[Abstract]The incidence of patients with diabetic dyslipidemia is very high ,so it is beneficial for their long-term prognosis to control dyslipidemia in patients with diabetes mellitus. With regard to diabetes combined with dyslipidemia, low-density lipoprotein cholesterol is still the primary target of lipid-lowering therapy. But apolipoprotein B is another new important target for lipid-lowering treatments in the latest guidelines, and a lower low-density lipoprotein cholesterol concentration is set for diabetic patients with the very high-risk cardiovascular risk. Statin is still the cornerstone in lipid-lowering therapy. Not only ezetimibe but, lately, proprotein convertase subtilisin/kexin type 9 inhibitors could be used as combination drugs, if the targets cannot achieve with statins alone.

[Abstract] ObjectiveTo investigate the effects of Sacubitril/Valsartan(Sac/Val) on cardiac function in patients with acute anterior wall ST-segment myocardial infarction(STEMI) complicated with heart failure with reduced ejection fraction(HFrEF).
〖WTHZ〗MethodsIn a prospective, randomized controlled study, 160 consecutive cases of acute anterior wall STEMI patients complicated with HFrEF after PCI were enrolled, and divided into the observation group(n=80) and the control group(n=80) using a randomized digital method. At one week after PCI, the control group received routine treatment and Valsartan(Val), while the observation group was given Sac/Val in addition to routine treatment, for a duration of 6 months treatment. Changes of plasma NT-proBNP concentration and echocardiography parameters were compared between two groups. The effect of Sac/Val in acute anterior STEMI patients with HFrEF after PCI was evaluated.
〖WTHZ〗ResultsAfter 6 months of follow-up, NT-proBNP, LVEF and WMSI were significantly improved compared with baseline in two groups(P<0.001). However, the above indicators improved more significantly in the observation group. At the end of the follow-up, the left ventricular end systolic volume (LVESV) decreased significantly, and the major adverse cardiac events(MACE) were significantly reduced in the observation group(P<0.05).
〖WTHZ〗ConclusionSac/Val can improve ventricular wall dyspraxia and improved left cardiac function and reduced MACE within 6 months after PCI in patients with acute anterior STEMI complicated with HFrEF.

[Abstract]ObjectiveTo investigate the correlation between Helicobacter pylori(H.pylori) infection and the expression of cell cycle regulator p27 protein in gastric cancer tissues in order to understand the pathogenesis of gastric cancer caused by H.pylori infection.
〖WTHZ〗MethodsSixty-two specimens of gastric cancer tissues undergoing gastric cancer resection were selected. The 14C-urease breath test was used to detect H.pylori infection in the specimens, and the immunohistochemical staining method was applied to measure the expression level of p27 protein, so as to analyze the correlation between H.pylori infection and p27 expression in gastric cancer patients.
〖WTHZ〗Results Forty cases in 62 patients with gastric cancer were infected by H.pylori(64.52%). The expression rate of p27 protein in gastric cancer tissues was closely related to the degree of tumor differentiation(P<0.01), there was no correlation between the expression rate of p27 protein and the gender, age, tumor size, and TNM stage, tumor invasion depth and lymph node metastasis(P>0.05). H.pylori infection was negatively correlated with p27 protein expression in gastric cancer tissues(P<0.01). Logistics analysis showed the degree of tumor differentiation was the main factor of positive expression of p27 protein in cancer tissues(P<0.01). Expression of p27 in gastric cancer tissues was lower than in paracaner tissues(P<0.01).
〖WTHZ〗ConclusionThere is a certain correlation between H.pylori infection and the expression level of p27 protein in gastric cancer patients. H.pylori infection may cause gastric mucosal canceration by reducing the expression level of p27 protein, and p27 protein can be used for the clinical diagnosis of gastric cancer and the determination of whether it is associated with H.pylori infection. p27 protein is also an important indicator to the later diagnosis, treatment and prognosis evaluation of gastric cancer.

