Biomechanical experiment of a new suture fixation method for tibial eminence fracture

摘要: 目的  探讨缝线网状套扎固定法治疗胫骨髁间嵴骨折的生物力学性能。
方法  取新鲜猪膝关节标本24个,制备Ⅲ型胫骨髁间嵴骨折模型,采用电脑随机方法分为缝线网状套扎固定组和缝线领带结套扎固定组,每组12个标本。分别进行疲劳试验和拔出试验,比较2组最大失败载荷、失效载荷、拔出刚度及循环位移。
结果  失败模式:A组位移超限12例,B组位移超限7例、骨块翘起3例,骨块旋转2例。生物力学显示,缝线网状套扎固定组最大失败载荷、失效载荷高于缝线领带结套扎固定组,循环位移低于缝线领带结套扎固定组,差异有统计学意义(P<0.05);2组拔出刚度差异无统计学意义(P>0.05)。
结论  缝线网状套扎固定Ⅲ型胫骨髁间嵴撕脱骨折的生物力学性能可能优于缝线领带结套扎固定法。

Abstract: Objective  To study the biomechanical stability of reticular loop ligature fixation for tibial eminence fractures.
Methods  A total of 24 fresh porcine knee joints were involved in this experiment. After type Ⅲ fracture model was established in uniform size, the 24 samples were divided into reticular loop ligature group(group A, n=12) and neck knot loop group(group B, n=12) by computer random method. Each specimen was subjected to cyclic test and pull-out test. The maximum failure load, yield load, stiffness and displacement were compared between two groups.
Results  The failure mode: Twelve cases in group A had displacement exceeding the predetermined limit. In group B, 7 specimens failed as a result of displacement exceeding the predetermined limit, 3 cases failed because of bone tilted up, and 2 cases experienced failure for bone rotated. The biomechanical results showed that the maximum failure load and yield load of group A were higher than those of group B, and the average displacement after cycle loads of group A was lower than that of group B, suggesting significant differences between two groups(P<0.05). The difference of average stiffness value between two groups had no statistically significance(P>0.05). 
Conclusion  The biomechanical property of reticular loop ligature fixation for type Ⅲ tibial eminence fracture may be superior to that of neckwear knot loop ligature fixation.
