤0 . 05 ). The duration of motor block of group C was longer than that of group A( P﹤0 . 05 ). The operation time in group B was shorter than that in group A and group C( P ﹤0 . 05 ). Compared with group A and group B,the discharge time in group C was later(P ﹤0. 05). The incidence of adverse reactions in group C was more than that of group A( P﹤0 . 05 ). There were no statistical differences in the degree of muscle relaxation among three groups. Conclusion Epidural chloroprocaine combined with different doses of fentanyl(1mg/L,2mg/L,3mg/L)all can be used in daytime anorectal surgery safely and effectively,but the latter two groups provide the longer analgesic time than the first group and 2mg/L fentanyl is the optimum composite dose with less adverse reactions.

    Objective To evaluate the efficacy and safety of recombinant adeno-viral human p53 gene( rAd-p53 ) in combination with concurrent radio-chemotherapy in patients with unresectable stage Ⅲ non-small cell lung cancer( NSCLC). Methods Forty-seven patients with P53 protein mutant NSCLC Ⅲ were diagnosed through pathology,immunohistochemistry. All patients were treated by recombinant human p53 adenovirus injection twice through bronchial artery infusion before concurrent radio-chemotherapy 3 days,which based on tumor size every( 2 -6 )× 1012 VPs( viral particles ). Concurrent radio-chemotherapy was performed,and the average dose was 60Gy/30 fractions. EP chemotherapy was used,cisplatin was given by intravenous drip 50mg/m2 at 1,8,29 and 36 days. VP-16 was given by intravenous drip 50mg/m2 at 1 -5 and 29 -33 days. Results All patients completed treatment and evaluation of toxicities and efficacy. The overall response rates were 76. 6%. Seven patients had 3 level esophagitis( 14 . 9%),five patients had 3 level radiation pneumonitis( 10 . 6% ). Local control rate was 82. 9%(29/35)in the patients followed-up of more than 1 year. One-year overall survival rate was 72 . 3%,and one-year progression-free survival rate was 51 . 1%. Conclusion Trans-catheter bronchial arterial infusion rAd-p53 combined with concurrent radio-chemotherapy in patients with unresectable stage Ⅲ non-small cell lung cancer is a safe,effective therapy.

    Objective To study the BACTEC MGIT 960 liquid culture method and the CLSI M24-A broth microdilution method for rapid susceptibility testing the significance of non-tuberculous mycobacteria. Methods The BACTEC MGIT 960 liquid culture broth microdilution method combined rapid susceptibility testing,and L-J solid culture method combined the absolute concentration susceptibility method were respectively used in 48 strains with nontuberculous mycobacteri,the culture time and sensitivity reporting period of two combination methods were compared. Results The average positive report time in 48 strains of nontuberculous mycobacteria culture with BACTEC MGIT 960 instrument was(7. 0 ± 5. 3)d,while that of L-J media was(17. 5 ± 10. 2)d,in comparison the results of two methods were significantly different( P ﹤0 . 05 ),the reported time of broth microdilution France results was 7 -14 d,the report time of L-J solid susceptibility test was 28d,the comparison showed significant difference,two kinds of susceptibility testing methods for the same type of drug resistance rate analysis showed no significant difference( P﹥0 . 05 ),broth microdilution method can detect more types of drugs than the L-J method. Conclusion BACTEC MGIT 960 liquid culture broth microdilution method combined rapid susceptibility testing has obvious advantages over L-J solid culture method and the absolute concentration susceptibility method in the incubation time and susceptibility results reporting time types.