[Abstract] ObjectiveTo investigate the effects of hypertensive disorder complicating hypertension(HDCP) on neonatal coagulation, neuron specific enolase(NSE) and procalcitonin(PCT) levels.
〖WTHZ〗MethodsA total of 592 neonates born from HDCP patients admitted to our intensive care unit(ICU) were enrolled in the study. The newborns were divided into three groups according to the serious condition of the maternal condition: group A(112 cases with severe pre-eclampsia), group B(197 cases with mild pre-eclampsia), group C(HDCP, 283 cases), 500 cases of neonates born from women who did not have HCDP at the same time were selected as the control group. The coagulation function indexes[prothrombin time(PT), activated partial thromboplastin time(APTT), fibrinogen(FIB), D dimer(D-D), platelet count(PLT), fibrinogen degradation product(FDP)], NSE and PCT levels of each group were measured at 2 h after birth, and comparison between groups was performed. Spearman correlation method was used to analyze the correlation between each index and the severity of HDCP.
〖WTHZ〗Results The differences of the indexes of A, B, C and the control group were statistically significant(P<0.01). Correlation analysis showed that the levels of PT, APTT, D-D, FDP, PCT and NSE were positively correlated with the severity of HDCP, while the levels of FIB and PLT were negatively correlated with the severity of HDCP(P<0.05 or P<0.01).
〖WTHZ〗ConclusionPregnancy-induced hypertension can easily lead to neonatal coagulopathy, which makes neonatal blood in a state of hypocoagulability and secondary fibrinolysis, and increases the risk of neonatal brain injury and infection. The more severe the maternal condition, the more likely the neonatal will cause coagulopathy and brain damage.

[Abstract] ObjectiveTo analyze the pathogen distribution and antimicrobial resistance in children with acute otitis media in Shijiazhuang area, and to provide evidence for clinical anti-infection treatment.
〖WTHZ〗MethodsThe clinical data of 291 children with acute otitis media were analyzed, the middle ear secretions were collected for culture. The strains were identified by biomerieux VITEK MS, and the antimicrobial sensitivity was detected by biomerieux VITEK2 Compact, BD Phoenix100 and K-B method.
〖WTHZ〗ResultsA total of 211 positive pathogen specimens were isolated from the purulent secretions in 291 samples with acute otitis media, and the positive rate was 72.5%, Among which 157 gram-positive strains were detected, the detection rate was 54.0%,and accounting for 74.4% of the total positive strains. The most common was streptococcus pneumonia(68), the detection rate and accounting for the total positive strains was 23.4% and 32.2% respectively. The second was staphylococcus aureus(62), the detection rate and accounting for the total positive strains was 21.3% and 29.4% respectively. The 42 strains of gram-negative bacilli accounting for 19.9% of the total positive strains were detected, the detection rate was 14.4%, in which the most common was haemophilus influenzae(28), with a detection rate of 9.6%, accounting for 13.3% of the total positive strains. There were statistically significant differences in the detection rates of pathogen in the four age groups of 0-1 year old, >1-3 years old, >3-6 years old and >6 years old(P<0.05), and the highest positive detection rate at 0-1 year old, however the lowest positive detection rate at >3-6 years old. According to the time of occurrence, seasonal distribution had statistical differences(P<0.05), with the highest detection rate in the fourth quarter and the lowest in the second quarter. The 68 strains of streptococcus pneumoniae from ear secretions were completely sensitive to Vancomycin and Linezolid, and completely resistant to Erythromycin, meanwhile the 62 strains of staphylococcus aureus were completely sensitive to Levofloxacin, Rifampicin, Gentamicin, Moxifloxacin, Vancomycin and Linezolid, and completely resistant to Erythromycin, Clindamycin and Penicillin, 34 strains of Methicillin-resistant staphylococcus aureus(MRSA) were found in Staphylococcus aureus, accounting for 54.8%. There were 28 strains of haemophilus influenzae, which were completely sensitive to Ceftazidine, Cefepime, Meropenem, Imipenem, Aztreonam and Ciprofloxacin, and higher resistance to cotrimoxazole.
〖WTHZ〗ConclusionStreptococcus pneumoniae, Staphylococcus aureus and haemophilus influenzae are the main pathogens of children′s acute otitis media in Shijiazhuang area. Many kinds of drugs commonly used in clinic are highly sensitive to them. In clinical, we can select reasonable antibiotics according to the results of bacterial culture and drug sensitivity test.
